AboutthewardsDonBainesWardandLeslieSmithWardare24beddedrehabilitationwards.Ouraimistoprovideaservicetomaximiseyourabilityto liveasindependentlyaspossibleandtoget youbackhome,whenitissafetodoso.

Wewillworkwithyouonyourrehabilitationgoalsfollowingyourillness/injurytoregain yourindependenceandusefulactivity.

Thewardsaredividedinto4bays,2with6beds,1with5beds,1with3bedsand4siderooms.Thesideroomsarecommonlyrequiredtotreatandisolatepatientswithinfections. Weareunabletoprovideprivatefacilities.

• Promoteyourindependence.

• Involveyouandanyappropriatefamilymembers/carersinyourrehabilitationanddischargeprocess(returningbackhome).

Rehabilitationstaff

Themulti-professionalrehabilitationteamconsistsof:

• Ward manager and ward sister–Thesearethemostexperiencedandseniornursesontheward,theymanageandoverseeresourcesandensurethecareofpatientsismaintainedthroughnationalguidelines,policiesandprocedures.

• Staff nurses–Areregisterednurseswhoplanandevaluateyourcareandsetyourrealisticandachievablerehabilitationgoalswithyouandthetherapists.

• Healthcare assistants–Supporttheregisterednursesandtherapistsindeliveringandimplementingyourcare,andassistingyouinachievingyourgoals.

• Medical cover –AdoctorisbasedonthewardMondaytoFriday9amto5pm.Outsidethesehoursthewardiscoveredbythe on-calldoctor.

• Associate specialist –Asenior,experienceddoctorwhoadvises,mentorsandsupportsjuniorstaff.

• Consultant physician –Workwiththemulti-professionalteamtwiceaweektoreviewpatientsontheward.TheyarebasedatSt.RichardsHospital.However,ifamedicalemergencyshouldoccur,youmaybetransferredtoSt.RichardsHospitalforinvestigationsandtreatment.

• Physiotherapist and occupational therapist –Thetherapistandnursingteamworkcloselytogether,ensuringcontinuityofcareandrehabilitation,enablingyoutoachievethe goalsset. • Ward clerk–Provideadministrativesupportforthewards.

• Social worker–Individualscanbereferredtoadultserviceswhowillassess,andensurecarerequirementsaremetondischarge.Theyprovidesupportandadvicetoindividualsandfamiliesifrequired.

• Speech and language therapist–Willassessyouiftheteamareconcernedaboutyourabilitytoswallowfluidsandfoodsafely.Theymayrecommendthatyourdietandfluidsaremodified.

• Dietician–Willassessyouifyouhavelostweightorhaveareducedappetite.Theymayrecommendnutritionalsupplements.

•Housekeeper –Supportstheteamandpatientsbyprovidingrefreshmentsandoverseesthemenuselectionandspecialistdietsforpatients.

UniformsYouwillseemanydifferentmembersofstaff.Hereisalistoftheirtitlesandtheuniformstheywillwear:

PersonaltherapyDuringyourstayyouwillbeencouragedtodoasmuchforyourselfaspossible,staffwillassistwithactivitiesthatyouneedhelpwith.

Youareexpectedtodressindayclothes(wehavealimitedselectionavailableuntilyourownarebroughtin),youwillrequirewashitems,includingsoap,flannel,toothbrush,toothpasteanddeodorant.

Washinganddressingideallytakesplaceinthebathroomsratherthanatthebedside.Washareas

areallocatedforbothmaleandfemaleuse.Shouldyoubeunabletostandforanyreason,wehavearangeofmanualhandlingequipmentthatwecanusetoensureyoursafetyandthatofthestaff.

Althoughyouwillbeseenregularlybyaphysiotherapist,thetherapycontinueswiththenursingstaff.Eachtimeyougetinandoutofbed,walktothetoiletorsimplysitupinbed,isconsideredtobetherapy. Yourlengthofstayonthewardwilldependonhowquicklyyouareabletoachievethegoals

setforyouinconjunctionwiththerehabilitationteam.Ouraimistodischargewithin21daysofadmission.

