Wounds/Excoriations 1. Ishii, Satoko- MASD to the buttocks area, with slight excoriation to the right buttocks 2. Watanabe, Delbert- excoriation to the right buttocks, skin tear to the left forearm 3. Ota, Minoru- inner thigh skin irritation and reddish scrotum area, skin tear to right foot 4. Lee Lai, Kitman- scab to the coccyx area 5. Inouye, Janet- excoriation to the buttocks area 6. Chow, Linda- Stage IV pressure ulcer to the coccyx area, measuring 5cm x5cm with undermining at 5 and 9 o’clock. 7. Sin, Chi Cheung- skin tear to the right wrist and left elbow 8. Ching, Beatrice- skin tear to right shoulder and knee 9. Shimizu- skin tear to right leg 10. Ebuchi- GT site dressing 11. Okuda- mid abdominal surgical wound x/p hysterectomy and salphingo oophorectomy 12. Isara- dermatitis under the breast area 13. Chun- admitted with gangrenous toe 14. Matsuki- Left forehead skin opening and skin tear to the right foot 15. Akiyoshi- dehiscence to the coccyx area
