Swissness committed to quality
When, in 1946, Robert Mathys, a pioneering mechanic working in a small
workshop, developed instruments from special acid-resistant stainless-
steel, he already had in mind a premium Swiss quality product. Nobody
at the time could have imagined that his chance encounter in 1958 with
young and inspired Swiss orthopaedic surgeons would, a few years later,
trigger a global revolution in fracture treatment.

5WELL CONNECTED WITH PHYSICIANSToday, Mathys focuses on the development, manu-facturing and distribution of a wide range of im-plants for hips, knees, shoulders, bone graft substi-tutes and associated instruments. The formerexchange of ideas with scientific experts has be-come the legally and financially independent RMSFoundation, (Robert Mathys Stiftung), a service lab-oratory and research institute offering advisoryand testing services to customers from all over theworld and playing an important role on an interna-tional scale. “At the RMS, under the direction of mybrother Robert Mathys, we conduct basic and ap-
plied research”, states Hugo Mathys, the dynamicChairman of the Board for the Mathys Group andone of two second generation members who joinedthe company in the 1970s. “We are co-operatingwith scientific institutions on a global scale, the RMS
giving grants to renowned universities to undertake
investigations on our behalf. It’s clear that we must
work together to move forward!”
STOP THE SHORT-TERM WAY OF THINKING!The Mathys team is fully motivated: “Since we livein a welfare state in a high-wage country, we have to
create products of the highest quality and high valu e
added along the entire value chain in order to pro-
vide new opportunities for growth and employ-
ment.” The profound changes in society and econ-omy pose a considerable challenge to the medtechbranch. People are growing older but, in contrastwith the prevailing attitude in the past, they wantto stay fit and healthy for longer. They have clearideas about what they want and place greater de-mands on their health care service providers. Hospi-tals are now seeing a new type of patient, veryyoung individuals who suffer an injury while prac-tising high-risks sports. “In earlier times, a joint replacement was expected to last about 10-20 years.
But nowadays it has to provide guaranteed high-
quality service for 30-40 years in order to deliver sig-
nificant socio-economic benefit.” That’s why there isno room for the short-term thinking that increas-

ingly prevails in the hospital environment. “All thatmatters is next year's budget”, says Hugo Mathyswith a shake of the head. “All reflections revolvearound the question ‘Where can I find the lowest
price for a joint replacement? Which manufacturer
will give me the biggest discount?`This horse-trading
endangers product quality and means that patients
may not receive the best individual solution.” If pa-tients have to undergo follow-up operations after10-20 years instead of 30-40 years for the simplereason that the second best solution had been im-planted in an expensive surgical intervention, thisis a misjudgement from the macroeconomic pointof view. The popular saying ‘greed is good’ certainlyhas no place where human beings are involved.
Hugo Mathys is an entrepreneur with heartand soul, and with a good feeling for the needs ofthe market, and with respect to ethics. “As an engi-neer, I have to abide by the principle that each med-
ical device has to perform its function to the highest
standard”, he comments. But the responsibility restsequally on the shoulders of each physician andeveryone working in the hospital system. “No com-promises are possible when it comes to product qual-
ity!” But Mathys also has to adapt to the economicneeds of a harsh market environment. “We do so bytrying to simplify products and solutions without
compromising on functionality and quality.”
INNOVATION IS A STATE OF MINDAs far as innovation is concerned, the Mathys peo-ple come up with plenty of creative ideas. A fewyears ago, they started using the ‘innovation fun-nel’. Project ideas, product developments and newmaterial technologies are poured into the funnel,reviewed in group discussions and eventually themost promising ones are selected. “Our develop-ment team is a sophisticated mixture of experienced
orthopaedic surgeons, open-minded designers, ma-
terials experts, and – very importantly – critical spir-
its!” explains Hugo Mathys. “It’s not a playgroundfor yes-men, as such people do not take us any fur-
ther forward!” What he considers important is the

He at-taches great importance to sound education andtraining. “Our collaborators have a high level of ex-pertise along the complete value chain.” A good ex-ample of one innovation is the recently developedartificial hip cup which uses a crosslinked polyeth-ylene enriched with vitamin E to increase its dura-bility and longevity.

