WEIGHT AND HEIGHT OF ADULTS

18-74 YEARS OF AGE

INTRODUCTION

T~e height and weight measurements in thisreport we~e obtained as a part of the Health andNutrition Examination Survey (HANES).HANES is a program in which measures ofnutritional status are collected for a scientif-ically designed sample representative of thecivilian noninstitutionalized population of theUnited States in a broad range of ages, 1-74years. The survey was also designed to obtainsome information on the general health status ofthe entire age group as well as more detailedhealth assessment data on those 25 through 74years of age. A detailed description of thespecific content and plan of operation, includingsample design, has been published. 1

In previous Health Examination Survey pro-grams, the U.S. Bureau of the Census cooperatedin the sample design and in the initial visits andinterviewing at selected eligible households inthe 65 primary sampIing units (PSU’S) through-out the United States. Additional householdvisiting, interviewing, history taking, and ex-plaining the examination portion of the programwere performed by members of the field teamsof the mobile examination center. The selected

sample persons for whom an appointment couldbe made were brought into the specially con-structed mobile examination centers which weremoved into a central location in each of thePSU’S. The teams which traveled to the varioussurvey locations throughout the country in-cluded professional and paraprofessional medicaland dental examiners along with technicians, interviewers, and other staff.

The probability sample design used in thestudy provided for a higher sampling ratio of thepoor, preschool children, women of childbearingages, and the elderly than of others in the civil-ian noninstitutionaIized population.

Field data collection operations for the firstHANES survey were started in April 1971 andcompleted in June 1974. Of the 28,043 personsaged 1-74 years who were selected in thenational probability sampIe to represent the 194million persons in that age group in the civiIiannoninstitutionalized population, 20,749, or 74percent, were examined. When adjustments aremade for the differential sampling ratios used toensure oversampling among the poor, preschoolchildren, women of childbearing age, and theelderly, the effective response rate is 75 percent.Among those 18-74 years of age at the time ofinterview for whom height and weight determin-ations were made, there were 13,645 examinedout of the probability sample of 19,572 selectedto represent the 128 million persons of thoseages in the population. This is an unadjustedresponse rate of 70 percent and an effectiveadjusted response rate of 70 percent.

In this report, weight and height data areanalyzed and discussed by age, sex, and race. The techniques of measurement are discussed. The present findings are compared with theHealth Examination Survey (HES) of 1960-62.2Analysis of weight and height studies withvariables other than age, race, ancl sex will bepresented in future reports. Consideration ofbody composition and configuration, of obesi~y,and of nutritional status will also be subjects offuture reports.

METHODS

The examinees changed from their streetclothing into disposable paper examination uni-forms and foam rubber slippers designed tofacilitate and standardize various elements of theexamination. Body measurements were made atvarious times throughout the day at each exam-ination center and in different seasons of theyear; thus body measurements were not stand-ardized with regard to diurnal and seasonalvariations. Weights vary between winter andsummer and may differ depending upon recencyof food and water intake.

Height

For persons aged 18-74 years, height wasmeasured with the examinee wearing disposablefoam rubber slippers, standing with feet tol-gether, back and heels against the upright bar ofthe height scale, head approximately in theFrank fort horizontal plant (“look straightahead”), and erect (“stand up tall” or “stand upreal straight” with some assistance and demon-s tration when necessary). However, upwardpressure was not exerted by the examiner on thesubject’s mastoid processes to purposefully“stretch everyone in a standard manner, ” as i~srecommended by some.

2National Center for Health Statistics: Weight, height, and selected body dimensions of adults, UnitedStates, 1960-1962, by H. W. Stoudt et al. Vital andHealth Statistics. Series 1 l-No. 8. DHEW Pub. No,,(HRA) 76-1074. Health Resources Administration.Washington. U.S. Government Printing Office, June1965.