Ifgoalsarenotachievableoryouarenotabletoparticipateinactiverehabilitation,wewillassistyoutoarrangesuitablecareinyourhometomeetyourneeds,asyouwillbeunabletoremainontherehabilitationunit.

StudentnursesThewardiscommittedtoprovidingapositivelearningenvironmentforstudentnurses,andworkexperiencestudents.

PropertyProperty,valuablesandcashshouldbekepttoaminimumduringyourstayinhospital.Anyproperty,valuablesorcashnothandedovertotheTrustwillbeheldatyourownrisk.

Pleaseadvisestaffifyouhavedenturesorahearingaid.Whennotinusetheseshouldbekeptinacontainerorinyourlocker.

Youmaywishtokeepasmallamountofcashforthetrolleyshopwhichvisitsthewardthreetimesaweekandsellssweets,drinks andtoiletries.

Ifanybodybringsinmoneyoritemsthatyoucherishpleasenotifyamemberofstaffsothattheycanbeaddedtoyourpropertyform.

LaundryAsthelaundryfacilitiesatthehospitalareverylimiteditisexpectedthatfamily/friends/carerswillberesponsibleforlaunderingpersonalclothes.Thelaundrywillbeavailableforcollectioninapatientpropertybaglocatedinthebedsidelocker.

Duetoinfectioncontrolstaffarenotallowedtowashoutfaecalsoiledclothes.

FoodandnutritionWeaimtoprovideprotectedmealtimeswhenallunnecessaryactivitiesonthewardwillstop.Visitorswillnotbeadmittedatthistime,unlesswithprioragreementwiththeseniornurses,norwillpatientsbeabletotaketelephonecalls.

Ifthesearrangementscausedifficultiesforyourfamily,pleaseaskthemtodiscussthiswiththenurseincharge.

Thehousekeeperwillprovideyouwithamenueachmorningforyoutochoosewhatyouwouldliketoeatthefollowingday.

Ifyouhaveaparticulardietorreligiousrequirementthekitchenwillcaterforthis.

Lunchisservedinthediningarea.

Youwillbeservedhotdrinksatintervalsduringthedayandwillalsohaveajugofwaterwhichischangedtwicedaily.

Unfortunately,duetofoodhygieneregulations,weareunabletostorefoodbroughtinfromhomeinthewardfridge.Wewouldsuggestthatanyfoodbroughtiniseatenimmediately.

Ifyouneedtoattendout-patient’sappointmentsorinvestigationsatanotherhospitalwewillprovideyouwithalunchpack.

Whilstinhospitalyouwillbeweighedweekly.Ifyouloseweightorareobservedtohavereducedappetiteyouwillbereferredtothehospitaldietician.

VisitingarrangementsVisitorsarewelcomeduringyourstay.Thefollowingpointsareonesthatyouandyourvisitorsmayliketoconsider:

Youareatarehabilitationward.Therefore,considerationmustbegiventoyourrehabilitationprogrammeandvisitsfromthedoctors,nursesandtherapists.

Thewardoperatesprotectedmealtimesandhoursforvisitingarelistedbelow.

Wedoaskvisitorstoconsiderotherpatient’swithregardstotheirprivacyandresttimes.

Ifthefirealarmsareactivated(orinanyothercrisissituation),weaskyouandyourvisitorstotakeadviceandinstructionsfromstaff.

Childrenvisitingmustbeaccompaniedandsupervisedbyanadult.TheymustNOTbepermittedtocrawlonthefloors.

Weaskthatvisitorsdonotsitonthebeds,orusepatient’sbathrooms.ThisisforinfectioncontrolpurposesasperourTrustpolicy.

Althoughwewelcomeflowersandplants,wedonothaveasupplyofvases.Pleaserememberthatspaceislimitedonthelockersandaroundthebedspacegenerally.

