DESCRIPTION
A variety of psychotherapies for depression have received substantial empirical support but the rates of recovery are still unsufficient and relapse is all too common. Research on cognitive conflicts (see e.g., http://onlinelibrary.wiley.com/enhanced/doi/10.1111/bjc.12050/) has shown that personal dilemmas can be found in over 90% of all patients with depression but current psychotherapies do not focus on these dilemmas at least in a direct way. Dilemma-Focused Therapy (DFT) is a brief intervention which can complement other therapies by targeting on personal dilemmas, an issue which is usually not dealt with but which could explain much of the reluctance to change and relapse that occurs in the course of the treatment for depression.DFT is constructivist-based intervention derived from personal construct theory which has already been supported in an RCT. It uses the Repertory Grid Technique (RGT) for the study of a person’s meaning system. One type of conflict which can be detected with the RGT is implicative dilemma: a desired change on one construct (e.g. “sad-happy”) has an undesired implication on another construct (e.g. “cares for others vs. selfish”). Thus, achieving change (e.g., becoming happy) implies an undesirable change in self-identity (e.g., becoming selfish). Instead of focusing only in symptoms or negative thoughts, DFT is oriented to help clients recognize both sides of the dilemma: the need for change and the need for continuity and for protecting the sense of identity. By empowering clients to deal with their internal conflicts they can resolve their personal dilemmas and govern their lives in a more integrated way.TRANSCRIPT

Varias psicoterapias para la depresin cuentan con buen apoyo emprico pero aun as la
proporcin de pacientes plenamente recuperados es baja y las recadas son muy frecuentes.
Las investigaciones sobre los conflictos cognitivos han encontrado dilemas personales en
alrededor del 90% de los pacientes con depresin. Sin embargo, las psicoterapias no prestan
atencin en dicho dilemas, al menos de forma directa. La Terapia Centrada en Dilemas* (TCD)
consiste en una intervencin relativamente breve (8 sesiones) que puede complementar otras
terapias. Como su nombre indica, esta intervencin se dirige a resolver los dilemas
encontrados en cada paciente, un aspecto que las otras terapias no suelen considerar como
objetivo teraputico. Estos conflictos pueden explicar muchas de las reticencias al cambio y las
recurrencias que tan frecuentemente ocurren en el curso del tratamiento de la depresin.
La TCD es una intervencin de base constructivista derivada de la Teora de los Constructos
Personales que ha recibido ya apoyo emprico con un ensayo clnico controlado. Utiliza la
Tcnica de Rejilla para el estudio sistemtico del sistema de significados de una persona. Una
modalidad de conflicto que puede detectarse con esta tcnica son los dilemas implicativos: un
cambio que la persona desea realizar en un constructo (p. ej., triste-feliz) tiene una
implicacin en otro constructo (p. ej., se preocupa por los dems-egosta) de forma que
cambiar en el primer constructo implica tambin un cambio no deseado en el segundo
(convertirse en egosta). As, el cambio en aspectos sintomticos, como el estado de nimo,
puede implicar cambios no deseados en la identidad personal. En lugar de centrarse solo en los
sntomas o pensamiento negativos, la TCD se orienta a ayudar a los clientes a reconocer ambos
lados del dilema: la necesidad del cambio y la necesidad de continuidad y de proteger el
sentido de identidad personal. Al apoderar a los clientes para manejar sus conflictos internos
se resuelven sus dilemas y gobiernan sus vidas de una forma ms integrada y saludable.
