Information Products Catalogue – Spring 2008

The Spring 2008 catalogue provides bibliographic data and descriptive texts on over 80 recentinformation products that have been published since October 2007 by the World HealthOrganization, its regional offices, the International Agency for Research on Cancer (IARC), and the Joint United Nations Programme on HIV/AIDS (UNAIDS). The catalogue is divided into three sections, corresponding to English, French and Spanish language editions. Two new sections have been added to the catalogue: a selection of WHO publications madeavailable for translations (see pages 30-33) and a selection of WHO books translated in Arabic by the WHO Regional Office for the Eastern Mediterranean (see pages 34-35).

This new collection of WHO information products reflects the work and priorities of theOrganization and provides guidelines and standards for medical, technical and scientificprofessionals working in many different fields.

On 7 April 2008, the World Health Organization celebrated its 60th Anniversary. To mark thisevent, we have included in this catalogue a list of "WHO classics" (page 2-3) published since1948 and which have made substantial contributions to global public health. The ten top WHO bestsellers since 1948 are featured on page 25. Many of these books have been revisedand reprinted and are still considered landmarks in their field.

Of special interest is the WHO report on the Global Tobacco Epidemic, 2008, which confirmsthat tobacco is one of the greatest public health treats of modern times. Other noteworthypublications are the expanded International Health Regulations (2005), the 2008 edition ofInternational Travel and Health, Supplement 1 to the International Pharmacopoeia, a newModel List of Essential Medicines, a second edition to Atlas of Health in Europe and a 2008edition of World Health Statistics.

Other selected information products in the catalogue cover the latest thinking on ageing,cancer, cardiovascular diseases, chemical safety, child health, food safety, mental health as well as on water and sanitation and zoonoses.

Ethical Considerations in Biomedical HIV Prevention Trials

Smokeless Tobacco and Some Tobacco-specificN-Nitrosamines

This eighty-ninth volume of the IARC Monographs is the thirdand last of a series on tobacco-related agents. Volume 83reported on the carcinogenicity of tobacco smoke andinvoluntary smoking (second-hand smoke or environmentaltobacco smoke) (IARC, 2004a). International MedicalGuide for Ships Including the Ship's Medicine ChestThird Edition

The third edition of the International Medical Guide for Shipsshows designated first-aid providers how to diagnose, treat, and prevent the health problems of seafarers on board ship.

Since its first publication in 1967, the International MedicalGuide for Ships has been a standard reference for medical careon board ships. The second edition, written in 1988, wastranslated into more than 30 languages, and has been used in tens of thousands of ships. This, the third edition, containsfully updated recommendations aimed to promote andprotect the health of seafarers, and is consistent with thelatest revisions of both the WHO Model List of EssentialMedicines and the International Health Regulations.

The International Labour Organization's Maritime Labour Convention2006 stipulates that all ships shall carry a medicine chest, medicalequipment and a medical guide. The International Medical Guide forShips supports a main principal of that convention; to ensure thatseafarers are given health protection and medical care as comparableas possible to that which is generally available to workers ashore.

By carrying this guide on board ships, and following its instructions,countries can both fulfill their obligations under the terms of theMaritime Labour Convention 2006, and ensure the best possible health outcomes for their seafaring population.

International Health Regulations (2005)Second Edition

In consideration of the increases in international travel andtrade, and emergence and reemergence of internationaldisease threats and other health risks, the Fifty-eighth WorldHealth Assembly in 2005 adopted the revised InternationalHealth Regulations (IHR). The new IHR entered into force on 15 June 2007.

Their stated purpose and scope are “to prevent, protectagainst, control and provide a public health response to the international spread of disease in ways that arecommensurate with and restricted to public health risks, andwhich avoid unnecessary interference with internationaltraffic and trade.” Because the IHR are not limited to specificdiseases, but are applicable to health risks, irrespective oftheir origin or source, they should remain current withdevelopments in the evolution of diseases and the factorsaffecting their emergence and transmission.

The IHR also require States to strengthen core surveillanceand response capacities at the primary, intermediate andnational level, as well as at designated international ports,airports and ground crossings. They further introduce aseries of new health documents, including ship sanitationcertificates and an international certificate of vaccination or prophylaxis for travelers.

Finally, this second edition includes a new foreword and the Health Part of the Aircraft General Declaration (as revisedby the International Civil Aviation Organization), as well asAppendices listing States Parties to the IHR and reservations,objections and declarations received from States Parties.

This report represents the conclusions of a Joint FAO/WHOExpert Committee convened to evaluate the safety of various food additives, including flavouring agents, with a view to recommending acceptable daily intakes (ADIs) and to preparing specifications for identity and purity. The Committee also evaluated the risk posed by two foodcontaminants, with the aim of advising on risk managementoptions for the purpose of public health protection.

