What is the Self assessment?

The Joint Self- Assessment is a way of checking how well health and social care services are working for people with learning disabilities and their family members.

In the past we did this in two ways. These were the:•  Health Self-Assessment Framework•  The Learning Disability Partnership Board

 Everybody thought it would be a good idea to bring the two together.

Areas of the Self assessment form

Section A – Staying Healthy This includes lots of things such as getting a good service from the doctors, chemist, dentists or when going to hospital

Section B – Being Safe This includes lots of things such as feeling safe in their own home, in hospital or out and about where they live.

Section C – Living Well This is about lots of things including people feeling part of their community

What We Did

Partnership Board were informed about the self assessment at the October PB Meeting.

Commissioners and other professionals gave us information and shared stories and views about services and support for people with learning disabilities and their families.