Eucomed is in permanent contact with Europeanregulators, politicians and other policymakers. To what extent can Eucomed exercise influenceover the formulation of the laws and policies affecting the medical technology branch? Canyou cite an example of a successful influence? Luciano Cattani: Eucomed engages with Europeanregulators, politicians and other policymakers todevelop and propose patient-centred policies thatenable people to live healthy and productive livesand provide solutions that significantly drive theproductivity and efficiency of healthcare systems.
In the pharma branch, the restrictions and approvals are increasingly time and money
consuming. Is this trend also evident in medical technology? Medtech and pharma are inherently different ani-mals. A centralized regulatory approach does notwork for medical technology. While health systemspurchase nearly 16,000 medicines, they requireover thirty times that number of medical devices.We must always bear in mind that unlike medicinalproducts, which represent one branch of science,medical devices represent a vast range of sciencesand technologies. Atomic physics, anatomy, bio-engineering, mathematics, engineering, materialsscience, radiology, fluid dynamics, thermodynam-ics, acoustics, computer science, electronics, micro-electronics. The list goes on and on and is only lim-

ited by doctors’ and scientists’ capacity to inventand innovate.
Patient safety is paramount and the EU hasstrict regulations for placing technology on themarket. We are hopeful that the future of EU med-ical device regulation will strike a mutually accept-able balance with patient safety rightfully at itscore – a balance that allows for increased safetywhile safeguarding innovation. As we see it, there isno ‘choice’ to be made between safety and fast ac-cess – you cannot choose access over safety – it isabout ‘combining’ safety and fast access.
Eucomed engages with over 65 expert groups dealing with regulatory, economic, internationaland legal affairs. How exactly should we interprettheir help especially for SMEs? Can you give us an example of effective and efficient support? In 2010 Eucomed launched an SME Task Force in aneffort to understand the needs of medical technol-ogy SMEs, which make up some 80% of themedtech sector in Europe. SMEs are also repre-sented in most of Eucomed’s working groupsthrough their national associations. We’ve surveyedmore than 7,000 SMEs, who have told us that smallbut powerful changes would reap rich rewards forEurope’s population, healthcare systems and eco-nomic development. We are working together withthe European Commission to make those small butpowerful changes to remove the big barriers thathamper the innovation potential of those SMEs.
A few years ago, doctors made the decisionsabout the products to develop in medical techno-logy. Today, the common law of business dictatesthe products to develop. Will we need legal
experts and lawyers instead of engineers in themedtech industry of the future? For the medical technology industry, collaborationwith healthcare professionals (HCPs) is an absolutemust. HCPs are the primary users of new technol-ogy and play an instrumental role in the successfuldiffusion of innovative medical devices throughoutEurope. In fact, HCPs often bring those ideas to thetable that lead to the development of new devicesand inspire continuous incremental improvementsto existing technology.
Industry also engages with patient groupswith the goal of improving the safety and usabilityof medical devices. We constantly strive to improvehealthcare delivery, creating tools which can helpminimize the chances of adverse events such ashealthcare associated infections (HCAIs). Further-more, by designing and developing easy-to-usemedical devices and procedures, the industry ishelping to reduce risk and increase quality of care.
Patients also have a role to play in improvingtheir own safety. Empowered patients, with im-proved access to information and patient training,can play an active role in managing their ownsafety. The industry seeks enhanced cooperationwith patients, to better understand the risks thatlead to adverse events, and to develop improved, pa-tient-centred solutions.
Because of a negative trade balance and nakedprotectionism more and more imports in coun-tries such as Brazil are subject to state restrictions.Do you think Eucomed can intervene in a meaningful way? In fact, the trade balance for medtech in the EU ispositive, as the graph below shows.