The first part of the report contains a general discussion of the principles governing the toxicological evaluation and assessment of intake of food additives (in particularflavouring agents) and contaminants. Health impact assessment (HIA) is a support tool forintersectoral decision- and policy-making. It is employed to assess the potential health consequences of pendingdecisions and it feeds this information back into thedecision-making process.

The InternationalPharmacopoeia Fourth Edition – First SupplementThe fourth edition of the International Pharmacopoeia comprisestwo main volumes published in 2006. This first Supplementincludes a collection of recommended procedures for analysis and specifications for the determination of pharmaceuticalsubstances, excipients and dosage forms that is intended to serveas source material for reference or adaptation by any WHOMember State wishing to establish pharmaceutical requirements.

New monographs New monographs are included for fiveantiretroviral substances, for sixteen antiretroviral dosageforms including two fixed-dose combination preparations, for one antimalarial dosage form and for six antituberculosisdosage forms including 2-, 3-, and 4-component fixed-dosepreparations. Such specifications support the joint UNICEF-WHO-UN Prequalification project, managed by WHO. In

developing these monographs WHO has worked closely withmanufacturers, national authorities and national quality controllaboratories. New general monographs for Liquid preparations for oral use and Oral powders are included to support the relevantspecific dosage-form monographs. The monograph for Liquidpreparations for oral use highlights that such preparations areoften the dosage form of choice for paediatric use. This generalmonograph also includes a statement on Safety concerns in thesection on Manufacture. This draws attention to the importanceof ensuring the quality of starting materials. Failure to ensure that starting materials are of the required quality can have veryserious consequences.

Revision In parallel with the development of the newmonograph for Doxycycline capsules, the publishedmonographs for Doxycycline hyclate and Doxycycline tabletshave been revised and replacement texts are included in thissupplement. A requirement for dissolution has been includedfor several monographs.

Infrared Reference spectra Many monographs in TheInternational Pharmacopoeia include an identification test usinginfrared spectroscopy; these tests usually allow comparison eitherwith a spectrum obtained from the ICRS or with a referencespectrum. The majority of the infrared reference spectra are newly avialable and published within this supplement.

Chromatographic tests In chromatographic tests includedwithin the monographs of The International Pharmacopoeia,the type of chromatographic column, column packing or TLCplate to be used is stated but reference to commercial sources of these supports is not given. When a draft monograph iscirculated for comment, the particular column that was usedduring the development of a liquid chromatographic test isprovided for information (usually in a footnote to the text). This information is now provided on the WHO Medicineswebsite to assist analysts with respect to choice of column forthose monographs recently introduced into the Pharmacopoeia.

The Expert Committee on Specifications for PharmaceuticalPreparations works towards clear, independent and practicalstandards and guidelines for medicines' quality assurance.Standards are developed by the Committee throughworldwide consultation and an international consensusbuilding process.

WHO has produced a first comprehensive analysis of global tobaccouse and control: The WHO Report on the Global Tobacco Epidemic,2008. The report confirms that the global tobacco epidemic is oneof the greatest public health threats of modern times, that, leftunchecked, could result in one billion deaths in the 21st century.The epidemic is shifting towards the developing world, where 80%of tobacco-related deaths will occur within a few decades. Theshift is caused by a global tobacco industry strategy to targetyoung people and adults in the developing world. The reportconcludes that although there has been progress in recent years,virtually every country needs to do more . Currently only 5% ofthe world s population is fully covered by any one of the keyinterventions that have significantly reduced tobacco use in thecountries that have implemented them.

The WHO Report on the Global Tobacco Epidemic, 2008identifies

as powerful response to the epidemic a set of the six key provenstrategies to drive down tobacco use - the mpower package - whoseimplementation provides the best chance for saving millions of livesdevastated by tobacco:

• Monitor tobacco use and prevention policies. • Protect people from tobacco smoke.• Offer help to quit tobacco use. • Warn about the dangers of tobacco.• Enforce bans on tobacco advertising, promotion

and sponsorship.• Raise taxes on tobacco.

The WHO Framework Convention on Tobacco Control (WHO FCTC)created political momentum for global tobacco prevention. Thempower package provides a clear roadmap to help countries fulfiland build on their WHO FCTC obligations and save many millions of lives by the middle of this century.

Countries are not alone in combating the tobacco industry which ispowerful and cash-rich. Assessing Tuberculosis Prevalence throughPopulation-based Surveys

This publication provides countries with practical guidelinesfor planning population-based surveys to estimate theprevalence of tuberculosis (TB) at a national level. TBprevalence surveys yield useful information in areas wherenotification data obtained through routine surveillance areincomplete or of unproven accuracy, and in areas with anestimated TB prevalence of more than 100 per 100 000.

