Founded in 2016 in Connecticut, Avon Family and Cosmetic Dentistry LLCwas formed in response to the needs of affordable, convenient and comfortable dentistry where every patient receives comprehensive and quality dental care. We ease the burden created by such obligations by performing time-consuming duties and services, creating time for more strategic planning.

We are building tradition of care and service at Avon Family and Cosmetic Dentistry LLC. We will expect no less than 100% from you as a team player and staff member. Changes in PolicyEffective Date: 02/01/2017

This employee handbook supersedes all previous memos, employee handbooks, and other written communication that may have been issued at any time on subjects covered in this employee handbook.

An effort is made to keep the employee manual current. However, as with all businesses, new rules and policies are frequently adopted. Such rules, when adopted, are fully applicable to the employment relationship, even if they are not yet reflected in the employee handbook. Any policies that are notified to the employees in an email or via a handout in the office is also considered applicable even if it is not yet reflected in this employee handbook.

Organizational change is inevitable and, as such, we reserve the right to interpret, modify, suspend, cancel, or dispute, with or without notice, all or any part of our policies, procedures, and benefits at any time. Changes will be effective on the dates determined by Avon Family and Cosmetic Dentistry LLC, and after those dates all previous policies will be null and void.

No individual supervisor or manager has the authority to alter policies in any way. Any employee who is unclear on any policy or procedure should consult a supervisor or HR Manager.

Policies and procedures introduced and practiced at Avon Family and Cosmetic Dentistry LLC are in place to safeguard everyone’s interest including employees, business vendors, patients and the business.

Non-compliance and non-adherence of these policies and procedures will be considered as a serious breach of business practices and failure to comply with the policies in any capacity or for any reason may lead to disciplinary action up to and including termination of employment.

Neither this manual nor any other practice document confers any contractual right, either express or implied, to remain an employee of the practice. Nor does it guarantee any fixed terms of conditions of your employment. Your employment is not for any specific time and may be terminated at will, with or without cause and without prior notice, by the practice, or you may resign for any reason at any time.

Second, the procedures, policies and benefits described here may be modified or discontinued from time to time. We will try to inform you of any changes as they occur, however it is an employee’s responsibility to periodically check and stay updated with changes in policies and procedures.

Third, this manual and information contained in it should be treated as confidential. No Portion of this manual should be disclosed to others, except practice employees and other affiliated with the practice whose knowledge of the information is required in normal course of business.

Nature of EmploymentEffective Date: 02/01/2017

Employment with Avon Family and Cosmetic Dentistry LLC is "at-will." This means employees are free to resign at any time, with or without cause, and Avon Family and Cosmetic Dentistry LLC may terminate the employment relationship at any time, with or without cause or advance notice. As an, at will-employee, it is not guaranteed, in any manner, that you will be employed with Avon Family and Cosmetic Dentistry LLC for any set period of time.

The policies set forth in this employee handbook are the policies that are in effect at the time of publication. They may be amended, modified, or terminated at any time by Avon Family and Cosmetic Dentistry LLC, except for the policy on at-will employment, which may be modified only by a signed, written agreement between the President and the employee at issue. Nothing in this handbook may be construed as creating a promise of future benefits or a binding contract. This Handbook is designed to acquaint you with Avon Family and Cosmetic Dentistry LLC Company and provide you with information about working conditions, employee benefits, and some of the policies affecting your employment. This Handbook is not a contract and is not intended to create any contractual or legal obligations. You should read, understand, and comply with all provisions of the Handbook. It describes many of your responsibilities as an employee and outlines the programs developed by Avon Family and Cosmetic Dentistry LLC to benefit employees. One of our objectives is to provide a work environment that is conducive to both personal and professional growth.

No Handbook can anticipate every circumstance or question about policy. As Avon Family and Cosmetic Dentistry LLC continues to grow, the need may arise and Avon Family and Cosmetic Dentistry LLC reserves the right to revise, supplement, or rescind any policies or portion of the Handbook from time to time as it deems appropriate, in its sole and absolute discretion. The only exception is our employment-at-will policy permitting you or Avon Family and Cosmetic Dentistry LLC to end our relationship for any reason at any time. The employment-at-will policy cannot be changed except in a written agreement signed by both you and the President of the Company. Employees will, of course, be notified of such changes to the Handbook as they occur.

Avon Family and Cosmetic Dentistry LLC employs only United States citizens and those non-U.S. citizens authorized to work in the United States in compliance with the Immigration Reform and Control Act of 1986.

Each new employee, as a condition of employment, must complete the Employment Eligibility Verification Form I-9 and present sufficient documentation establishing identity and employment eligibility. Former employees who are rehired must also complete the form, if they have not completed an I-9 with Avon Family and Cosmetic Dentistry LLC within the past three years, or if their previous I-9 is no longer retained or valid.

Confidentiality of Payroll InformationEffective Date: 02/01/2017

CONFIDENTIALITY OF PAYROLL INFORMATION

An employee's wages or salary information should be treated as confidential between the employee and the management.

Please be aware that discussion of your wages and salary can undermine the spirit of teamwork within the practice. Accordingly, we ask that you safeguard the confidentiality of your wage and salary information.Violation of this policy may lead to disciplinary action, demotion of wages and/or immediate termination of employment.

Avon Family and Cosmetic Dentistry LLC reserves and retains the right with sole and absolute discretion, to increase or decrease compensation at any time and in any amount, for any reason or for no reason.

Outside EmploymentEffective Date: 02/01/2017

Employees may hold outside jobs in unrelated businesses or occupations as long as the employee meets the performance standards of their position within Avon Family and Cosmetic Dentistry LLC. Unless an alternative work schedule has been approved by Avon Family and Cosmetic Dentistry LLC, employees will be subject to the organization's scheduling demands, regardless of any existing outside work assignments. This is the responsibility of employee to obtain any such approval in writing with specific dates and times from Avon Family and Cosmetic Dentistry LLC management at least 2 weeks in advance of anticipated working schedule at an outside job.

