WelcometotheCalderdale‘What Next?’booklet-Wehopeyoufindituseful

An Education, Health and Care (EHC) Plan is written to help you learn, develop independence skills and achieve your hopes, dreams and aspirations.

Your EHC Plan is reviewed every year to make sure it is up to date and focuses on what you need to help you achieve the best outcomes for you.

Annual Reviews from Year 9 onwards include an important focus on preparing for adulthood. Thinking about preparing for adulthood makes sure that your EHC Plan looks forward to:

• Employment and or volunteering;

• Independent living;

• Friends, relationships and being an active member of your community;

• Maintaining good health.

This will help you prepare for when it is time to leave school or college.

There is a lot of help available in Calderdale to support young people.

This booklet is part of the preparing for adulthood focus. It includes:

• Information on possible education opportunities after Y11;

• The support available to help you further develop independence after leaving school;

• Ideas on local community support.

Reading the booklet at home and in school will help give you an idea of the options open to you.

You do not have to plan everything in detail at Year 9 but it is important that you begin to look ahead. The EHC Plan Annual Reviews can help you do this. Your school will make sure that these reviews are really helpful for you.

Education:

Education after Y11 can be planned around individual interests and aspirations.

Most young people continue education in the Calderdale area. Some young people follow A Level courses at a local school before considering university options.

Other young people look at vocational courses. You can find out about some of the options on pages 6 to 9.

Some young people look at courses out of the Calderdale area, perhaps at Kirklees College, Huddersfield New College or Leeds City College. Occasionally young people look at specialist colleges.

At school you can talk about possible education and training opportunities with an adviser from C&K Careers.

Whichever school or college you look at, it is important that you discuss this with the Special Educational Needs (SEN) team. Their contact details are: [email protected]

About us

The Five Day Offer is a supportive framework which focuses on young people aged 18+ with a learning disability who are leaving full time education. It helps them to create a positive transition from school to college and employment by developing links around further learning, independent living skills, volunteering, paid work and leisure in their local community.

Referrals are made to us through the Special Educational Needs and Disability (SEN) Team. Young people will be referred to the Five Day Offer if they:

About us

We offer a Vocational Skills course which is a specialist and personalised course for Sixth Form learners with an Education, Health and Care Plan and none or few formal qualifications. We also offer a course which includes qualifications such as Functional Skills in Maths and English and Personal Development.

Calderdale College offers a wide variety of Vocational courses from Entry Level to Higher Education for 16-18 year olds or 16-25 year olds with an EHC Plan.

Young people interested in attending College can access personalised packages to suit their individual need which may include additional therapies and activities alongside their main programme of study.

Individualised Additions can include:

We provide a wide range of courses from Maths and English to IT, art and cookery, aimed at promoting self-confidence and independence.

We offer a wide range of full and part-time courses throughout the year to adults over 19 in a friendly, supportive environment. Calderdale Council commissions C&K Careers to provide Careers Information, Advice and Guidance to young people with Special Educational Needs and Disabilities (SEN).

C&K Careers has a dedicated SEN team of Careers Advisers who work with all young people at Ravenscliffe High School and with all young people with an EHC Plan in Calderdale mainstream secondary schools and Calderdale residents in out-of-area schools. We also have Careers Advisers working in Calderdale College and other local Further Education Colleges. Where young people have not found something suitable, we have advisers who can help find the right course or training.

We also offer support to young people who are home educated.

What we do

About us

Project SEARCH provides students age 18+ with the chance to work for local businesses and institutions with the aim of gaining paid work in the future. This course is free for students age 18+ with an Education, Health and Care Plan. This is a work-based learning option that offers hands on opportunities to try out different job roles and work with a teacher and job coach to develop work skills and apply them in a real work situation. There are no qualifications attached to Project Search as the focus of the course is to help people into employment. All students will receive full support in their work and studies.

