Introduction
The Native American Elders Outreach Project
The creation of the Wisconsin Native American Elders Resource Directory 2000 and
subsequent updates are funded by Wisconsin Judicare, Inc.’s Native American Elders
Outreach Project (NAEOP). The NAEOP began in March 1993 with a grant from the
Wisconsin Department of Health and Family Services using federal Health Care Financing
Administration funds. Over the last seventeen years, one of the primary components of the
project has been visits to Native American elders at tribal nutrition sites. These visits have
included presentations and information focused on health care and income maintenance
programs. Specific topics have included Medicare, Medicaid, Social Security, Indian Health
Service, and Veterans Benefits, to name a few.
During the nutrition site visits, a staff attorney from Wisconsin Judicare is available to
meet with individual elders to answer legal questions and, in some cases, provide legal
representation or a referral to another attorney. Part of the NAEOP grant funds have also
been used to create videos and booklets for Native American Elders, such as this Directory.
Additionally, the NAEOP has organized training sessions for lay advocates, tribal benefit
specialists, county benefit specialists and tribal aging unit directors.
Native American elders who live in Wisconsin who have questions on health and
income maintenance topics such as Medicare, Medical Assistance (Medicaid), the Indian
Health Service, Social Security, Supplemental Security Income (SSI), elder abuse, powers of
attorney for health care and living wills should contact Wisconsin Judicare for assistance.
There is no cost to elders who participate in the project and all Native American elders
in Wisconsin are eligible regardless of income. Elders interested in information on these
benefit programs can call Wisconsin Judicare at 1-800-472-1638 and ask for an NAEOP staff
attorney.

programs of interest to Native American elders. For more information on any of the
following programs, elders should contact the governmental agency or program directly. The
advocates described under the General Benefits Assistance heading and the Legal Assistance

Disability Benefit Specialist Program. Disability Benefit Specialists provide confidential
assistance at no charge to adults ages 18 to 59 with physical disabilities, developmental
disabilities, mental illness and/or substance abuse disorders. Individuals age 60 and older can

First Call for Help. Many counties have a free telephone hotline that can assist elders in
finding telephone numbers in their area. To find a local number for this service, look under

Medical Benefit Specialist. Many Tribes have a Medical Benefit Specialist at their health
care centers to assist with applications for Medical Assistance. Contact your Tribal health
care center (listed in Part 2 of this directory by Tribe) for more information.
National Association of Area Agencies on Aging (N4A). The umbrella organization for the
655 area agencies on aging (AAA) and more than 230 Title VI Native American aging
programs in the U.S. N4A advocates on behalf of the local aging agencies to ensure that
needed resources and support services are available to older Americans. Contact: 1730

Native American Elders Outreach Project (NAEOP) staff attorney. Wisconsin Judicare’s
Native American Elders Outreach Project has a staff attorney available to work with Native
American elders who live anywhere in Wisconsin. This attorney can provide information on
benefits and, in some case, represent elders in appeals and other matters. For more

Tribal and County Aging Units/Aging & Disability Resource Centers. Each Tribe and
County has a department or unit that is dedicated to providing services to elders. Services

provided can include home-delivered meals, chore services, exercise programs and other
services. A partial list is in Section two of this directory. A full list can be found at the

Tribal Benefit Specialists and County Benefit Specialists. Each Tribe and County has a
benefit specialist who can explain benefit programs, assist elders with applications for
benefits and refer elders to other agencies that can provide assistance. The services provided
by a benefit specialist are free. The local Tribal and County Benefit Specialists are listed in

Tribal Disability Benefits Specialists. Enrolled tribal members who live on or near a
reservation in Wisconsin may receive disability benefit specialist services through Great

Income Benefits & Assistance The following organizations can offer information and assistance specifically related to
income benefits.
Energy and Weatherization Assistance. These programs provide assistance to low-income
individuals. Different agencies administer these programs throughout the state. To find out
how to apply in your area, contact your Tribal or County Social Services office, or a Benefit
Specialist, and ask for information on the Low-Income Energy Assistance Program. Website:

Food Stamps and Commodities. Food Stamps or distributions of food baskets are available
to low income, low-asset individuals. For more information, contact Tribal or County Social
Services.
Kinship Care. Elders who are caring for minor children may be eligible for financial
assistance. For more information contact Tribal or County Social Services.
Pension Rights Project. The Upper Midwest Pension Rights Project utilizes a network of
benefit specialists, law students, attorneys and volunteers to help answer individual’s
questions regarding pensions, including tracking lost pensions, survivor’s rights, benefit

Railroad Retirement Board. Railroad employees and their families may be eligible for

State of Wisconsin SSI Supplement. All Federal SSI recipients in Wisconsin automatically
receive a State SSI supplement. Additionally, there are a small number of people who

Supplemental Security Income (SSI). This program provides a financial benefit and free
Medical Assistance to low-income, low-asset individuals who are disabled or at least age 65.

