dr. secchi is Assistant professor of Orthodontics-Clinician educator
and Clinical director of the department of Orthodontics at the
university of pennsylvania, usA and visiting professor at the
university of los Andes, Chile. dr. secchi received his dmd,
Certificate in Orthodontics, and a master of science in Oral Biology
from the university of pennsylvania. dr. secchi is a diplomate of
the American Board of Orthodontics and member of the edward h.
Angle society of Orthodontists. At the university of pennsylvania,
he has developed and implemented courses on Orthodontic
Treatment mechanics, straight Wire Appliance systems and
Functional Occlusion in Orthodontics. In addition, he maintains an
active orthodontic practice in philadelphia.

Since the early days of the orthodontic specialty,
we have looked for the best, fastest, most consistent
and easiest ways to achieve ideal orthodontic
correction. this continuous quest has allowed
emerging technologies to integrate, although
slowly sometimes, with existing practices until
they eventually become routine. As new materials,
improvements in appliance design and function and
innovative ideas transform the way we practice, it is
important for the contemporary orthodontist to be
cognizant of these changes, and to be able to embrace
and incorporate the best of them. Today’s fixed
orthodontic appliances are a blend of technologies
that have been around for decades. Straight Wire
Appliances, self-ligating brackets and low deflection
thermal-activated archwires are all examples of the
progress over the past one hundred years.
Complete Clinical Orthodontics represents a
further improvement that, when correctly applied,
enhances the capabilities of appliances, improves
treatment mechanics, and more importantly,
produces better results. to understand what
Complete Clinical Orthodontics or “CCO” is, let’s
start with a few words about what it isn’t. CCO is not
a technique or technology, but a unique way to treat
each and every case to achieve the most successful
clinical outcome for that patient.
By combining the knowledge amassed over
a century in clinical orthodontics with the best
technology currently available, the CCO system
is the answer to providing control, predictability,
and efficiency for all cases. The inclusive system
integrates sound orthodontic principles and proven
techniques from many of the brightest minds in
orthodontics (Angle, tweed, Ricketts, Andrews, Roth
Alexander, Mclaughlin and Damon to name a few)
to create a significant point of differentiation for you
and your practice.
today, we have a much deeper knowledge of how
and why some of the best appliances have worked,
and as a result, we have been able to improve
upon them. We have a more comprehensive
understanding of biomechanics with pre-adjusted
active self-ligating appliances, effective and efficient
early treatment, adult orthodontics with periodontal
challenges and esthetic considerations for each
patient’s face and smile.
Based on extensive and ongoing work, and with
the help of Dr. Antonino Secchi, DentSply GAC
now offers this comprehensive orthodontic system
that addresses diagnosis, treatment planning and
treatment delivery in a single inclusive approach;
a system that integrates the latest technological
advancements to best facilitate an effective new
overarching treatment philosophy.
By unifying these proven and time-tested
treatment planning strategies, the CCO orthodontic
system stresses 3D skeletal and dental diagnosis.
Additionally, CCO focuses on integrating concepts
from periodontics, restorative dentistry, and oral
surgery to provide a comprehensive treatment
planning system to propel the diagnostic scope of
your entire practice to an even higher level. Our
simple and efficient mechanical approach, combined
with state-of-the-art, active self-ligating appliances,
provides the necessary elements to consistently
deliver exceptional results, case after case.
Controlling the variables of what we do in
all aspects of our lives leads to productivity and
predictability of results. the CCO system commands
total orthodontic control through a logical treatment

In-Ovation® bracket systems haveprovided the best alternative forclinicians looking for an efficientand reliable “true” straight wire, active self-ligating appliance forover a decade.
New design features, plus a newprescription introduced by dr.secchi, will make In-Ovation® define the future of self-ligation.

“I sometimes use this analogy:
in racing a high-performance car,
the engine is important, but does
nothing without the transmission
and a driver. The driver shifts the
transmission in a logical and consistent
sequence from first gear to second, to
third, etc., to achieve the maximum
performance from the engine and
the rest of the car. In orthodontics,
the bracket is the engine. The wires
are the transmission, being changed
methodically in specific sequence to
achieve the fastest, most predictable
result. As for which appliances you
choose: You can buy a Porche or a
Ferrari, but if you don’t know how to
drive it, you’ll either crash in the first
turn, or see no better performance
than you would from a toyota.”

sequence – one that is easily determined through
planning and simulation – before ever initiating work
on a patient. Unlike traditional orthodontic treatment
where treatment strategies can vary on a visit-by-visit
basis, the objective of the CCO system is to emphasize
goal-directed treatment that ends in a pre-visualized
result. This not only instills confidence in the
practitioner for the patient, but leads to a more efficient
and higher quality outcome.
In his educational courses, which have been called by
many “the best courses in clinical orthodontics today,”
Dr. Secchi masterfully illustrates how the straight
wire appliance integrates with self-ligation, and how
the In-Ovation® appliance, combined with a specific
archwire sequence can help the orthodontist to more
efficiently correct different types of malocclusions.
Relevant characteristics of active self-ligating appliances,
a modified prescription, along with a specific wire
sequence for different orthodontic scenarios are among
other topics he discusses. “If I were to choose 10 cases
that accurately depict ‘everyday’ problems we face in
our practices, the treatment mechanics for all ten cases
can be divided into three stages: stage 1 - leveling and
alignment, stage 2 - working stage and stage 3 – the
finishing stage.
At each of these stages, there are specific movements
of teeth that will occur and specific goals that have
to be achieved before continuing to the next stage of
treatment. As you would expect, both the treatment
outcome and efficiency will be greatly improved if the
orthodontist follows the stages as recommended. the
stages of treatment mechanics, with their respective
wire sequences, have been tailored for active self-ligating
brackets, although they can be applied to any straight
wire appliance. that’s the beauty of CCO.”
Orthodontics is in the details. the accepted
principles of leveling and aligning still hold true today,
but there is a strong need for solid mechanics and a
unified approach to using proven methodologies capable
of treating every case individually. there are unique
challenges with every patient; every face is distinctive
in some way and we need to learn to diagnose and
treatment plan cases with those details in mind.
Again, the goal of CCO is to capitalize on the wealth
of knowledge available to us as orthodontists, and to
incorporate new technology and proven concepts to
achieve a higher level of efficiency. Excellent treatment
mechanics should allow the orthodontist to deliver
optimal treatment to the majority of his or her patients
and to do it in the most efficient and simple way that
maximizes an often-overlooked commodity — tIMe.
“When I think about the future of clinical orthodontics, I don’t think of a new technological discovery that will make us forget everything we have done in the past. I see an intelligent integration of the best concepts our predecessors gave us with the most efficient technologies we currently have.”

CaSe StuDy 4:Katherine C.
Katherine is a 12 year, 9 month old female who presented with a Class II end on molar, block out canine and crowding. I treated her with extraction of maxillary and mandibular first premolars. After proper inclination of anterior teeth was achieved and the occlusal plane was leveled, minimum anchorage mechanics was used to close spaces. This mechanics allowed good lip support and facial esthetics although extractions were performed.