These surveys are used to evaluate the performance of the TB programme and to assess trends over time. To achieve thisobjective, data are collected through standards methods in awell-defined study population.

This document is meant to provide information on the coresurvey methods, including diagnostic tests for TB, screeningstrategies, and case definitions. The target audience includesTB experts and advisers at national and international levels,and investigators involved in prevalence surveys.

WHO Guidelines on Tularaemia

Tularaemia is a bacterial zoonotic disease of the northernhemisphere. The bacterium (Francisella tularensis) is highlyvirulent for humans and a range of animals such as rodents,hares and rabbits. Humans can infect themselves by directcontact with infected animals, by arthropod bites, by ingestionof contaminated water or food, or by inhalation of infectiveaerosols. There is no human-to-human transmission.

In addition to its natural occurrence, F. tularensis evokes great concern as a potential bioterrorism agent. F. tularensissubspecies tularensis is one of the most infectious pathogensknown in human medicine.

In order to avoid laboratory-associated infection, safetymeasures are needed and consequently, clinical laboratoriesdo not generally accept specimens for culture. However, sinceclinical management of cases depends on early recognition,there is an urgent need for diagnostic services.

This first edition of WHO Guidelines on tularaemia providesbackground information on the disease, describes thecurrent best practices for its diagnosis and treatments in humans, suggests measures to be taken in case ofepidemics and provides guidance on how to handle F. tularensis in the laboratory.

Can calcium and magnesium (‘hardness’) in drinking watercontribute to preventing disease? The Blue Trunk Library has been developed by the libraryof the World Health Organization for installation in districthealth centres in Africa as a means of compensating forthe lack of up-to-date medical and health information.After 10 years of existence more than 1800 libraries have

been distributed to health districts in poorcountries of the world, where healthinformation access is difficult. Trainingworkshops, for the optimal use of theinformation contained in each blue trunk, was also provided to more than 550 healthprofessionals during this period.

The collection, which is organized accordingto major health subjects, contains more than one hundredand fifty books on medicine and public health. Thecollection has recently been revised and updated with the latest information available.

Among the works chosen, priority has been given topractical manuals (especially those published by WHO)offering easily accessible solutions to the medical, public

health and management problems medical staff mayhave to face. Differing levels of education amongdistrict medical staff have also been taken into account.The same topic may be addressed in differentpublications from a different point of view: that of thephysician, the nurse, the nursing auxiliary or health worker.

In order to make it easier to transport and store, thecollection has been packed into a blue metal trunk fittedwith two shelves on which cardboard boxes containingthe books are arranged by field of interest.

Voilà près de 25 ans que la Classification internationale desmaladies pour l’oncologie (CIM-O) constitue l’ouvrage deréférence pour coder les diagnostics de néoplasmes dans les registres des tumeurs et du cancer et les laboratoiresd'anatomopathologie. La CIM-O, qui en est maintenant à sa troisième édition, est une double classification avec des systèmes de codage topographique et morphologique.

Si tous les codes topographiques restent les mêmes que dans l’édition précédente, les codes morphologiques ont été entièrement réexaminés et, le cas échéant, modifiéspour améliorer leur précision diagnostique et leur valeur

pronostique. La plupart des modifications reflètent la nécessitéurgente de coder les nouveaux diagnostics en hématopathologie.

La chimioprévention se sert des médicamentsanthelminthiques disponibles, seuls ou en association, pouren faire un outil de santé publique et prévenir la morbiditéimputable à des infections dues en général à plus d’unhelminthe à la fois. Dans certaines situations épidémiologiques,elle permet également d’obtenir une réduction durable de latransmission. Comme nombre de ces médicaments ont unlarge spectre d’action et permettent de lutter contre plusieursmaladies à la fois, les actions de chimioprévention doiventêtre conçues davantage en fonction du médicament que de la maladie : l’accent doit porter sur la meilleure façond’utiliser de manière coordonnée les médicamentsdisponibles, plutôt que sur des helminthiases spécifiques.

Bien que le présent manuel porte essentiellement sur lachimiothérapie des helminthiases humaines, il existe unpotentiel énorme pour intégrer celle-ci dans le traitementd’autres maladies. La chimioprévention doit donc commencerà un âge précoce et il ne faut laisser passer aucune occasionde traiter les populations exposées au risque.

Dans ce manuel, nous avons voulu donner des informationsutiles pour la mise en oeuvre coordonnée d’interventionsrégulières, systématiques et à grande échelle qui fournissentla chimiothérapie anthelminthique en tant qu’élémentessentiel d’une lutte conjointe et en synergie contre leshelminthiases que sont la filariose lymphatique, l’onchocercose,la schistosomiase ou encore les géohelminthiases.