Donated Dental Services. The Wisconsin Dental Association provides a low-cost program to

Hospital Patient Assistance Programs. Many hospitals and medical centers have programs
that pay some or all health care costs for those with limited resources who earn too much to
qualify for Medical Assistance. For more information, contact the medical facility and ask
for information on patient financial assistance programs.
Indian Health Service (IHS)/Contract Health Service (CHS). All Tribes in Wisconsin have
health clinics that are funded in part by federal dollars from the Indian Health Service. In
addition to providing health care on site, the clinics provide vouchers through the Contract
Health Service program for Native Americans to obtain treatment at non-Indian facilities.

Medicare (Center for Medicare and Medicaid). Medicare provides health care insurance at
age 65 to those who are insured through the Social Security program. Certain disabled
persons are also eligible for Medicare. There is no monthly premium for Part A, but there is
a monthly premium for Part B. To apply for Medicare, contact the Social Security
Administration, either through the national 800 number, 1-800-772-1213, or by contacting a
local Social Security office. Note: Railroad Retirement Beneficiaries should contact the
Railroad Retirement Board to enroll in Medicare. Website: www.medicare.gov or

Medicare Supplement Plans. Individuals can purchase additional private health insurance to
supplement their Medicare plan. For information on Medicare Supplement Plans, contact the
State of Wisconsin Office of the Commissioner of Insurance at 1-800-236-8517 or the
Medigap Helpline at 1-800-242-1060.
Medigap Helpline (Medicare Supplement Insurance). General number: 1-800-242-1060.

Medical Assistance (Medicaid). This program provides health insurance to low-income, low-
asset individuals who are disabled or at least age 65. Some caretakers of minor children also

receive Medical Assistance. In Wisconsin, Supplemental Security Income (SSI) recipients
automatically receive Medical Assistance. Bad River, Forest County, Lac du Flambeau,
Oneida, Red Cliff, Sokaogon, Stockbridge-Munsee Tribal Elders can receive services directly
from their Tribal Social Services, otherwise contact your local County Social
Services/Human Services Department. For specific questions regarding your Medicaid

Patient Assistance Program (Indigent Drug Program). Low-income patients who need
assistance paying for prescription drugs may be eligible to receive free prescription drugs
from drug manufacturers. For more information, ask your doctor about this program or
contact a Tribal or County Benefit Specialist. You can also download a free directory of

PhRMA 2006 Directory of Prescription Drug Patient Assistance Programs. (free prescription

QMB, SLMB, and SLMB+. These programs pay some of the unreimbursed costs associated
with Medicare, such as the Part B Medicare Premium, for Medicare recipients who meet
income and asset guidelines. The financial limits are higher than the limits for Medical
Assistance, so it can be easier to qualify for these programs. To apply, applicants should
contact their Tribal or County Social Services Office.
SeniorCare. SeniorCare is a Prescription Drug Program for Wisconsin residents aged 65 or
over and who meet eligibility requirements. There is a $30/year enrollment fee, and
eligibility is based upon income. There is no asset test. Contact your local Agency on Aging
or the SeniorCare customer service hotline at 1-800-657-2038.