Occasionallytheremaybereasonsto“close”thewardtovisitorstominimisecrossinfection.Weaskvisitorsnottovisitiftheyhaveanycoldsymptoms,nauseaordiarrhoea.

Itisgenerallybusierinthemorningsopleaselimitcallsatthistime.

Itwouldbehelpfulifonenamedpersoncouldmakeenquiriesandthenpassontheinformationtootherfamilyandfriends.

Acordlessphoneisavailableforpatientstoreceiveincomingcalls.Youmaybeaskedtocallbackifthereisahighdemandforthisservice.

QueriesIfatanytimeyouwishtotalktosomebodyaboutanyworriesorproblems,pleasedonothesitatetoaskthenursingstaff.

Howeversmalltheproblem,wearehere24hoursadaytomakeyourstayascomfortableandtroublefreeaspossible.Wewelcome anycommentsandsuggestionsthatyou haverelatingtoqualityofcareandwaysinwhichstandardscanbeimprovedforyouoryourfamily.

SmokingSussexCommunityNHSTrustoperatesano-smokingpolicyon-site.

OccupationaltherapyTheoccupationaltherapistwilladviseyouonthebestwaystocarryoutdailytasks,whetheritispersonalcare,householddutiesorsocialactivities.

Theoccupationaltherapistwillhelpyouidentifyhomehazardsandrecommendequipmentandadaptationstomakeyourhomesafer.

Ifyouoryourfamilyhaveanyconcernsaboutyourabilitytomanageathome,includingifyouhaddifficultiespriortoadmission,pleaseasktoseetheoccupationaltherapist.

HomevisitassessmentAspartofyourrehabilitationprogrammeyoumaybeaskedtoparticipateinahomevisit.Theyinclude:

•Theoccupationaltherapistandanothermemberofstafftakingyoutoyour ownproperty.

•Yourabilitytoaccessyourpropertyandmobilisearoundyourhomeasyouwouldondischargewillbeassessed,includingyourabilitytotransferonandoffyourbed,chairandtoiletetc.

•Theassessmentwillenableyou,yourfamily/carers,aswellasthetherapyteamtohaveanunderstandingofhowyouaregoingtomanageinyourhomewhenyou leavehospital.

•Recommendationsaregivenwhereindicatedtoensureyourindependenceandsafetyismaintainedondischarge.Equipmentandrailsmaybeprovidedifaneedisidentified.

•Youarewelcometohavefamily/carerspresentonthevisit.

•Yourabilitytocompletecertaindomestictaskse.g.makingahotdrinkmaybeassessedonthehomevisit.

AccessvisitIfitisidentifiedthattakingyoutoyourhomebeforedischargeisnotrequired,theoccupationaltherapistwillaskforyourconsenttovisityourpropertytoassessforanyequipmentyoumayrequireondischarge,withoutyoubeingpresent.

PhysiotherapyThephysiotherapistwillassessyouintermsofyourmobility,balanceandconfidence.Youwillbegivenasuitableaidtohelpyouifrequired.Youwillbeadvisedonpersonalexercisestohelpyouprogressandassistyourrehabilitation.Ifyouneedtomanageasteporstairsyoucanpracticethesetaskswith thephysiotherapist.

Thephysiotherapist’saimistoimproveyourlevelofmobilitytoensurethatyoufeelconfident,safeandhappytobedischarged.Ifthephysiotherapistfeelsitisappropriate,followuparrangementscanbemadetoprogressyourmobilityafteryouaredischarged.

ExpectationsfromyourrehabilitationprogrammeDuringyourstayyouwillbeencouragedtoparticipateinarehabilitationprogrammedesignedtohelpyouregainyourindependence.Aninitialassessmentwillbecompletedandfromthisarehabilitationprogrammewithgoalswillbeagreed.Therehabilitationprogrammecaninvolve:

•Becomingmoreinvolvedwithyourpersonalactivitiesofdailyliving,e.g.washing,dressing,toiletingetc.

