There’s No Place Like a Medical Home

One of the best ways to take care of your health is to have a primary care provider (PCP). Your PCP is the main doctor who gives you most of your care and makes referrals. Your PCP will get to know your health history — and know what’s right for you and how to best treat you.

You should think of your PCP as your “Medical Home” — the place that knows you the best!

Your To-Do List:✓ Choose a PCP • The name of your PCP is listed on your

Passport ID card. If you want to see a different doctor, you can change it. Just call Member Services at 1-800-578-0603.

• You can choose any PCP on our list of doctors by visiting passporthealthplan.com/members/find-a-doctor/. Or, you can call Member Services and we’ll help you find one.

✓ Choose a PCP you can trust • Pick a PCP you feel comfortable seeing and

one that can really get to know you.

• Talk openly and honestly with your PCP. You want to trust them enough to talk about anything.

✓ Set up a “well visit” with your PCP• Go ahead and schedule a visit with your PCP,

even if you aren’t sick. Your PCP can get to know your medical history and long-term health goals.

• Think about it. Do you really want to go in for your first doctor visit when you’re feeling sick? That’s not the best time to explain your medical history and long-term goals!

Colorectal or colon cancer is a cancer that starts in the colon or the rectum.

It’s the 3rd most common cancer in men and women in the United States.

No matter which test you get, the most important thing is to get tested!

Getting regular tests for colon cancer is the key to preventing it. Have your first colon test at age 45, or sooner if you have family history of colon cancer.

There are several types of tests for colon cancer:

• Stool-based tests look for cancer in your stool.

• Visual exams, such as a colonoscopy or CT colonoscopy, look for polyps and cancer in your colon or rectum.

Get Tested Early for Cervical Cancer

What causes cervical cancer?It is usually caused by the human papillomavirus (HPV). HPV is passed from one person to another during sex. Most of the time, you won’t even know you have it. Girls and women ages 9 to 26 can get an HPV shot to help prevent cervical cancer.

• The way you speak, smile, eat, and show emotions.

• Your self-esteem, how you do in school, and your ability to go to work and school.

It’s important to take steps to care for your oral health. You and your kids can follow these easy tips listed on this page.

Oral Tips for Adults• Brush your teeth twice a day with

fluoride toothpaste and floss between your teeth to remove dental plaque.

• Visit your dentist every 6 months, even if you have no natural teeth or dentures.

• Do not use any tobacco products.

Oral Tips for Kids• Kids need to brush their

teeth twice a day with fluoride toothpaste.

• Kids need to visit a dentist for their first checkup by age 1 and keep visiting the dentist regularly.

• Kids younger than 2 should not use fluoride toothpaste unless a doctor or dentist says to do so.

• Kids younger than 6 need only a pea-sized amount of toothpaste. They should always spit out the toothpaste after brushing.

When Kids Take Medicine for Behavior Health Needs

Healthy Teeth at Every AgeWhat are psychotropic medicines? Medicines sometimes used for anxiety, attention deficit hyperactivity disorder (ADHD), and depression. Some of these medicines have side effects such as changes in weight, greater risk for diabetes, or changes in sleep.

What can parents do?Be sure to follow-up with the doctor who ordered the medicine. Ask the doctor these questions:

• Has this medicine been tested and approved for my kid’s age?

• How fast will the medicine work and what changes will I see?

• What are the risks and possible side effects?

• What other treatments does my kid need?

• When and how often do you need to see my kid while they are on this medicine?

When kids have behavioral health

needs, they may be treated with therapy and psychotropic medicines. Some kids need therapy

and medicine. Other kids only need

therapy.

Take Care of Your Breasts at Every Age

Your To-Do List:

Get a Mammogram —A mammogram is an x-ray of your breast. It can find breast cancer early before you feel a lump or have any other symptoms. Many doctors suggest starting regular mammograms at age 50. If you have a family history of breast cancer, you may need to start earlier. Some doctors suggest mammograms as early as age 40. Just ask your doctor what’s right for you.

Do Self-Breast ExamsA self-breast exam is when you feel your breasts and check for any changes in the size or shape. All women need do this every month, the same way each time. You can lie down or sit up with your arm overhead. If you feel or see any changes in your breast, call your doctor right away.

Taking care of your breasts is important at all ages. You can keep your breasts

healthy with self-breast exams and yearly mammograms.

What happens at a well-visit?

• The doctor looks at your kid’s health and talks to you about what’s normal at their age. If you have a teen, the doctor may talk to your teen alone.

• The doctor will look at the medicines your kid is taking.

• The doctor will check your kid’s blood pressure, weight and height, and listen to their heart.

• The doctor will look at your kid’s “whole health” and try to see everything that may be affecting it. Don’t be surprised the doctor asks about friendships, school, or other things.

• The doctor will talk to you about healthy habits.

Chronic Obstructive Pulmonary Disease (COPD), is a long-term lung disease. COPD is also called emphysema or chronic bronchitis.

You CAN Manage COPD!

Even though COPD gets worse over time, there are ways you can manage it:

• Visit your primary care provider (PCP) on a regular basis.

• Talk to your PCP about how you feel. Ask about how to take your medicines and how to manage your COPD.

• Over time it will get easier to talk to your PCP, it just takes practice!

When you visit your PCP, he or she can find and treat problems before they get worse. Your PCP can help you:• Stop COPD from getting worse

• Do better with exercise

• Improve your health

• Avoid and treat any problems (complications, exacerbations)

Caring for Your Premature Baby

A premature baby is born too early or before 37 weeks of pregnancy. These babies may need extra medical attention such as:

• Medical testing

• Breathing and heart support

• Medicines
