Should there be any concerns or special needs, please speak with us immediately. Also, you may want to make a checklist of important issues to consider and complete. The below items and information furnished by you and your family will allow us to more efficiently assist you in personalizing an experience that you would appreciate.

1) The things to remember. This booklet is for you to keep at home and record all the food, telephones calls, and flowers. Also it is used to record all the friends who assist you with various details before, suring and after the ceremony. Please refer to this booklet when using the acknowledgement cards after the services.

2) The information Fact Sheet. Please complete this form accurately because it will become a permanent document. Return this form to us during the arrangement Conference. (please print)

Reflections We realize this is a very difficult time for you and your family. We would like you to know that we’re here to help any way that we can. During this time, there are many decisions that must be made and numerous facts and documents that must be gathered. The enclosed documents are designed to help guide you through ta very personal planning process. Also, this folder is a central pocket in which to keep documents, photos, or important information that you will want to have with you over the next few days. No one can understand exactly what you are going through at this time but please know that you are not alone. We value the trust placed in us and we will honor your special request and needs as you personalize a meaningful ceremony. Below are some suggestions that may help you personalize the funeral service.

-Bring in favorite sons of your loved one and we will share assist you with playing them for the visitation and or/funeral.

-Use a collection of photographs spanning the live of your loved one. -Eulogize your loved one and share the story like no one else.

- Saying goodbye in a person al way by placing cherished items in the casket such as pictures, keepsake items, and handwritten notes.