The equipment consisted of a Ievel[ platformto which was attached a vertical bar with a steeltape. Attached to the vertical bar peWendicu-larly was a horizontal bar which was brought

down snugly on the examinee’s head. Attachedto another bar in the same plane as the’ horizo-ntal measuring bar was a Polaroid camera whichrecorded the subject’s identification numbernext to the pointer on the scale giving a precisereading. The camera not only gave a permanentrecord minimizing observer and recording errorbut, by sliding up and down with a h,brizontalbar and always being in the same plane, alsocompletely eliminated parallax. That is, if thepointer had been in the space in front of thescale, it would have been read too high if theobserver had looked up at the scale from beIowor too low if read down from above.

Weight

A Toledo self-balancing scale that mechan-ically printed the weight to one-quarter of apound directIy onto the permanent record wasused. This direct printing was used to minimizeobserver and recording errors. The scale wascalibrated with a set of known weights, 1and anynecessary fine adjustments were made at thebeginning of each new trailer location, i.e.,approximately every month. The r’ecordedweight was later transferred to a punched cardto the nearest 0.25 pound (lb.). The totalweights of all clothing worn ranged from 0.20 to0.62 lb.; this was not deducted from weightspresented in this report. (The weights, then, are0.20 to 0.62 lb. above nude weight recorded tothe nearest 0.25 lb.). The examination clothingused was the same throughout the year so thereis no seasonal variation in the weight of clothing.

FINDINGS

Weight

Age and sex. –The estimated numbers andpercent distributions of men and women aged18-74 years in each of the weight gropps arepresented in tables 1-3. Table 4 summarizesthese distributions in terms of basic statistics forexamined persons by age for men and women.

Table 4 shows that the mean weight of menaged 18-74 years was 172 pounds. Table 4 andfigure 1 show that the mean weight of men in-creased from 165 pounds in age group 18-24years to 178 pounds in age group 35-44 years.Thereafter it decreased with age to 175 poundsin age group 45-54 yearn, 171 pounds at 55-64years, and a low of 164 pounds in the oldest agegroup measured, 65-74 years.

The mean weight of women was 143pounds, 29 pounds less than that for men (table4). Table 4 and figure 1 show that the meanweight increased with age from a low of 132pounds at 18-24 years to a high of 149 poundsat ages 45-64 years and then declined to 146pounds in the oldest age group.

The age patterns of men and women de-scribed previously for mean weight were alsogenerally evident when the percentiles at boththe upper and lower ends of the distributionwere examined (table 4, figure 2).

Race, sex, and age. –Estimated numbers andpercent distributions of white and black adultsin each of the age groups according to theirweights are shown in tables 5-10. Table 11 andfigure 3 summarize these distributions. Thepattern in mean weight with age found for adultmen in the United States was also found for

decreased consistently with age from 45-54years of age on. A similar pattern was not evi-dent for black men. There was little consistentdifference among the varicrus age groups be-tween the mean weights of black and white men.

Data on weight by age for black and whitewomen tend to parallel with those shown formen, with the exception that the increase inmean weight of white women continued up tothe age group 45-54 years and then declined(table 11). The means for black women showeda plateau effect and a slight decrease, however, ateach age black women had larger mean weightsthan white women, with the differences aver-aging about 20 pounds for the age groups 35-64years.

The interracial differences in weight for bothmen and women were generaHy greater, particu-larly for women, at the upper end of the dis-tribution than at the lower end (figure 4). Theweight distribution for black men had morevariability than that for white men. The stand-ard deviation of the weight distribution is great-er at each age group for black men than forwhite men, as is the distance between ~e 95th

Age and sex. –The estimated numbers andpercent distributions of men and women aged18-74 years by height are presented in tables12-14. Table 15 and figure 5 summarize thesedistributions in terms of basic statistics forexamined persons by age for men and women.