Avon Family and Cosmetic Dentistry LLC office space, equipment, computers, and materials are not to be used for personal purposes or purposes related to outside employment. Failure to comply may result in disciplinary action and or immediate termination of employment.

Staff MeetingsEffective Date: 02/01/2017

Avon Family and Cosmetic Dentistry LLC strongly believes in open communication among Management and all employees. As such, Avon Family and Cosmetic Dentistry LLC will conduct regular staff meetings. These meetings will provide an opportunity to inform employees of recent Avon Family and Cosmetic Dentistry LLC activities and initiatives, discuss organizational development and improvement, and recognize employees for outstanding effort and achievement.

Employee GrievancesEffective Date: 02/01/2017

It is the policy of Avon Family and Cosmetic Dentistry LLC to maintain a harmonious workplace environment that is free from intimidation or harassment. Avon Family and Cosmetic Dentistry LLC encourages its employees to express concerns about work-related issues, including workplace communication, interpersonal conflict, occupational health and safety, and allocation of work.

Employees are encouraged to raise such concerns with their supervisors on an informal basis in the first instance. If not resolved at this level, an employee must submit in writing a signed grievance immediately to the supervisor. This written grievance must: contain a brief statement of the facts or evidence surrounding the grievance; and include a proposed resolution.

After review of the written grievance, Avon Family and Cosmetic Dentistry LLC will hold a meeting with the employee, the immediate supervisor, and any other individuals who may assist in the investigation or resolution of the problem. All discussion of grievances will be confined to those involved with and who can assist with the resolution.

A written decision will be given to the employee stating the resolution and appropriate action regarding the grievance. The grievance is not considered resolved until both parties agree.

If two employees have any sort of ongoing disagreement, or if an employee has a grievance or disagreement with any of the management members/ supervisor, Avon Family and Cosmetic Dentistry LLC expects employees to bring the grievance to the management instead of discussing with co-workers and other employees as it results in several lost hours of productive work and negatively affects the morale of other employees. Such behavior will be considered against the interest of the business and the Dental office and may result in disciplinary actions and or immediate termination.

Avon Family and Cosmetic Dentistry LLC assures that all employees filing a grievance or complaint can do so without fear of retaliation or reprisal.

SecurityEffective Date: 02/01/2017

All employees who are issued keys are solely responsible for their safekeeping. These employees may be required to sign a Key Disbursement Form upon receiving the key. The last employee, or a pre-designated employee, to leave the premises at the end of the business day assumes responsibility for ensuring that all doors are securely locked, the alarm system is armed, thermostats are set on appropriate evening and/or weekend setting, and all appliances and lights are turned off with exception of lights regularly left on for security purposes.

Employees are not permitted on Avon Family and Cosmetic Dentistry LLC property after hours without prior authorization from HR Manager. These employees must demonstrate a legitimate need to be on the premises.

The employer cannot be held responsible for the safety of employees’ personal belongings.

Internal CommunicationEffective Date02/01/2017

Effective and ongoing communication within Avon Family and Cosmetic Dentistry LLC is essential. As such, the organization maintains systems through which important information can be shared among employees and management. Communication between staff at Avon Family and Cosmetic Dentistry LLC within office premises may be recorded or documented in order to maintain a safe environment for everyone and to ensure proper usage of the policies in place.

Bulletin boards are posted in designated areas of the workplace to display important information and announcements to facilitate communication and share access to documents.

Employees may correspond and send files via e-mail. For more information on the appropriate usage of e-mail in the workplace, employees may refer to the Internet Usage and Computer Usage policies.

All employees are responsible for checking internal communications on a frequent and regular basis and responding as necessary. Internal communication between employees using emails, phone calls or any other medium has to be related to the operations & workings within Avon Family and Cosmetic Dentistry LLC and cannot be of a personal nature.

Background ChecksEffective Date: 02/01/2017

In order to protect the welfare of the clients, customers, employees, and resources of Avon Family and Cosmetic Dentistry LLC, it is the policy of Avon Family and Cosmetic Dentistry LLC to conduct background checks on those applicants and employees with a job-related need. Background checks may include, but are not limited to: reference checks, credit history checks, driving record checks, criminal record checks, and education and credential verification.

Applicants and current employees seeking promotional opportunities or otherwise qualifying for a background investigation must consent to and pass a background check prior to any final appointment or promotion decisions relating to full-time, part-time, and temporary positions.

Effective Date: 02/01/2017

It is the intent of Avon Family and Cosmetic Dentistry LLC to clarify the definitions of employment classifications so that employees may understand their employment status and benefit eligibility. Placement in any of these classifications does not guarantee employment in any way.

Under the Fair Labor Standards Act (FLSA), each employee is designated as either non-exempt or exempt from federal and state wage and hour laws. Nonexempt employees are entitled to overtime pay under the specific provisions of federal and state laws. Non-exempt employees are those whose positions do not meet FLSA criteria for exemption and who are paid for hours worked in excess of 35 hours per week. Exempt employees are excluded from specific provisions of federal and state wage and hour laws. An employee's exempt or nonexempt classification may be changed only upon written notification by Avon Family and Cosmetic Dentistry LLC. In addition to the above classifications, each employee will belong to one other employment classification:

EXEMPT:

Employees whose position meet specific tests established by the Fair Labor Standards Act (FLSA) and applicable stae law and who are exempt from overtime pay requirements.

NON-EXEMPT:

Employees whose postion do not meet FLSA and state exemption tests and who receive additional pay for hours worked in excess of forty per week.

FULL-TIMEFull-time employees are those who are regularly scheduled to work 35 or more hours per week. Generally, they are eligible for Avon Family and Cosmetic Dentistry LLC's benefit package, subject to the terms, conditions, and limitations of each benefit program.

