Community influencers achieve remarkable results: 100% of refusal children vaccinated in Ganyan district In the south-east region, community influencers - namely mullahs - are used in the absence of social mobilisers. 77 influencers are working in 15 districts across four provinces: Paktia, Khost, Ghanzi and Paktika. Their objective is to address the concerns of 9,479 chronic and new refusal families.

Before each campaign, they conduct meetings with community elders, make announcements in mosques, and share information on the benefits of polio vaccination. During the campaign, they visit refusal families.

The results are remarkable. In Ganyan district, the influencers managed to vaccinate 100% of refusal families: 158 children. Across the remaining 14 districts, 2,953 children have been vaccinated that would have otherwise been refused the vaccine.

• One new wild poliovirus (WPV1) case was reported in June from Nad-e-Ali district of Helmand province.

• 3 WPV1 positive environmental samples were reported in June: 2 from Kandahar, and 1 from Nangarhar, bringing the total number of positive samples to 22 in 2018.

POLIO TRANSMISSION

• 160 acute flaccid paralysis (AFP) cases (75 girls and 85 boys) reported in June. In 2018, 1,642 AFP cases have been reported, of which 1,478 have been discarded as “non-polio AFP” and 155 cases are pending classification.

• Non-polio AFP rate is 16, stool adequacy above 95%, and non-polio enterovirus rate above 15% at national level. In all regions, non-polio AFP rate is above 12, stool adequacy above 89%, and non-polio enterovirus rate is ranging from 12 -20 %.

AFP AND ENVIRONMENTAL SURVEILLANCE

• Between 4-8 June, a nomad-specific campaign vaccinated 53,872 children (under 10 years) in the southeastern region and a recovery campaign in Bermal district of Paktika province vaccinated 27,402 children (under 5).

• Case Response campaign on 6-10 June in Shahwalikot district of Kandahar vaccinated 27,575 children under five.

SUPPLEMENTARY IMMUNIZATION ACTIVITIES

• 15 cross-border vaccination points, 42 cross-border teams and 445 permanent transit teams (PTTs) were operational across Afghanistan during June 2018.

• Permanent transit teams vaccinated 1,164,676 children and cross-border teams vaccinated 74,397 children in June.

• A total of 1,234 children under age of 10 were vaccinated with OPV and 433 with IPV by teams in UNHCR repatriation centers and IOM sites receiving returnees and refugees from Pakistan and Iran. From 1st Jan to date, 10,304 returnee children have received OPV and 6,277 IPV.

• In compliance with International Health Regulations (IHR), 9,351 international travelers were vaccinated during June.

COMPLEMENTARY VACCINATION ACTIVITIES

Polio-Free Afghanistan

A qualitative study to find out the reasons behind refusals has expanded to the south-east and east. These two regions are known to have a high number of refusals, resulting in children who miss the vaccine. This follows on from successful studies in the south, which shows misconceptions that the vaccine is an “agenda by the Americans” and/or it makes their children sick and behave badly. The findings will be used to re-formulate specific strategies that will reduce refusal families and ensure more children in high-risk areas are vaccinated.