The meam height for men aged 18-74 yearswas 69.0 inches. The highest mean height, 69.7inches, was found in the youngest age group,18-24 years, and is 2.4 inches taller than themean height in age group 65-74 years, 67.3inches. This corresponds with a small decrease inheight with increasing age, which averages lessthan one-half inch per decade of age over therange 18-74 years,

For women the mean height was 63.6inchei, 5.4 inches less than the mean height formen. Th? mean height for women was highest,64.3 inches, in the youngest age group, as it wasfor men. The mean height decreased sIightIy to64.1 inches at 25-34 years, remained constant at35-44 years, and then declined to 63.6 inches at45-54 years, 62.8 inches at 55-64 years, and62.3 inches in the oldest age group, 65 years andover. The difference between the youngest andoldest age group is 2.0 inches. The decrease inheight with age averages less than one-half inchper decade over the age range of 18-74 years(figure 5).

The age patterns of men and women de-scribed previoudy for the mean height were alsogenerally evident when the percentiles at theupper and lower ends of the distribution wereexamined (figure 6).

Race, sex, and age. –The estimated popula-tion of white and black adults by height ispresented in tables 16-19 and equivalently aspercent distributions in tables 20 and 21. hieanheights are shown by age for white and blackmen and women in table 22 and figure 7. Ineach of the five age groups in the age range18-64 years, white men were taller than blackmen, The average difference between heights of

~vhite and black men in these age groups wasabout one-haIf inch, ~vhich difference could beattributed to sampling variability. At age group65-74 years, the mean height for black andwhite men was the same. This pattern by ageand race is not observed for women, but averagedifferences for women, as for men, were small.At age groups 18-24 and 25-3-+ years, white\vomen tvere, on the average, 0.25 inch tallerthan black women. At ages 35-44 years, blackwomen ~yere f). 1 inch ta]]er thm white women;

at ages 45-54 years, they were 0.2 inch taller;and at ages 65-74 years, they were 0.3 inchtaIler. At ages 55-64 years, the mean height forboth races was the same. Small differences suchas these couId easily arise through samplingerror, and there seems to be no justification forstating that there is any difference in averageheight between black and white women.

The decrease in mean heigh~ with age shownpreviously for the total population of men isalso obsened for black and white men sepa-rately. This pattern is also et’iclent for black andwhite women with the exception of the meanheight for black women at age 35-44 years.

years were greater at the upper end of the heightdistribution than at the lower end. This picturewas reversed for men at the Iower ages. Theinterracial differences in height, however, weresmall and could easily arise through samplingerror (figure 8).

The differences between white and bIackwomen discussed previously for the mean of theheight distribution were also observed at aboutthe same order of magnitude for the Iower endof the distribution, for the median, and for theupper end of the distribution (figure 8).

AGEIN YEARS

Figure 8. Selected estimated percentiles of height distribution in inches of men end women 18-74 years, by age and race: United States,1971-74

Findings from the present study show thataduIts in the United States weighed more, on theaverage, than their counterparts in 1960-62. TheHANES data on weights for men are consist-ently higher by an average of 6 pounds acrossthe age range 18-74 years; for women, they arehigher by an average of 3 pounds for this agerange.

The pattern of differences in average weightfor men shows a slight variation with age. Thedifferences ranged from an average of 5 poundsin ages 45-54 years to an average of 8 pounds inages 35-44 years. For women, the differences inaverage weight ranged from less than 1 pound inages 55-64 years to an average of 6 pounds inages 25-44 years (table 23, figure 9).

Tables 24 and 25 show cumulative percentdistributions of weight for men and women byage in 1960-62 and 1971-74. In the most recentsurvey, 1971-74, the percent of men in eachage group at the cutoff points indicated weregreater than the corresponding percents of thosein HES, 1960-62. This observation was alsogenerally true for women with the exception ofwomen ages 45 years and oIder.