PART-TIMEPart-time employees are those who are regularly scheduled to work less than 35 hours per week. Regular part-time employees are eligible for some benefits sponsored by Avon Family and Cosmetic Dentistry LLC, subject to the terms, conditions, and limitations of each benefit program.

TEMPORARYTemporary employees include those hired as interim replacements to assist in a specific function or in the completion of a specific project. Employment beyond any initially stated period does not in any way imply a change in employment status. Temporary employees retain temporary status unless and until they are notified, by Avon Family and Cosmetic Dentistry LLC Management, of a change. They are not eligible for any of the Avon Family and Cosmetic Dentistry LLC's benefit programs. Interns are classified as

Performance Evaluations & Performance of workEffective Date: 02/01/2017

Performance evaluations are normally conducted annually from the date of hire. All performance evaluations are based on merit, achievement, and performance measured against job objectives.

Wage increases will be based upon this review, as well as past performance and degree of improvement; dependability; attitude; cooperation; need for disciplinary action; adherence to all employment policies; and employee status within a given pay range. Supervisors will review and discuss pay range and status within that range during performance evaluations. If promoted, employees may be eligible for an increase as dictated by the pay range of the new position.

An annual performance evaluation does not guarantee raise in the wage and is determined by the management based on an employee’s overall performance through the year in addition to the above-mentioned points.

Supervisors will conduct performance evaluations and planning sessions with all full-time and part-time employees. Supervisors may conduct informal performance meetings and planning sessions more often if they choose.

Performance evaluations and planning sessions are designed for the supervisors and employees to discuss current job tasks; encourage and recognize achievement; and discuss positive, purposeful approaches for meeting professional goals. Together, supervisors and employees create action plans to improve performance and develop skills.

Performance of work: An employee is expected to perform all assigned tasks for which he or she is qualified, competent, and legally authorized. This extends to the full range of tasks pertinent to the employee’s position. It also includes the proper maintenance of patient charts and records, as well as avoiding patient neglect. All registrations, certifications, permits, continuing education, or other governmental authorizations must be kept current. Employees must comply with all statutes and regulations (including, but not limited to, OSHA and HIPAA) applicable to this office. It is employee’s responsibility to periodically update themselves with provided OSHA/ HIPAA Manuals.

Personnel FilesEffective Date: 02/01/2017

Employee personnel files maintained by Avon Family and Cosmetic Dentistry LLC include, but are not limited to the following:

⦁ resume

⦁ training documentation

⦁ salary history

⦁ disciplinary reports, warnings, and other records

⦁ performance evaluation documentation

⦁ records of coaching and/or mentoring

Personnel files are the sole property of Avon Family and Cosmetic Dentistry LLC and access to information contained therein is restricted. Management personnel of Avon Family and Cosmetic Dentistry LLC with a legitimate reason to view the file will be permitted to do so.

Employees who wish to view their own files should contact HR Manager. With reasonable advance notice, employees may view their own personnel files on Avon Family and Cosmetic Dentistry LLC premises and in the presence of HR Manager.

It is the responsibility of each employee to promptly notify their supervisor or HR Manager of any changes in personal data. Such changes may affect your eligibility for benefits, the amount you pay for benefit premiums, and your receipt of important company information.

Expense ReimbursementEffective Date: 02/01/2017

Employees must be granted prior written approval before incurring any company expenses. Reimbursement under $100 will be included in the employee's next regular paycheck. If the reimbursement amount is in excess of $100, the expense report or request for reimbursement will be processed as an invoice. All completed reimbursement requests should be submitted in a timely manner to their manager.

Termination of EmploymentEffective Date: 02/01/2017

Termination of employment is an inevitable part of personnel activity within any organization, and many of the reasons for termination are routine. Common circumstances under which employment is terminated include the following:

⦁ Resignation - Voluntary employment termination initiated by an employee.

⦁ Termination - Involuntary employment termination initiated by Avon Family and Cosmetic Dentistry LLC. In most cases, Avon Family and Cosmetic Dentistry LLC will use progressive disciplinary actions before dismissing an employee. However, certain actions warrant immediate termination.

⦁ Layoff - Involuntary employment termination initiated by Avon Family and Cosmetic Dentistry LLC for non-disciplinary reasons.

⦁ Retirement - Voluntary employee termination upon eligibility for retirement.

Nonexempt employees who intend to terminate employment with Avon Family and Cosmetic Dentistry LLC, shall provide Avon Family and Cosmetic Dentistry LLC with at least two weeks written notice. Exempt employees shall provide at least four (4) weeks written notice. Such notice is intended to allow the Company time to adjust to the employee’s departure without placing undue burden on those employees who may be required to fill in before a replacement can be found.

Since employment with Avon Family and Cosmetic Dentistry LLC is based on mutual consent, both the employee and Avon Family and Cosmetic Dentistry LLC have the right to terminate employment at will, with or without cause, during the Introductory/ Trial period.

In the case of employee termination, the employee will receive their accrued pay in accordance with all federal, state and local laws.

Any employee who terminates employment with Avon Family and Cosmetic Dentistry LLC shall return all files, records, keys, and any other materials that are property of Avon Family and Cosmetic Dentistry LLC.

Contact with Patients and the Public

This office expects every employee to display good judgment, diplomacy, and courtesy when dealing with patients and the public, and to adhere to the highest standards of personal and professional ethics.

With that in mind, every person must be offered every courtesy when contacting the office, whether in person, by mail, or by telephone. Employees should handle all contact efficiently and promptly. It is especially important that telephone calls be returned without delay, on the same day whenever possible. Arrogance or rudeness from a staff member will not be tolerated.