Eucomed promotes the free trade of medical tech-nology products that are manufactured to globallyrecognized high standards of safety. Tariffs act as abarrier to trade and restrict the types and quality oftreatments available to citizens. They also distortmarkets and act as an impediment to local suppli-ers wishing to become globally competitive by creating artificially benign business conditions inlocal markets.
Today the returns in the pharma sector have decreased to such an extent that business is nolonger cost-effective and companies cease to deliver their products to certain countries. Do you fear a similar development for the medtechindustry? Late payments in the public sector threaten thelivelihood of Europe's medical technology industry.Recognizing this, the European Union adopted a revised EU legal framework legislation in January2011 which introduced a limitation of paymentterms for public authorities to 30 days, withhealthcare providers being exempt up to 60 days.The European Directive needs to be transposedinto national law by March 2013 at the latest.
More than two years later, the situation hasnot seen much improvement. The total outstand-ing debt amount has remained largely the same,with about Euro 11 bn of overdue payments. The pay-ment term, however, has grown even longer.

12 To date, new business models for a rapidly trans-forming healthcare environment have rarely sur-faced. Since 2009, solution bundling (i.e. technologypaired with services, disposable goods, training, ITsupport, etc.) has been the main focus in overcom-ing the “triple challenges” to margins: price erosion,cost pressure and intensified competition, particu-larly from commodity players. In parallel, manage-ment is faced with increasingly tighter regulatoryand market access hurdles on a global scale, push-ing the innovation hurdles ever higher.

13Little thought is given to latent client /marketneeds that can be addressed by players combiningleading-edge technologies in a unique way. As inthe past, medtech companies strive to improve op-erational processes and try hard to expand intonew markets or segments, overcoming regulatoryhurdles. Established players often focus on improv-ing profitability through optimization in market-ing, sales and distribution. However, the interna-tionalization of competition intensifies in parallelwith growing pressures on prices from healthcarepurchasers, resulting in a squeeze on the globalmedtech industry profit pool. Unfortunately, the in-dustry complains increasingly about the diminish-ing availability of next-generation managementskills and expertise, making access to talent andknow-how a game-winning discipline to secure fu-ture success.
WHERE TO PLAY AND HOW TO WINWhen planning to propel business forward into thenext decade, one has to review two fundamentalaspects: where to play and how to win. Given the in-creasing innovation hurdles, medtech companieshave to consider the options for exploiting all possi-ble business areas, including high-volume price-sensitive segments. Emerging markets offer thebest absolute growth opportunities, sometimeswith radically redesigned products, as evidenced bythe GE MAC 400, “Making ECG available to everyphysician, every patient, everywhere!”, including inrural India. Diversification across healthcare hasbeen an option for a while, effectively exploited bythe Fresenius Group with its integrated dialysisbusiness Fresenius Medical Care, the hospitalgroup Helios and VAMED, operating in the projectand management business of health care facilitiesworldwide.
Medtech companies have exploited strate-gic value chain configuration for cost reasons for awhile, particularly in manufacturing. The innova-tion challenge has now also led to value chainadaptations in R&D, such as open source ap-
proaches. Commercial models have increasinglybeen adapted from traditional product/sales force-centered commercial models targeted at surgeons/physicians to healthcare stakeholder-centered ap-proaches, as evidenced by the strengthening of KeyAccount Management capabilities, as exemplifiedby the OneMedtronic initiative. Most current inspi-rations stem from the flexibility/close collaborationbetween medtech firms and physicians on the jointdesign and development of new products tailoredto their needs. The most exciting future develop-ments might increasingly emerge at the interfacesof technical capabilities. Thus, the future flash of in-spiration appears to require highly heterogeneousteams.

FACTS AND FIGURES FORM THE BASISMore than ever, the umbrella association emergedfrom the Swiss suppliers of medical products andthe Swiss manufacturers for medical and hospitalequipment has to pull out all the stops to ensurethe best possible conditions for an unhindered mar-ket access of the medtech industry, especially its250 member companies.
It is understandable that the problems oc-curring in Europe 2011 with hip implants and the irresponsible production of breast implants haveprovoked insecurity in a wide range of medical prac-titioners and patients: The call for a state approvalsystem – a pre-market approval – is growing louder.But Dr Melchior Buchs, General Secretary FASMEDwarns against throwing out the baby with the bath-water: “In contrast to medication, medical devices arenot subject to authorization. Responsibility for con-
formity rests with the distributor who has to prove it
with the CE marking.” To turn one's back on a triedand trusted system of many years would imply thatnew product innovations had a longer lead-time. Bytaking over the ‘New and Global Approach’, Switzer-land has harmonized the placing of medical devices on the market with the European system.“More important than a pre-market approval are a
more intensive market surveillance and test criteria
for a CE labelling adapted to the increasing require-
ments”, comments Nicolas Markwalder, FASMEDPresident. “In my opinion, we also need internation-ally a stricter qualification and supervision of the
review bodies.”
An important contribution to quality assur-ance is the new Swiss Implant Register SIRIS put inplace by FASMED and other parties from health care.From September 2012 onwards, all knee and hipjoints implanted in Switzerland will be automati-cally registered in SIRIS. “There are about 22’000 hipand 17’000 knee prostheses manufactured by 10
companies and implanted by about 400 surgeons in
180 hospitals”, specifies the General Secretary.“Thanks to this digital system, implants can now be
traced to each patient. This creates an early warning
system in the event of any damage. From the data
collected it is possible to consider the quality of the
products used and to compare the treatment out-
come between different hospitals.”