The pattern of differences in weight at

various cutoff points found between the twosurveys for total maJe adults in the United Stateswas also consistently found for white males(table 26) but not for black males. Relatively

more black males ages 25-34 weighing 160pounds or more were found in HISS, 1960-62,than in HANES, 1971-74. There were also somleindications of this pattern in percent of males c)fweights 170 pounds or more in ages 25-44 and45-54 years. The proportion with those bodyweights in both HES and HANES was greateramong white men than among black men inalmost all age groups.

The proportion of white and black femalesin ages through 44 years having body weightsabove 140 and 150 pounds was greater inHANES than in HES (table 27). This pattern indifference in proportion was not consistent inthe older ages (tables 28-30). There was a greaterpercent of women at ages 18-44 years whoweighed 170 pounds or more, a crude estimateof obesity, in HANES than in HES. Women atthese ages were heavier on the average thanthose of 10 years ago, particularly black wometl. The proportion of women with these boayweights in both HES and HANES was greateramong black women than among white womenin all age groups.

Although heights of U.S. adults as measuredfrom the Health and Nutrition Examination Sur-vey, 1971-74, were taller than those from theHealth Examination Survey, 1960-62, the differ..ences are numerically small. HANES data formen’s heights range from less than one-half inchto an inch taller than HES data. Correspondingvalues for women are from less than one-halfinch to slightly more than three-quarters of aninch taller (table 23, figure 10).

TabIes 31 and 32 show the cumulative per-cent distributions of height for men and womenby age in 1960-62 and 1971-74. Men andwomen in HANES, 1971-74, were taller thanthose in HES, 1960-62. Fifty percent of the menin HANES were 69 inches or more as comparedwith 38 percent in HES across the age range of18-74 years. The corresponding values forHANES and HES for heights of 70 inches ormore are 36 and 28 percent, respectively. In thisage range, 45 percent of the women in HANESwere 64 inches or more as compared with 37percent for HES. The corresponding values forHANES and HES for 65 inches or more are 30and 21 percent, respectively. The proportions ofmen and women whose height exceeded any

other specified height may be found in tables31-32.

Tables 33-36 show by age the percent ofwhite and black men and women having heightsof 69 inches or more and 64 inches or more.(These are the median heights for men andwomen, respectively.) In most ages and in eachof the sex and race groups the proportions ofpersons above these selected heights were greaterin 1971-74 than in 1960-62. The proportion ofpersons whose heights exceeded any other speci-fied level may be calculated from the datapresented in table 37.

The HANES data on weigh,ts for me,n wereconsistently higher by an average of 6 poundsacross the age range 18-74 years. The; corre-sponding values for women were higher by anaverage of 3 pounds’ over the same age range.Heights of adults as measured during HANESwere slightly greater than those from HES. Sinceweight increases per inch of height usually rangeon an average from 4 to 5 pounds in mien andfrom 2 to 4 pounds in women, it can be statedthat the increase in average weight for the popu-lation between 1960-62 and 1971-74 can be

Page 14: WeightAndHeightOf Adults 18-74 YearsOf Age · WeightAndHeightOf Adults 18-74 YearsOf Age: United States, 1971-74 Weight and height measurements of white and bIack adults 18-74 years

attributed only in part to a corresponding in-crease in height.

‘The pattern of weight gain in the middleyears and gradual decline with increasing age, inboth surveys for men as well as for women, isillustrated in figure 11, which shows the pro-portionate change in average weight with ageover the mean for the group aged 18-24 years. This trend in both surveys is influenced by secu-lar changes in body size for successive genera-tions represented in this cross-sectional data.

Women in HES generally attained their max-imum weight about two decades later than mendid and had a greater relative gain with age. Thelatter pattern is also evident for women inHANES but they attained their mzcsimumweight about one decade later than men did. This delay, in both periods, may be attributed tothe greater effort of weight-conscious youngerwomen. Here again, it is not possible to deter-mine from the data how much these findingsmay be influenced by any changes in body sizefor the successive generations in this cross-section of the population.

On the average, the relative decrease ofheight with age from 45 years or more was simi-

lar for men and women in both surveys (figure12).