Avon Family and Cosmetic Dentistry LLC considers staff sitting at the front desk as “The Face of Avon Family and Cosmetic Dentistry LLC”. Keeping that in consideration, staff at front must be neatly groomed and professionally dressed at all times. The reception area must be staffed at all times during office hours. No persons other than employees are permitted behind the reception desk. Patients should be greeted promptly and courteously by the person sitting at the reception desk. Product vendors, charity collectors etc are not permitted to solicit in the dental office and they can leave their card or information with the receptionist and should not be encouraged to solicit at the dental office during their visit.

Patient Confidential InformationWhen discussing sensitive information with the patient regarding their

treatment, accounts or any other information that may jeopardize patient privacy, employees are expected to use their judgment and ensure that the conversation is only heard between the employee and the patient in keeping with HIPAA regulations. It is employees responsibility to stay up to date with HIPAA and OSHA regulation manuals available at the front desk in the office.

Careful custody and handling of patient information and documents or materials containing confidential information or trade secrets are of great importance. Each employee is responsible for safeguarding against theft, loss, unauthorized use, or disclosure of this information. Therefore, all employees must take whatever steps are necessary to ensure that all documents and information are handled, stored, transmitted or destroyed in a manner that will preserve confidentiality and eliminate loss or misuse.

To safeguard patient information, lock computer screens when walking away from your desk, put away checks and sensitive mail in safe custody until these items are processed. Failure to safeguard patient information and violation of HIPAA regulations may result in disciplinary action including instant termination.

SafetyEffective Date: 02/01/2017

Avon Family and Cosmetic Dentistry LLC strives to provide a clean, hazard-free, and safe environment for employees in accordance with the Occupational Safety and Health Act of 1970. Each employee is expected to take an active role in maintaining this safe environment. Employees must observe all posted safety rules, adhere to all safety instructions, and properly use safety equipment when required. Workspaces must be kept neat, clean and organized.

It is the responsibility of employees to familiarize themselves with the location of all safety and emergency equipment, as well as safety contact phone numbers. All safety equipment will be provided by Avon Family and Cosmetic Dentistry LLC, and employees are responsible for the reasonable upkeep of this equipment. Any problems with, or defects in, equipment should be reported immediately to a supervisor.

Each employee is required to comply with all safety rules observed by Avon Family and Cosmetic Dentistry LLC, assist in maintaining a hazard-free environment, immediately inform management of any accidents or injuries, and report any unsafe equipment, working conditions, processes, procedures, or potential breach of safety to a supervisor.

Any employee may report safety violations or injuries anonymously to a supervisor, if he or she is not the injured or violating party. No employee will be punished or reprimanded for reporting safety violations or hazards.

Any deliberate or ongoing safety violation, or creation of a safety hazard by an employee, will be dealt with thorough disciplinary action by Avon Family and Cosmetic Dentistry LLC, up to and including suspension or termination.

Work-related accidents are covered by Workers' Compensation Insurance, pursuant to all applicable state and local laws.

In the case of an accident that results in injury, regardless of how minor the injury may appear, employees must notify their supervisor.

Company HoursEffective Date: 02/01/2017

Avon Family and Cosmetic Dentistry LLC is open for business, Monday through Friday and on some Saturdays. The hours vary each day based on patient schedule for that day. This schedule is subject to change any time with or without prior notice in keeping with the business need or based on change in our patient base and their needs.

The standard workweek for full-time employees is 35 hours of work, and less than 35 hours for part-time employees. For the purposes of calculating employee benefits and pay periods, the workweek is considered Sunday at 12:00 a.m. through Saturday at 11:59 p.m.

To ensure meeting the needs of patients and goals of practice, it is imperative that the office is staffed at all times during these office hours.

From time to time, we may need to change your schedule to meet scheduling or other needs. Notice will be given as far in advance as possible.

If you must leave work early for any reason, notify the dentist on premises and make your best effort to contact HR Manager immediately. You are expected to work until end of each day; patients are best served by having the staff available during the entire time for which the office opens. If extraordinary circumstances prevent a staff from working their full shift, they will be paid only for the hours they are present in the office.

Avon Family and Cosmetic Dentistry LLC DOES NOT have fixed hours for employees and in the event that the office has to close due to inclement weather or lack of patients scheduled or any other unforeseen reasons, employees will only be paid for the hours worked and will not be paid for the day/s when the office is closed or closes earlier than scheduled.

Vehicles parked in spaces designated for use by other businesses in the plaza will be towed at the owner's expense.

Avon Family and Cosmetic Dentistry LLC Employees are encouraged to park in the farther parking spots whenever possible and leave the front parking spots available for our patients and clients.

Smoking & Tobacco Use PolicyEffective Date: 02/01/2017

Avon Family and Cosmetic Dentistry LLC endeavors to provide a healthy environment and therefore prohibits smoking and tobacco use in company buildings.

The use of tobacco products is not permitted anywhere on Avon Family and Cosmetic Dentistry LLC premises. Avon Family and Cosmetic Dentistry LLC employees are also not permitted to use Tobacco products immediately outside office premises in keeping with the nature of the business and respect for our patients. Due to the nature of a Dental office business, employees work in close proximity to the patient. All employees are urged to use tobacco products after their working hours are over for the day.

Any complaints from co-workers or our patients of Tobacco smell from the clothing, uniforms, mouth etc will not be tolerated and may result in disciplinary action or instant termination.

LunchEffective Date: 02/01/2017

Avon Family and Cosmetic Dentistry LLC has a staggered lunch policy. Employees will have to stagger their lunch times with their co-workers, for example starting & quitting times, in order to cover Front end of the office and back end of the office all hours that the office is open.

Full-time employees are provided one thirty (30) minutes meal period each workday. On certain busy days or days when office is short staffed, supervisor will schedule meal periods to accommodate business needs. Employees will be relieved of all active responsibilities and restrictions during meal periods and, as such, will not be compensated for that time. Meal periods at Avon Family and Cosmetic Dentistry LLC are unpaid.