But in thisfield, different aspects have to be taken into consid-eration like the differences in purchasing power.We also must bear in mind whether individualproducts or whole service bundles are in questionand whether supplementary services are includedin the price.
An effective instrument seems to be parallelimports. Since 1996, CE-certified medical devicescan be imported into Switzerland and, since 2002,products certified in Switzerland may be exportedto the EU and EFTA countries. FASMED agrees withthe free transfer of goods as it is the decisive suc-cess factor for the Swiss medtech branch. “But theincreasing political demand to encourage parallel
imports may be detrimental to the patient”, pointsout Melchior Buchs. “A possible consequence may bethe reduction of patient safety: the importer is not en-
trusted with responsibility as the supplier to the mar-
ket is today. Also the quality of treatment could be en-
dangered as the services would disappear when

Reinhard Straumann was the offspring of a watchmaking family, always
brimming with innovative ideas for novel alloys for watches. The break-
through came when he developed new alloys with properties such
as resistance to fatigue and corrosion, while remaining non-magnetic.

THAT’S WHERE THE SHOE PINCHESStraumann is a citizen of the world, employing ap-proximately 2450 people worldwide and offeringits products and services in over 70 countries. “Sincethe cultural differences can be significant, we have

manufacturer tries to minimize the variations in
parts in serial production in order to cope with the

Serial production –customized production – digital products: It’s atough nut to crack, as they all have different coststructures. It’s true that, after the economic down-turn in 2009, the annual growth rates are still at 2 –6%. But the pressure on margins is intensifying; thepressure on competition is ever greater, especially inthe upper segment.

Professor Langhans is the Head of the Departmentof Health Sciences and Technology, created in Janu-ary 2012 at ETH Zurich (Swiss Federal Institute ofTechnology, Zurich), and which combines teachingand research in health sciences and medical engi-neering. Natural scientists and engineers cooperatein four specialized areas: Food and Nutrition, Med-ical Engineering, Movement Sciences and Sports,and Neurosciences.
LEVERAGING SYNERGIES – BUNDLING FORCESGlobal developments like the high rates of popula-tion growth, global warming, the consumption ofnon-renewable resources and demographic changespose challenges to present-day society and ourhealth care system that require an all-inclusive ap-proach. The complexity of the forthcoming tasksand problems requires integrated multidisciplinaryresearch. That’s why ETH Zurich combines naturaland engineering sciences, bringing togetherstrengths in the biomedical domain – from the mo-lecular to the systemic level – with medical technol-ogy, neurosciences and public health, to develop syn-ergies and common potential. The wide range ofskills available in the institutions makes this interdis-ciplinary approach possible, which is a prerequisitefor integrated solutions. Close co-operations takeplace with scientific colleagues, for example, withthe Paul Scherrer Institute, the University of Zurichand University Hospital Zurich. But other research in-stitutions are also particularly valuable for the newdepartment, like the World Food System Center(worldfoodsystem.ethz.ch) or NCCR Neuro, the Neu-ral Plasticity and Repair National Center of Compe-
tence in Research, (nccr-neuro.ethz.ch), a research in-strument of the Swiss National Science Foundation.
The focus lies on technology transfer fromthe research laboratories to the private sector. “Wedo not carry out specific industrial research, but we
open our laboratories to interested parties from in-
dustry and show them what is technically possible
and economically sensible, and then we find tailor-
made solutions.” The collaboration with hospitals isgranted priority status, as industry can only adoptnew technologies that have been practically testedin hospital life.
ON THE ROAD TO SUCCESS WITH A ‘USP’“There is nothing comparable at international aca-
demic level that features the combination of these
four areas”, says an enthusiastic Wolfgang Lang-hans, whose key research interests are the physiol-ogy and pathophysiology of the neuroendocrinecontrol of eating and energy balance. This includesthe most common disabilities in the industrialworld, obesity and type-2 diabetes, the causes ofwhich are still uncertain. “Zurich is an innovationplatform, as we have a leading Technical University,
a renowned University Hospital and an outstanding
Medical Faculty.” He relates how, following the deci-sion of ETH Zurich, the researchers from the variousinstitutes came together to get to know each otherand, finally, to start a dialog on benchmarking. “Weall immediately gained the impression of a real en-
richment and felt that we can get things moving
much more efficiently when working together than
as lone fighters”, remembers Langhans. All in thesame boat, they defined three preliminary projects,