Comparable response rates and similarmeasurement processes for recording height andweight are necessary to compare the bodymeasurements from the t]i’o sunwys.

The HES sample had a response rate of 95percent intervie~ved and 86 percent examined, ahighly representative sample of the civilian non-institutionalized population of the UnitedStates. The corresponding rates for HANES wereover 95 percent interviewed and 70 percent ex-amined. The lower examination rate in theHANES sample than in the HES sample mighthave biased the mean \veight–but not the meanheight–downward because heavier persons, whoare reported to be associated with more medicalproblems, might be less likely to come for exam-ination. This is, however, unlikely, because an&alysis of medicaI histories comparing the non-examinees with the examinees indicates no Iargedifferences between the nonexamined group andthe examined group for the medical statisticscompared. For example, 12 percent of the per-sons examined reported having an illness or con-dition which interfered with their eating as com-

The correspond-ing percents for nonexamined persons were:arthritis, 18 percent; high blood pressure, 22percent; and diabetes, 4 percent.

Measurement processes employed in bothsurveys for heights and weights were similar, highly standardized, and closely controlled. Twopounds were deducted from HES data to allowfor weight of clothing. Total weights of all cloth-ing for HANES ranged from 0.20 to 0.62 pound, which was not deducted from weight. In bothsurveys, height was measured without shoes.

SUMMARY

Findings on height and weight among adultsin the civilian noninstitutionaIized population ofthe United States obtained in the Health andNutrition Examination Survey of 1971-74 arepresented and analyzed in this report. Majorfindings are as follows:. Men average 172 pounds in weight, with

about 90 percent falling between 128 and 225pounds. Weight decreases with age from themaximum average of 178 pounds for those35-44 years of age to 164 pounds at 65-74 years.. For white men, the average weight is 173pounds, with about 90 percent falling between129 and 224 pounds. Their maximum andminimum average weight is simiIar to that of allmen within the same age group.. The average weight of black men decreaseswith age from the maximum average of 182pounds for those 35-44 years of age to 161pounds at 65-74 years. The overall averageweight is 171 pounds, with about 90 percentfalling between 125 and 239 pounds.● Women average 143 pounds, with about 90percent falIing between 104 and 203 pounds.Their maximum average weight is 149 pounds inthe 45-64 year age group.● White women average 142 pounds, withabout 90 percent falling between 104 and 199pounds. Their maximum average weight is 147pounds for those 45-64 years of age, an increaseof 15 pounds from 132 pounds at 18-24 years.● About 90 percent of black women faIIbetween 104 and 228 pounds, with an averageof 156 pounds. The maximum average weight is169 pounds for those 45-54 years of age, and

the minimum average weight is 138 pounds a’18-24 years.● Men average 69.0 inches in height, ~vitlisome 90 percent between 64.4 and 73.6 inchesTheir average height decreases with age from amasimum of 69.7 inches to to 67.3 inches.. The average height for ~vhite men is simila’to that of all men, 69.1 inches, with about 9(percent falling bet-ween 64.6 and 73.6 inche:The average height of white men ranges from -imaximum of 69.8 inches at ages 18-24 years to67.3 inches in the oldest age group.. Black men average 68.6 inches, with about90 percent between 64.1 inches and 73.3 inches.Average height decreases with age from a mzw-imum of 69,5 inches to 67.3 inches.. Women average 63,6 inches in height. Ho\v-ever, there is a general decline in stature withage, beginning with 64.3 inches at 18-24 yearsand falling to 62.3 inches at 65-74 years. About90 percent of all adult women are between 59.5and 67.8 inches in height.. For white women, the average height is 63.7inches, ~vith some 90 percent between 59.5 and67.864.362.3

inches. The m~ximum average height isinches at ages 18-24 years and declines toinches at ages 65-74 years.