Front office is required to be covered at all times during business hours and staggered lunch times between staff are non-negotiable.

Rest PeriodsEffective Date: 02/01/2017

Based on the number of hours worked, two (2) rest breaks are allowed per day besides the meal period. Each of these two breaks may not be longer than ten (10) minutes.

These breaks must be taken at separate intervals and must have a minimum of four hour gap in-between. These breaks cannot be combined with the meal period. Two employees from the same department may not take a break together. One of them must be available to cover that department in absence of the other employee.

On busy days and when office is short staffed, dentist

may need to schedule Two (2) rest period breaks to accommodate business needs. Rest period breaks are unpaid breaks.

In the situation when the office is understaffed, employee must try and take these breaks during less busy times in the office and keeping in consideration the patient schedule and smooth functioning of the Dental office.

Reporting InjuriesEffective Date: 02/01/2017

All injuries involving time loss must be reported to the division of Labor Statistics and Research. Serious injuries, illnesses or deaths must be reported to the Management personnel as soon as possible. Injury reports will be filed by the employer (Office Manager) with the insurance carrier within 24 hours of the injury.

If an employee suffers a work-related injury, it is the employee’s responsibility to report such injury immediately to the Office Manager. Avon Family and Cosmetic Dentistry LLC assumes no responsibility if any health hazard is caused due to lack of timely reporting of the injury by the employee in question.

Workplace rules and standards of conduct observed by Avon Family and Cosmetic Dentistry LLC are essential to a healthful and productive workplace. All employees are urged to familiarize themselves with these rules and standards, as they will be held accountable for upholding them. Please be aware that any employee who disregards, or deviates, from any of these rules or standards will be subject to disciplinary action, up to and including termination of employment.

While not intended to be an all-inclusive list, the examples below represent behavior that is considered UNACCEPTABLE in the workplace. Behaviors such as these, as well as any other misconduct, will not be tolerated. The below misconducts do not qualify for “Progressive Discipline Steps” and such conduct will result in instant termination.

⦁ Working under the influence of alcohol or illicit drugs

Disciplinary Action

(Progressive Steps Disciplinary Program)

Effective Date: 02/01/2017

Disciplinary action at Avon Family and Cosmetic Dentistry LLC is intended to fairly and impartially correct behavior and performance problems early on and to prevent reoccurrence. At Avon Family and Cosmetic Dentistry LLC we follow progressive step disciplinary program.

As a first step of Disciplinary action, and upon first occurrence of an incident, a Verbal warning (in writing, signed by employee & employer) is given to an employee, if a second occurrence of the same nature is seen, then a Written warning (in writing, signed by employee & employer) is given to the employee in question. Upon a third occurrence of the same nature employee may be instantly terminated. A corrective action will be written in each of these warnings by the employer to help employee understand, behavior or performance modifications required as a corrective measure.

Employee’s signature on any of the warnings/ write-ups only signifies Acknowledgement of Receipt of the warning and does not necessarily signify employee’s agreement with the written warning.

Avon Family and Cosmetic Dentistry LLC reserves the right to administer disciplinary action at its discretion and based upon the circumstances.

Avon Family and Cosmetic Dentistry LLC recognizes that certain types of employee behavior are serious enough to justify either a suspension or, in extreme situations, termination of employment, without observing other disciplinary action first.

These violations include but are not limited to:

⦁ Workplace violence

⦁ Harassment

⦁ Theft of any kind

⦁ Insubordinate behavior

⦁ Vandalism or destruction of company property

⦁ Directing Avon Family and Cosmetic Dentistry LLC Staff or co-workers to make changes to office/patient schedule without written permission from management

Attendance and PunctualityEffective Date: 02/01/2017

Employees are expected to arrive at work prior to their scheduled start times so that they may begin work at their scheduled start times. All time off must be requested in writing, in advance, as outlined in Avon Family and Cosmetic Dentistry LLC Paid Time Off (PTO) policy.

Avon Family and Cosmetic Dentistry LLC considers excellent attendance to be the foundation of great performance and, as such, attendance is assessed during performance reviews. All unapproved absences will be documented in employee files. Excessive absence will result in disciplinary action, up to and including termination.

Absenteeism and tardiness place an undue burden on other employees and on the organization itself; Avon Family and Cosmetic Dentistry LLC expects regular attendance and punctuality from all employees.

Employees unable to report for work for any reason must notify management as early as possible, but always before their scheduled start time. Employees are responsible for notifying their manager directly of absence or tardiness. It is not acceptable to leave a message on a supervisor's voice mail, except in extreme emergencies. In the case of leaving a voice-mail message, a follow-up call or email must be sent later that day.

Should occurrences of absence or tardiness become regular or excessive, disciplinary action may be taken, up to and including termination of employment.

Requests for special work hours will be considered on a case-by-case basis, depending on both the needs of the employee and of Avon Family and Cosmetic Dentistry LLC.

Employees are expected to:

1. Be at work as scheduled;

2. Be on time, ready to begin work at the start of the shift and following lunch and other breaks;

3. Work a full shift including any scheduled or emergency overtime as may be needed;

4. Be certain that when an absence occurs, it is for unavoidable reasons;

If you are unable to report to work for any reason, if you will arrive late, or you must leave early, notify the HR Manager as soon as possible.

Internet access to global electronic information resources on the World Wide Web is provided by Avon Family and Cosmetic Dentistry LLC to assist employees in obtaining work-related data and technology. The guidelines in this policy have been established to help ensure responsible and productive Internet usage.

All Internet data that is composed, transmitted, or received via our computer communications systems is considered Avon Family and Cosmetic Dentistry LLC’s property and, as such, is subject to disclosure to law enforcement agencies or other appropriate third parties. Consequently, employees must always ensure that the information contained in Internet e-mail messages and other transmissions is accurate, appropriate, ethical, and lawful.