Tout en étant un document générique, applicable dans toutes les situations épidémiologiques, il garde sa spécificitéen ne traitant que des médicaments et de leur utilisationcoordonnée : il ne couvre pas d’autres interventionsimportantes pour la santé publique, comme l’éducationsanitaire, l’assainissement, la salubrité de l’eau, la lutteantivectorielle, etc., nécessaires pour obtenir une réductiondurable de la transmission, puis l’élimination des helminthiases.Il reviendra aux administrateurs de programme d’adapter lecontenu du manuel à la situation régionale, nationale, localeet aux ressources disponibles.

Directives pour le traitement du paludisme

Le but de ce document est de fournir des lignes directricescompréhensibles, de portée mondiale, reposant sur des bases factuelles, pour aider à formuler des politiques et desprotocoles de traitement du paludisme. Des indications y sont présentées sur le traitement du paludisme simple,notamment dans des groupes particuliers (jeunes enfants,femmes enceintes, personnes VIH-positives, voyageursprovenant de zones non endémiques pour le paludisme), lors des épidémies et dans les situations d'urgence complexes; du paludisme grave. Ce guide ne traitent pas des utilisationspréventives des antipaludiques telles que le traitementpréventif intermittent ou la chimioprophylaxie.

Ces lignes directrices sont principalement destinées auxresponsables de l'élaboration des politiques des ministères de la santé et les spécialistes de la santé publique et despolitiques de santé travaillant dans les hôpitaux, lesministères, les organisations non gouvernementales et les services de soins de santé primaires, les professionnels de la santé (médecins, personnel infirmier et paramédical).

Elles offrent un cadre pour l'élaboration des protocoles detraitement précis et plus détaillés, prenant en compte lescaractéristiques nationales et locales de la pharmacorésistancedes plasmodies et les infrastructures disponibles dans lesservices de santé. Les parties du monde dont les populations sont les plustouchées par des situations d’urgence complexes sontsouvent celles où la charge de paludisme est la plus forte.Cette maladie est donc une importante cause de morbidité et de mortalité dans les situations d’urgence complexes.

Ce manuel interorganisations insiste sur les mesures efficacesde lutte antipaludique nécessaires dans les situations d’urgencecomplexes, surtout lors de la phase aiguë de dépendancemaximale de l’aide humanitaire internationale. Le manuelfournit aux responsables politiques, aux planificateurs, aux responsables des programmes sur le terrain et auxcoordonnateurs médicaux des conseils pratiques sur la miseau point et l’application des mesures permettant de réduire la morbidité et la mortalité palustres.

Ces mesures doivent tenir compte des besoins aussi bien des personnes déplacées que de la population d’accueil et aussi de l’évolution de ces besoins à mesure que l’on passe d’une urgence aiguë à une situation plus stable. Ciudades globales amigables con los mayoresUna guía

El envejecimiento y la urbanización de la población son dos tendencias mundiales que, combinadas, suponen unpotente fenómeno que dará forma al siglo XXI. Al mismotiempo que crecen las ciudades, aumenta la proporción desus moradores que tiene más de 60 años. Las personasmayores son un recurso para sus familias, sus comunidades y sus economías si viven en un entorno que las apoya y las favorece.

La OMS considera que el envejecimiento activo es un proceso que dura toda la vida y que depende de variosfactores que, tanto de forma independiente comocombinados, favorecen la salud, la participación y laseguridad del adulto de más edad. Informado por elenfoque de la OMS sobre el envejecimiento activo, elpropósito de la presente Guía es animar a las ciudades a convertirse en lugares más accesibles para las personasmayores con el fin de aprovechar el potencial querepresentan para la humanidad.

En 33 ciudades de todas las regiones de la OMS, laOrganización ha trabajado con grupos de personas mayores que han descrito las ventajas y los obstáculos que experimentan en ocho aspectos distintos de la vidaurbana. En la mayoría de las ciudades, las observaciones de los mayores se complementaron con opiniones degrupos de cuidadores y proveedores de servicios de lossectores público, voluntario y privado. Los resultados deltrabajo de los grupos llevaron a la elaboración de unconjunto de listas de comprobación del grado en que las ciudades son amigables para los mayores.

La Red Panucraniana de Personas que Viven con el VIH fue fundada a finales de la década de 1990 por personasalarmadas ante el rápido crecimiento de la epidemia en supaís y por la falta de recursos y apoyo para sí mismas y paraotras personas afectadas por el VIH.

Desde entonces, la Red ha crecido para proporcionar serviciospor todo el país. Sus elementos estratégicos fundamentalesson: el creciente acceso a la atención y apoyo; el trabajo depromoción para proteger los derechos de las personas queviven con el VIH; los esfuerzos por aumentar la aceptaciónsocial de las personas que viven con el VIH, y el mejoramientode la capacidad organizativa de la Red.