new ‘Health Sciences and Technology’ study course in 2011, 175 students
registered for the Bachelor's program. In 2012, 270 students have

decided to pursue two of them further and to im-plement the third. This specific project aims toidentify and collect information on how our brainfunction is influenced by eating and physical activ-ity, and vice versa. It’s the USP, the ‘Unique SellingProposition’ at the new department with the high-est potential for success.
Since the institutes are currently at variouslocations in and around Zurich, the exchange of in-formation plays a major role. This is why seminarsare organized on a regular basis, during which grad-uate students in their 2nd or 3rd academic year of aPhD present their latest research results and put for-ward good ideas for identifying common projects.
QUALIFIED SPECIALISTS FOR THE MEDTECH INDUSTRYThe Health Sciences and Technology Study Pro-gram communicates basic knowledge about thehuman systems from the whole body perspectiveto the cellular and molecular levels, while integrat-ing fundamental concepts of technical sciences.These topics serve to fuel an innovative educa-tional course dealing with the problem area ‘hu-man being – health – technology’.

young people to the new Health Sciences and Tech-
nology Study Program was a big surprise to the initia-
tors. They were delighted, but also concerned about
whether they had a sufficiently developed infrastruc-
ture to cope with all these students.“Wolfgang Lang-hans is particularly happy about the fact that, in
two thirds of all cases, women are interested in en-rolling in the new bachelor's program. “This meansthat the combination of health-related topics with
engineering sciences is very attractive across gender
lines and seems to be able to tempt women into en-
gineering sciences where, in general, women are few
and far between.”
In 2016, the scientists will move closer to-gether when the new Medtech Center in Glorias-trasse – a building just a stone’s throw away fromZurich University Hospital – is operational andready to house the currently scattered communityunder one roof.
The Center of Attention (CoA) – a platform for an informal andinterdisciplinary exchange between research institutes atclinics, universities, technical colleges and the medical techno-logy industry – will be the mainmeeting point at the fair.

gather them up!” The chemist mentions the exam-ple of the inventor who wondered how to dry inter-nal wounds. He had an idea, went to the nearest supermarket for building materials, bought asponge and designed a prototype. The conceptworked, the end result was a simple but efficientdrainage system for internal wounds that is now inuse around the world. “The challenge is not to findbrilliant ideas, but to turn them rapidly into sustain-
able market successes and a broader product portfo-
lio.” This is evident when you look at the number ofpatents published by the European Patent Office,where medical technology remains at the forefront.
Give free rein to creativityInnovation is successful when a system is well thought out,
easy to understand, everything fits together and ‘clicks’ in the
mind. An example is the safety clip technology of B. Braun’s
Introcan Safety Catheter, designed to virtually eliminate
accidental needlestick injuries and avoid infections. It is a
global blockbuster because it increases safety and makes
life easier for the medical staff.

Nevertheless, the fact remains that 80% of new developments are incremental improvements, whilejust 20% are substantially new innovations.
A CREATIVE ENVIRONMENT AS A BREEDING GROUNDTo pave the way for future growth of the company,B.