. For black women, the average height is 63.7inches, with some 90 percent between 59.6 and67.8 inches. The maximum average height of 64.2inches at ages 35-44 years declines to 62.6 inchesat ages 65-74 years.● For the age range 18-74 years, about 50percent of men are 69.0 inches or taller and 45percent of women are 64.0 inches or taller inHANES as compared with 38 percent of menand 37 percent of women in HES.● For the age range 18-74 years, about 49percent of the men are 170 pounds or more and45 percent of the women are 140 pounds ormore in HANES as compared with 45 percent ofmen and 45 percent of women in HES.. At ages 18 through 44 years, there is agreater percent of women weighing 170 poundsor more in HANES than in HES; relatively more\vomen, particularly black women, are heavy, possible obese, than was the case about 10 yewsago.. The masimum average weight of women inHES, 150 pounds, is in the 55-64 year agegroup, or about 20 years later than that for men.The m~ximum weight for women in HANESoccurs in 45-64 years, or about 10 years Iaterthan that for men.

STATISTICAL NOTES

Survey Design

The sampling plan of the Health and Nutri-tion Examination Survey (HANES) foIlowed ahighly stratified multistage probability design inwhich a sample of the civilian noninstitu-tionalized population of the coterminous UnitedStates, 1-74 years of age, was selected. Excludedfrom the selection process were those personsconfined to institutions or residing on any of thereservation lands set aside for use by AmericanIndians. Successive eIements dealt with in theprocess of sampling are the primary samplingunit (PSU), census enumeration district (ED),segment (a cluster of households), household, eligible person, and finally, sample person.

The starting points in the first stage of thisdesign were the 1960 decennial census lists ofaddresses and the nearly 1,900 PSU’S into whichthe coterminous United States was divided. EachPSLJ is either a standard metropolitan statisticalarea, a single county, or two or three contiguouscounties. The PSU’S were grouped into 357strata for use in the Health Interview Survey andsubsequently collapsed into 40 superstrata forthe Health and Nutrition Examination Survey.

Of the 40 superstrata, 15 contained a singlelarge metropolitan area of more than 2 millionpopulation. These 15 large metropolitan areaswere chosen for the sample with certainty. Theremaining 25 superstrata were formed by classi-fying the noncertainty strata into 4 populationdensity groups within each of 4 geographicregions. Then using a modified Goodman-Kishcontrolled-selection technique to assure propor-tionate representation of specified State groupsand rate-of-population-change classes, 2 PSU’Swere chosen from each of the 25 noncertaintysuperstrata with the probability of selection of a

PSU proportionate to its 1960 population. Inthis manner, a total first-stage sample of 65PSU’S or “stands” included the areas with inwhich a sample of persons would be selected forexamination. The PSU’S were scheduled to besampled over a 3-year period with 300-600persons to be examined per stand.

Although the 1970 census data were used asthe frame for selecting the sample within thePSU when they became available, the calendarof operations required that the 1960 census databe used for the first 44 locations in the HANESsample. The 1970 census data were used for thelast 21 stands of the sample.

Beginning with the use of the 1970 censusdata, the segment size was changed from anexpected 6 households selected from compactclusters of 18 households to an expected com-pact cluster of 8 households. The change wasmade because of operational advantages, andresearch by the Census Bureau indicated thatprecision of estimates would not be appreciablyaffected by the change from noncornpact clus-ters to compact clusters.

For ED’s not having usable addresses (gen-erally located in rural areas), area sampling wasemployed and consequently some variation inthe segment size occurred. To make the samplerepresentative of the current population of theUnited States, the address or ED segments weresupplemented by a sample of housing units thathad been constructed since the 1960 and 1970decennial censuses.