•Participatingintherapyprogrammesaimedatyourspecificneeds,toimproveyourstrengthandconfidenceasadvisedbythetherapists.

•Undertakingfamiliarhouseholdtasksaspartofagradedprogrammeunderdirectionoftherapistse.g.kitchenactivities.

•Demonstratingsafeworking,handlingandmovingofutensils,cutleryandequipmentinakitchenfacility,i.e.whenmakingahotdrink,snacksormealswiththedirectionofstaff.

•Learningtouse,wherenecessary,specialiseditemsofequipmenttomakedailylivingeasier.

•Workingtogetherwiththerehabilitationteamandyourfamily/carerthroughanagreedrehabilitationprogrammeinpreparationformanagingtoliveathome.

•Involvementwithyourfamilyandcarerswill beencouragedthroughoutyourstay,sothattheyfeelconfidenttohelpyoutomanagefollowingdischargewhenyouleavetherehabilitationunit.

Whenrehabilitationpotentialisreachedoryouareunabletocurrentlyparticipateinanactiverehabilitationprogrammeoryoudecideyoudonotwanttoparticipateinrehabilitation,

wewillneedtodiscussandmakedischargearrangementswithyou,withinanagreedtimeframe.Ondischarge,wewillsendyourGPalettersummarisingyourcareintheward.

MedicationAspartofyourrehabilitationprogramme,wewillassessyourabilityandanyneedsyouhavetoenableyoutosafelymanageyourownmedicationaspreparationforwhenyouaredischarged.

Shouldyouhaveanyconcernsregardingyourmedication,pleaseasktospeaktotheregisterednurseonduty.

Ifyouneedpainrelief,itisreadilyavailable,pleaseaskamemberofnursingstaff.Mostpainreliefisgivenatmealtimes,butletusknowifyourpainisnotwellcontrolled.

FallsfactsThedangeroffallinginhospitalisveryreal.Thereareseveralfactorsthatincreasethisrisk:

•Illnessandbeingawayinnewand unfamiliarsurroundings.

•Certainmedications,suchaspainrelievers,bloodpressuretablets,waterpills,laxativesandsleepingaids.

•Previoushistoryoffalls.

•Shortnessofbreath,muscleweakness,unsteadygait,andfever.

•Newconfusionfromillnesssuchasinfectionorchangesinbodychemistry;dementia,depressionorpsychosis.

•Sensoryimpairments,suchasnumbnessinfeet;visionorhearingproblems.

•Posttreatmentprocedure/surgery.

Havingahigherrisktofallmayoccuratdifferenttimesthroughoutyourhospitalstay.Thenurseandtherapistswillassessyourriskoffallingateachshiftandwhenyourconditionchanges.

Ifitisdeemedthatyouareatahighriskoffallsthenaspecialplanofcaretoaddresssafetyissuesandreducethedangerofanaccidentalfallandinjurywillbeimplemented.

•Avoidstretchingorbendingtoreachthings.

•GetoutofbedS-L-O-W-L-Y.

•Toleavethepatientsbedareatidyandreplacechairswhereyoufoundthem.

•Makesurethepatientscallbelliswithin easyreach.

•Askstafftoreplacebedrailsmovedduringyourvisit.

•Reportanypotentialhazardssuchasspillsofliquidonthefloor,trailingwiresorcables,obstaclesaroundthebedspaceetc.

GeneralInformation•Thereisaloungeavailableforpatientstosocialiseandseevisitors.Atelevisionisprovidedinthelounge.

•Ifyouhavereligiousorculturalneeds,pleaseletusknowandwewilldoourbesttohelpfulfilthesewithhelpfromthelocalcommunity.ThehospitalChaplainholdsaserviceonThursdayafternoonsinthehospitalchapel.Pleaseinformamemberofstaffifyouwish

toattend.

•TheFriendsoftheHospitalprovideasmallmobiletrolleyserviceonTuesdayandFridaymornings.Youwillbeabletopurchasetoiletries,squashandconfectionery.