Calderdale Royal Hospital is the host business providing employment opportunities in various roles including reception, housekeeping, administration and portering. The students currently work in a variety of departments including the IT Department, Stroke Rehabilitation Wards, Resuscitation Training department and with ISS, one of the main contractors within the hospital. The course also focuses on supporting students to improve their interview skills, create a CV and register with job sites and complete application forms to better improve their chances of achieving paid work.

All learning takes place at a host employer with a College tutor and an HFT Job Coach supporting the teaching and skills development. Delivery methods include group work, role play, multi modal blended learning and practical activities in the classroom and out in the community.

About us

ISCAL is a Coaster Production Company. We produce a wide range of printed paper products such as, Coasters, Napkins, Dripmats, Hotplate Holders, Trayliners and many other products which can be viewed on our website.

We aim to give meaningful training and job opportunities to people with a range of disabilities. We provide a working manufacturing environment, in conjunction with basic skills training to act as a stepping stone to permanent employment.

We provide a programme of weekly employment focused workshops and practical support for Autistic adults 18+ to prepare, access, and maintain work placements, voluntary work, further training, and paid employment.

We offer Autism Specific group workshops and tailored 1-1 support for adults to develop skills and confidence to move into new opportunities. We provide support to create CVs, complete application forms, and engage in recruitment processes for work placements, voluntary work, paid employment, and further training. Individuals who are interested to join the workshops are encouraged to contact facilitator Nicole to discuss the referral process – due to the nature of the client group, workshops do not operate on a ‘drop in’ basis.

About us

As the name suggests, Halifax Opportunities Trust is based in Halifax in the Borough of Calderdale, West Yorkshire. Our projects span the length of Calderdale from the economic centre that is Halifax to the suburbs such as Boothtown and Siddal and valley towns of Sowerby Bridge, Hebden Bridge and Todmorden but our home is very much centred in the Park Ward area.

About us

Each year Café HX1 provides 6 adults (age 19+) with a learning disability an opportunity to develop their skills in the field of catering and hospitality. Each student completes 2 day practical work in a kitchen environment based at Higgins Close day service providing a meals and drinks service to approximately 40 people. The students also complete 1 day a week in a classroom setting developing their knowledge about food safety. The programme starts in September each year and concludes in August the next year. During the year long programme students develop their knowledge and skills with the aim of being better able to gain paid employment. The programme offers the opportunity to gain experience in other catering establishments in order to further confidence and skills.

We provide additional support from a vocational instructor and a catering support worker.

What we do

About us

We offer a range of structured activity sessions both within the activity centre and local community. Our aim is to develop activities and engagement opportunities for people with learning disabilities, bespoke to their needs and interests.

All aspects of occupation are important; we provide social, leisure, education, life skill development and employment as a platform for everyone.

Each person we support has an assessment and individualised plan. We encourage employment opportunities, and provide interview skills for those who aim to work; we actively pursue opportunities and support work retention.

We support adults, children and young people with learning and physical disabilities. We have a sensory room, computer room, music room and kitchen. Our facilities are accessible and include a tracking hoist system.

Our packages can be delivered on a 1:1 basis up to a 1:4 group session dependent on need. All ages are welcome between 9am and 4.30pm; extensions on request.

About us

We are a charity run organisation, established in 2006 to support adults age 19+ (currently 19-35) with learning disabilities and complex health needs on a 1:1 and 2:1 support ratio. Our objective is to provide a comprehensive day service along with individual support and exciting daily activities.

We also offer homecare and outreach to the young people who access our service. In 2017 we also opened a respite house where young people can spend a short break away from home. All centres as well as our respite accommodation are fully fitted with specialist equipment including hoists and changing facilities.

The Addy The Addy in Mixenden is a day service with a difference! Alongside our regular day service activities we also run a “Preparation for Employment” course which teaches people the skills needed to transition into work. The course offers people the chance to set realistic short and long term employment goals, the opportunity to develop self confidence and all round individual skills and specific training and experience in areas of career interest. People also get to complete a CV and job applications.