The equipment, services, and technology provided to access the Internet remain at all times the property of Avon Family and Cosmetic Dentistry LLC. As such, Avon Family and Cosmetic Dentistry LLC reserves the right to monitor Internet usage, retrieve and read any data composed, sent, or received through our online connections and stored in our computer systems.

Data that is composed, transmitted, accessed, or received via the Internet may not contain content that may be considered discriminatory, offensive, obscene, threatening, harassing, intimidating, or disruptive to any employee or other person. Employees using Avon Family and Cosmetic Dentistry LLC equipment are forbidden to access any restricted services or websites. These include, but are not limited to, those that promote or relate to pornography, gambling, illegal activity, racism, promotion of violence, or gender degradation. Violation of these terms may result in disciplinary action, up to and including termination of employment.

Any web portal or sites which are not used for performing your job duties including but not limited to chatting sites, messengers, social media sites like Twitter and Facebook are strictly prohibited. Any violation of internet usage policy may lead to immediate termination of employment.

Abuse of Internet access provided by Avon Family and Cosmetic Dentistry LLC in violation of law or Avon Family and Cosmetic Dentistry LLC policies will result in disciplinary action, up to and including termination of employment. Employees may also be held personally liable for any violations of this policy.

In an effort to respect the rights of all employees and avoid disruption in the workplace, employees must inform family members and friends to limit personal telephone calls during working hours. No long distance calls are to be made on company phones which are not strictly business related.

Any employee found in violation of this policy will be subject to disciplinary action, up to and including termination.

Cell Phone Usage

Cell Phone Use Policy

1. Purpose: The purpose of this policy limiting the use of cell phones and other communication devices at work is to protect you. Inappropriate use of communication devices at work can cause injuries because it’s distracting and may interfere with the proper and safe use of equipment and machinery. Devices and headphones or wireless ear pieces may also get tangled in machinery or interfere with the proper use of personal protective equipment.

2. Devices Covered: The devices covered by this Policy include cell phones, Blackberries, mobile phones, text pagers, two-way radios and other wireless devices, whether owned by the Company or the individual worker (collectively referred to as “Devices”).

3. Persons Covered: This Policy applies to workers, contractors, consultants, temporary workers and other workers at the Company, including all personnel affiliated with third parties working at Company facilities.

4. Activities Covered: The rules set out in this Policy apply to all work-related activities, including but not limited to driving to and from work and to conduct job-related activities, whether such vehicles are owned by the Company or the worker. The Policy applies to all conversations, whether personal or business-related.

5. Prohibited Uses

a. General. While in the workplace during work hours, workers are expected to focus on work and may not inappropriately use any Device in the workplace for any inappropriate purposes, including but not limited to:

• Engaging in personal conversations;

• Playing games;

• Surfing the internet;

• Checking e mail; and

• Sending or receiving text messages.

b. Driving. While operating a vehicle, workers may not answer a communication device unless and until they pull over in a safe spot (or let a passenger answer the call). If it’s urgent, workers may accept or return the call, provided that they remain parked off the roadway. They may not resume driving until their conversation is over. Workers may not make outgoing calls while driving. If workers need to place a call, they must first pull over in a safe spot.

6. Permitted Uses: Workers may use Devices while they’re not working in the following designated areas: break room, lunch room or outside of office premises. Use of hands-free devices while driving is not permissible.

7. Violations: Workers who violate this policy will be subject to disciplinary measures up to and including dismissal, depending on the circumstances.

Cell phones must be turned off or put on a silent mode during business hours. No cell phones are allowed on the working desks, lab area and operatories. Employee must provide the office number as the emergency contact number to their family members, Daycare providers or other contacts in case of an emergency.

Cell phones may be used in the kitchen area during lunch breaks and/or two (2), ten minute rest breaks.

Violation of the cell phone usage policy will lead to “Progressive Disciplinary steps” for the employee. For understanding the disciplinary steps, please refer to the definition of “Progressive Disciplinary Steps Program” page.

Only authorized persons may purchase supplies in the name of Avon Family and Cosmetic Dentistry LLC. No employee whose regular duties do not include purchasing shall incur any expense on behalf of Avon Family and Cosmetic Dentistry LLC or bind Avon Family and Cosmetic Dentistry LLC by any promise or representation without express written approval.

Emergency/ Inclement weather Closings:

On occasion, emergency situations - such as severe weather, power failure, or natural disaster - may disrupt company operations. The decision to close or delay regular operations will be made by Avon Family and Cosmetic Dentistry LLC management. If a decision is made to close or delay regular operations, employees will receive notification from Avon Family and Cosmetic Dentistry LLC via email or text.

Other Closings:

Dental office may experience full day, partial day or several days closings due to dentist being on vacation, patient cancellations, changes in patient schedules and other unforeseen circumstances.

Time off from scheduled work due to any closings will be unpaid for all employees. During Inclement weather if an employee requests to leave early, and the request is granted, they will only be paid for time worked. If the management decides to cancel patients and close the office due to any reason, employees will only be paid for the time worked.

Medical EmergenciesEffective Date: 02/01/2017

In the event that an employee requires medical attention due to injury or illness while at work, the employee's emergency contact (usually a family member) will be contacted to transport the employee to the appropriate facility.

If an emergency arises that requires an emergency medical service to evaluate the injury/illness of an employee on-site, the employee will be responsible for any charges incurred of an ambulance, medical bills etc. Furthermore, no other Avon Family and Cosmetic Dentistry LLC employee will be responsible for transporting another employee to an emergency facility, due to safety and liability issues.

A physician's "permission to work" notice may be required prior to the employee's return to work.