United States Department of Veterans Affairs Health Program. The U.S. Department of
Veterans Affairs provides health benefits to veterans at a variety of health centers throughout
the state. Veterans can contact their Tribal or County Veterans Service Officer for more

Domestic Abuse. Elders who experience Domestic Abuse or who have questions about

Financial Exploitation Project is focused on serving the direct needs of elders age 60 or over

Disability
Aging and Disability Resource Centers. Pursuant to a new long-term care redesign program
in Wisconsin, there are many Aging and Disability Resource Centers in Wisconsin that
provide information and assistance in accessing benefits and services. More centers may
open in the future. You can find a list of Aging and Disability Resource Centers near you at

Social Security Administration Programs. Disabled individuals can receive financial benefits
from the Social Security Administration through the Social Security Disability Insurance
(SSDI) program and/or the Supplemental Security Income (SSI) program. The Social
Security Administration also has several programs designed to help disabled individuals

Long-Term Care
Nursing Home Care. Numerous nursing homes and skilled nursing facilities provide services
to elders in need of special care. For information on nursing homes and other facilities in
your area, talk to your doctor or hospital social worker. The hospital social worker, a benefit
specialist or an attorney can also provide you with information on financing nursing home
care privately or through the State of Wisconsin’s Medical Assistance Nursing Home
program. Residents of nursing homes who experience problems in the facility are
encouraged to contact a Long-Term Care Ombudsman at 1-800-815-0015.
Home and Community-Based Programs. The State of Wisconsin has several programs that
provide home-based services to at-risk individuals so that they can remain in their homes
instead of entering a nursing home or other long-term care facility. These programs include

the Community Options Program (COP), COP-Medical Assistance Waiver (COP Waiver),
Community Integration Program (CIP II), and Family Care. Funds for these programs are

Great Lakes Inter-Tribal Council
The Great Lakes Inter-Tribal Council (GLITC) supports member tribes in
expanding self-determination efforts by providing services and assistance. GLITC
uses a broad range of knowledge and experience to advocate for the improvement
and unity of tribal governments, communities, and individuals. Throughout these
activities, GLITC maintains deep respect for tribal sovereignty and reservation

Senior Companion Program
This program offers opportunities for healthy older adults to provide services to
elders who may be homebound, alone, and isolated without family support. Senior
Companions serve clients in tribal elderly housing, nursing homes, and private

Power of Attorney for Health Care
A power of attorney for health care allows a designated individual to make medical
decisions, according to your wishes, in the event that you become unavailable to
do so. More information about power of attorney for health care can be found at

Wills
Although planning for the future can be difficult, it can also give peace of mind. A
plan not only helps us protect ourselves and our families, it can also help preserve
cultural resources, such as Indian trust land, so that the resources can be enjoyed
by future generations.
The Indian Wills project of Wisconsin Judicare exists to help tribal members with
these difficult planning decisions, and with navigating the variety of tribal, federal,
and state laws that effect the estate plans of tribal members. Judicare offers this
service free to low-income tribal and/or elderly members. For American Indians
who are ineligible for free services from Wisconsin Judicare we maintain a list of
private attorneys who are trained to provide estate planning services to American

Supplemental Security Income
If you are at least 65 years old OR blind OR disabled, and you satisfy certain
income and asset requirements, you may be eligible for Supplemental Security
Income (SSI). For more information on how to apply, contact Wisconsin Judicare

Social Security
If you have worked 40 quarters (10 years) in your life, or you are the spouse,
former spouse, eligible dependant, or widow(er) of someone who is eligible for
Social Security, then you may qualify for Social Security benefits. Contact

Medicaid
Wisconsin Medicaid is a program that provides some medical care, including some
prescription drugs; physician’s services; inpatient and outpatient hospital care;
laboratory and x-ray charges; hearing aids and glasses; dental care; and other
services to individuals with low income and few assets. To be eligible you must be
at least 65 years old, disabled, or blind, and have low income and few assets. For
more information, please contact Wisconsin Judicare at 1-800-472-1638.
Medicare
In general, all persons 65 years of age or older who have been legal residents of
the United States for at least 5 years are eligible for Medicare. However, if neither you
nor nor spouse have paid Medicare taxes for a minimum of 10 years (40 quarters), then
you must pay a monthly premium to be enrolled in Medicare. In certain circumstances,
these premiums can be waived or reduced. Contact Wisconsin Judicare at 1-800-472-
1638 for more information. Part B
o If you are having trouble paying your Medicare Part B premiums, or
declined Medicare Part B because the premiums were too high, the
Medicare Saving Program might be able to assist you. If you are eligible for
Medicare and have low income and few assets, you may be eligible for the

Part D
o If you need help paying for prescription medications, you may be able to
receive assistance from Medicare Part D. If you are 65 years or older, and
have low income and few assets, then you may be eligible for the Extra
Help Program in which you will receive a monthly subsidy to help pay for
premiums, deductibles, and/or co-payments under your Medicare Part D