About us

We offer a variety of day opportunities for adults with a learning disability age 19+. The service offers support to individuals with high support needs either due to the severity of their learning disability, their health needs, needs around their behaviour or autism. The service offers an individualised package of support which can involve both building and community based activities. We are a volunteer befriending scheme who seek to match adults and young people aged over 16 with a learning disability and/or autism with a volunteer who likes the same things they do.

We offer regular social meet ups which include people who have not yet been matched where we go to local events including theatre, pubs and comedy shows to provide a way for people who might be socially isolated to mix with similar other people. Sport Services coordinate and deliver a wide range of disability specific and adapted mainstream sports sessions for children and young people aged 6–25 years.

Additional to our ‘out of school’ sessions, an inclusive PE and Sports Scheme operates during term time within primary and secondary schools, focusing on SEN pupils being fully included in curriculum PE & Sport activities.

All sessions and programmes are delivered by fully qualified sports coaches and support staff and are based in our Sports Centres and Swimming Pools throughout Calderdale.

Beat It

What we do

About us

Open to all skill levels, Beat It music sessions offer guided learning based on individual aspirations. Either in small group sessions or 1:1, join our inclusive music classes. Singing, drumming, songwriting, performing, our sessions cover many skills and are tailored to the individual/group. Private 1:1 sessions are also available. The preferred choice for some are 1:1 sessions (costing £30 per hour) which provide more focus on individual development. Group sessions run every Wednesday, 12:30-3:30pm and 5:30-8:30pm and 1:1 sessions can be arranged at a time to suit the individual.

The sessions are for adults and young people aged 16+ who have disabilities or mental health conditions. Within the umbrella of Imagineer, we also offer support brokerage, circles of support and management support.

Phoenix Radio offers young people the opportunity to broadcast on FM Radio and/or learn technical skills. We provide placements for school children age 14+ and volunteering from age 16. We encourage people to get as involved in broadcasting as possible as it is a great way to build confidence and develop communication skills.

We are a parent led organisation which offers support, guidance and a range of services to families of children with any kind of additional need aged 0-25 to help them lead ordinary lives.

We provide a broad range of services for parent carers ranging from coffee mornings/afternoon teas, training, sleep support from Sleep Fairy, Independent Support and self-advocacy sessions to social events, information and focus groups.

Family Voice Calderdale, the parent carer forum for Calderdale, is a project of Unique Ways. The forum works in partnership with local services to seek the views of parent carers to improve services.

We have a sensory Toy Library and are also a distributor for Max cards, which offer discounts on family days out.

The Specialist Inclusion Team is responsible for the support of children and young people with Autistic Spectrum Disorder, hearing and/or visual impairment from birth into adult services. It is one of several available for children and young people and comprises two teams:

About us

The Adult Disability Team forms part of the All Age Disability Service.

Our approach is focused on improving people’s lives, focussed on breaking down barriers for people and between agencies and services, and on making sure people’s happiness, aspirations and successes are never limited due to a disability. Our person centred approach will ensure people have experiences they value:

• developing relationships• making choices• contributing and having valued roles.

We support adults and young people who have a learning or physical disability to focus on building resilience and independence so that we can help adults to be as independent as they can be. Those adults have their rights promoted, for some this will include protecting them from all forms of abuse or discrimination and enable people to take appropriate risks and balance this with duty of care.

We have close links with Ravenscliffe, Ryburn, Calderdale College and the SEN team and meet with individuals before they leave education to ensure they are aware of what support is available to them. This does not always mean that individuals require support from Social Care, information may be given about the other services which are included in this brochure.

We are an education support service, operating predominantly during term time. We offer family support, assessment, training, teaching and advice to schools and settings for children and young people from 0 to 25.

The Autistic Spectrum Disorder Team offers a specialist service to mainstream schools, early years settings and colleges.

We provide support and advice for children and young people with a confirmed diagnosis of ASD, aged 0 to 25 years in Calderdale, to successfully access the learning and social environment, work towards independence and achieve their potential.