Employees of Avon Family and Cosmetic Dentistry LLC are to present a clean and professional appearance at all times when representing the organization. Management, sales personnel and those employees who have regular contact with our patients and the public are expected to dress in accepted corporate attire. Employees should always be well groomed and observe high standards of personal hygiene.

While not intended as an all-inclusive list, the examples below are considered inappropriate dress in the workplace:

⦁ Open-toed sandals or shoes

⦁ Spaghetti-strapped shirts

⦁ Tank tops or revealing shirts

⦁ Short skirts

⦁ Sneakers

⦁ Jeans or denim pants

⦁ Gym pants or gym leggings

If management occasionally designates "casual days," your supervisor will inform you of what type of dress will be considered casual.

You are expected to have a neat appearance at all times, every day during business hours. Hair should be neatly groomed and your appearance should be professional and presentable as per the business. Business appropriate hairstyling and hair grooming is expected.

Fragrances: Hair Sprays, styling gels, body fragrances are discouraged in keeping with patient care.

Nails: Artificial nails, which include any product applied to the natural nail other than polish, are prohibited. These products include, but are not limited to tips, wraps, extenders, appliques, acrylics, gels and nail jewelry. While nail polish is permitted, clear polish is preferred. Nails should be well manicured and nail length should not interfere

Any questions regarding what is considered appropriate attire should be directed to a supervisor or member of the management. UniformsEffective Date: 02/01/2017

Uniform coats provided by Avon Family and Cosmetic Dentistry LLC are part of the office uniform, keeping in mind the hygiene required in the Dental office. Those coats are mandatory while in the office during business hours. While Avon Family and Cosmetic Dentistry LLC takes the responsibility to have the uniforms dry cleaned/ laundered periodically, employees are responsible to maintain a neat uniform and appearance at all times during business hours.

Uniforms are the property of Avon Family and Cosmetic Dentistry LLC and are returnable upon termination of employment whether by employer or employee.

Avon Family and Cosmetic Dentistry LLC will not, under any circumstances, condone, tolerate, or allow conduct which may constitute sexual harassment on the part of its management, supervisors or non-management employees. It is our policy that all employees have the right to work in an environment free from unlawful harassment of any kind.

Any employee found to be engaged in any form of sexual harassment may be subject to disciplinary action, up to and including termination of employment.

Sexual harassment may be defined as:

⦁ Conditioning employment upon employee's submission to unwelcome sexual advances or requests for sexual favors;

⦁ Basing an employment decision upon an employee's submission or rejection of unwelcome sexual advances, requests for sexual favors or verbal or physical conduct of a sexual nature; or

⦁ Creating an intimidating, hostile or offensive working environment or atmosphere through:

Any form of retaliation is considered a direct violation of this policy and may result in disciplinary action, up to and including termination of employment. Avon Family and Cosmetic Dentistry LLC considers drug and alcohol abuse a very serious problem, and is committed to maintaining a work place free of substance abuse. This policy applies to all employees of Avon Family and Cosmetic Dentistry LLC without exception, including part-time and temporary employees.

No employee is allowed to consume, possess, sell, or purchase any alcoholic beverage on any property owned by or leased on behalf of Avon Family and Cosmetic Dentistry LLC, or in any vehicle owned or leased on behalf of Avon Family and Cosmetic Dentistry LLC. No employee may use, possess, sell, transfer or purchase any drug or other controlled substance which may alter an individual's mental or physical capacity in any way. Exceptions are aspirin or ibuprofen based products, as well as legal drugs which have been prescribed to that employee and are being used in the manner prescribed.

Avon Family and Cosmetic Dentistry LLC will not tolerate employees who report for duty while impaired by use of alcohol or drugs.

All employees should report evidence of alcohol or drug abuse to their supervisor or HR Manager immediately. In cases in which the use of alcohol or drugs creates an imminent threat to the safety of persons or property, employees are required by Avon Family and Cosmetic Dentistry LLC to report the violation. Failure to do so may result in disciplinary action.

Employees found in violation of Avon Family and Cosmetic Dentistry LLC’s Drug and Alcohol Abuse Policy will be subject to disciplinary action, up to and including termination. It is our policy at Avon Family and Cosmetic Dentistry LLC to assist employees and family members who suffer from drug or alcohol abuse. Employees may be eligible for a leave of absence, are encouraged to contact HR Manager for details.

As a part of our effort to maintain a workplace free of substance abuse, Avon Family and Cosmetic Dentistry LLC employees may be asked to submit to a medical examination and/or clinical testing for the presence of alcohol and/or drugs. Within the limits of state and federal laws, we reserve the right to examine and test for drugs and alcohol at our

Avon Family and Cosmetic Dentistry LLC has guidelines to avoid real or potential conflicts of interest. It is your responsibility as an employee of Avon Family and Cosmetic Dentistry LLC to follow the guidelines included herein. Questions or concerns regarding this policy should be directed toward your supervisor or the HR Manager.

Employees should not allow their responsibilities outside of work to create a conflict of interest. Avon Family and Cosmetic Dentistry LLC does not automatically assume that there is a conflict of interest when an employee has a relationship with another company or Dental Office. However, as an employee at Avon Family and Cosmetic Dentistry LLC you may not take information of Avon Family and Cosmetic Dentistry LLC workings, systems, patient data, modus Operandi, marketing strategies and or any other information pertaining to Avon Family and Cosmetic Dentistry LLC to any outside Dental office or organization. You musr inform Avon Family and Cosmetic Dentistry LLC if you foresee any such conflict of interest. By informing us that there is the possibility of an actual or potential conflict of interest, we can establish safeguards to protect everyone involved.

In cases where your family members are employed by, or serve on committees of, our suppliers or competitors you are required to notify Avon Family and Cosmetic Dentistry LLC to make a formal notice of such association. By informing us that there is the possibility of an actual or potential conflict of interest, we can establish safeguards to protect everyone involved.

An actual or potential conflict of interest occurs when you are in a position to influence a decision or have business dealings on behalf of Avon Family and Cosmetic Dentistry LLC that might result in a personal gain for you or for one of your relatives or friends. For this purpose, a relative is any person who is related to you by blood or marriage, or whose relationship with you is similar to being a relative even though they are not related by blood or marriage.

The possibility for personal gain is not limited to situations where you or your relative has a significant ownership in a firm with which Avon Family and Cosmetic Dentistry LLC does business. Workers' CompensationEffective Date: 02/01/2017

Employees who are injured on the job at Avon Family and Cosmetic Dentistry LLC are eligible for Workers' Compensation benefits. Such benefits are provided at no cost to employees and cover any injury or illness sustained in the course of employment that requires medical treatment.

Employees who sustain work-related injuries or illnesses must notify their supervisor immediately so that Avon Family and Cosmetic Dentistry LLC can notify the workers' compensation insurance carrier as soon as possible.

Lost time or medical expenses incurred as a result of an accident or injury which occurred while an employee was on the job will be compensated for in accordance with workers' compensation laws.

All job-related accidents or illnesses must be reported to an employee's supervisor immediately upon occurrence. Supervisors will then immediately contact the HR Manager to obtain the required claim forms and instructions.

Social Security / MedicareEffective Date: 02/01/2017

Avon Family and Cosmetic Dentistry LLC withholds income tax from all employees' earnings and participates in social security and Medicare withholding and matching programs as outlined in the Federal Income Insurance Contribution Act (FICA).

Old Age and Survivors' Insurance, commonly referred to as social security, became effective January 1, 1937, and provides benefits for employees after they retire or otherwise qualify under the provisions of the Act. The plan is designed to protect the future security of employees and their dependents.

In accordance with FICA, a required percentage of each employee's salary is deducted from their paycheck; the company matches employees' deductions dollar for dollar.

Employees are encouraged to contact their local Social Security office to find out more about their Social Security benefits, including a record of your past wages that were subject to Social Security taxes.

Jury DutyEffective Date: 02/01/2017

Avon Family and Cosmetic Dentistry LLC encourages employees to fulfill their civic responsibilities by serving jury duty when required. Employees must provide their immediate supervisor with a copy of their jury summons as soon as possible so that the supervisor may make arrangements to accommodate their absence.

Employees on jury duty must report to work on workdays, or parts of workdays, when they are not required to serve. Either Avon Family and Cosmetic Dentistry LLC or the employee may request an excuse from jury duty if it is determined that the employee's absence would create serious operational difficulties.

Avon Family and Cosmetic Dentistry LLC only pays employees for hours worked however, Jury duty will be paid if required by applicable state law. If paid, jury duty pay will be calculated on the employee's base pay rate times the number of hours the employee would otherwise have worked on the day of absence.

Avon Family and Cosmetic Dentistry LLC’s bereavement leave policy allows some time off to eligible employees for a death in the immediate family. If the time off required is any more than 90 days, it may become necessary to for the Avon Family and Cosmetic Dentistry LLC to convene with the employee and mutually decide further course of action.

Employees in the following employment classification(s) are eligible for bereavement leave:

⦁ Full-time employees

You may request up to an additional 2 days, which must be approved by your immediate supervisor and Avon Family and Cosmetic Dentistry LLC management.

Bereavement leave for a death outside of an employee's immediate family must be approved by his or her immediate supervisor and Avon Family and Cosmetic Dentistry LLC management.

Personal leave if granted by the employer for personal business that cannot be conducted at times other than during regular working hours. Avon Family and Cosmetic Dentistry LLC encourages employee to keep their personal leave time to a minimum as a record is kept for personal time taken and is taken into consideration during annual employee review.

Such reviews may be conducted more frequently under some circumstances (i.e. for a newly created position or following a recent promotion).

Accurately recording time worked is the responsibility of every employee. Time worked is the time actually spent on a job performing assigned duties. In some cases, employees may be responsible for accurately documenting time spent on individual jobs.

Please note that Avon Family and Cosmetic Dentistry LLC does not include time spent on personal matters or extended breaks when calculating hours worked.

Timekeeping is a legal matter. Altering, falsifying, or tampering with time records or recording time on another employee's time record will result in disciplinary action, up to and including termination of employment.

Overtime hours are permitted on an as needed basis, and must be approved in advance by a direct supervisor. Employees are expected to work necessary overtime when requested to do so, and non-exempt employees will be paid for time worked in excess of 40 hours per week. Exempt employees are not entitled to overtime pay.

House keeping includes but is not limited to vacuuming at the end of the day, dusting and wiping all surfaces, trash disposal on a daily basis and keeping the Guest Bathrooms well stocked and clean at all times.

PaydaysEffective Date: 02/01/2017

Avon Family and Cosmetic Dentistry LLC employees are paid on a biweekly basis. As required by law, Avon Family and Cosmetic Dentistry LLC will deduct Federal Social Security and Income Tax from your paycheck each pay period.

Avon Family and Cosmetic Dentistry LLC encourages its employees to take advantage of Direct Deposit of biweekly pay; this service saves time and provides added security. With Direct Deposit, each paycheck will be automatically deposited to employees' checking or savings account per their instruction. Each payday, employees will receive a pay stub for their records.

For the purposes of calculating employee benefits and pay periods, the workweek is considered Sunday at 12:00 a.m. through Saturday at 11:59 p.m. In the event that a regularly scheduled payday falls on a weekend or holiday, employees will be paid. If a regular payday falls during an employee's vacation, the employee's paycheck will be available upon his/her return from vacation.

An employee's paycheck will not, under any circumstances, be given to any person other than that employee without appropriate written authorization. Paychecks may also be mailed to the employee's listed address upon request.

Employees receive their pay checks biweekly.