Within each PSU, a systematic sample ofsegments was selected. The enumeration districtsthat fell into the sample were coded into one oftwo economic classes. The first class, identifiedas the “poverty stratum ,“ was composed of

45

Page 51: WeightAndHeightOf Adults 18-74 YearsOf Age · WeightAndHeightOf Adults 18-74 YearsOf Age: United States, 1971-74 Weight and height measurements of white and bIack adults 18-74 years

“Current Poverty Areas” thfit had been identi-fied by the Bureau of the Census in 1970(pre-1970 census), plus other ED’s in the PSLJwith a mean family income of less than $3,0100in 1959 (based on the 1960 census). The secondeconomic class, the “nonpoverty stratum, ” in-cIuded all ED’s not designated as belonging tothe poverty stratum.

All sample segments classified as bcirlg in thepoverty stratum were retained in the s~mple. For the first 42 stands, sample segments innonpoverty stratum ED’s were divided into 8random subgroups and one of the subgroups waschosen to remain in the HANES sample. F,c-search indicated ti~at efficiency of estimatescould be increased by changing the ratio ofpoverty to nonpoverty segments from 8:1 to2:1. Therefore in the later stands, the selectedsegments in the nonpoverty-strat urn ED’s weredivided into two random subgroups and one ofthe subgroups was chosen to remain in theHANES sample. The differential sampling per-mits a separate ana]ysis with adequate reliabilityof those classified as being below the povertylevel and those classified as being above thepoverty level.

After identification of the sample segments,a list of all current addresses within the segmentboundaries was made and the households we:re

interviewed to determine the age and sex of eachhousehold member as well as other demographicand socioeconomic information required for thesurvey. If no one was at home after repeatedcalls or if the household members refused to beinterviewed, the interviewer tried to determinethe household composition from neighbors.

To select the persons in sample segments tobe examined in HANES and at the same time tooversimple certain groups at high risk of mal-nutrition, all household members aged 1-74years in each segment were first listed on asample selection worksheet with each householdin the segment listed serially. The number ofhousehold members in each of the six age-sexgroups shown in table I were then listed on theworksheet under the appropriate age-sex grou]pcolumn. The sample selection worksheets werenext put in segment-number order and a sys-tematic random sample of persons in eachage-sex group was selected to be examined usin!;the sampling rates shown in table 1.

The persons selected in the 65-staLhd sampleof HANES comprised a representative sample ofthe target population and included 28,043 sam-ple persons 1-74 years of age of whom 20,749 or74 percent were examined. When adjust-ments were made for different sampling forhigh-risk groups, the response rate became 75percent.

All data presented in this report are based on“weighted” observations. That is, data recordedfor each person are inflated to characterize thesubuniverse from which that sample person wasdrawn. The weight for each examined ~erson is aproduct of the reciprocal of the probability ofselecting the person, an adjustment for nonre-sponse cases (i.e., persons not examined), and apoststratified ratio adjustment which increasesprecison by bringing survey results into closeralignment with known U.S. population figuresfor 20 age, race, and sex groups as of November1, 1972, the approximate midpoint of HANES.

A more detailed description of the surveydesign and selection technique can be found in“Plan and C)peration of the Health and NutritionExamination Survey, United States, 1971-1973 ,“ Vital and Health Statistics, Series 1-Number 10a.

Nonresponse

In any health examination survey, after thesample is identified and the sample persons arerequested to participate in the examination, thesurvey meets one of its more severe problems, namely that of nonresponse. Usually a sizablenumber of sample persons will not participate inthe examination. A further potential for biasresults if the sample persons who do notparticipate differ from the sample persons ex-amined with respect to the characteristics underexamination. Intensive efforts were made inHANES to develop and implement procedures

46

Page 52: WeightAndHeightOf Adults 18-74 YearsOf Age · WeightAndHeightOf Adults 18-74 YearsOf Age: United States, 1971-74 Weight and height measurements of white and bIack adults 18-74 years

and inducements that would reduce the numberof nonrespondents and thereby reduce the po-tential of bias due to nonresponse. These pro-cedures and inducements are discussed in “Planand Operation of the Health and NutritionExamination Survey, United States, 1971-1973,” Series l-Number 10a.

Despite these intensive efforts, 25 percent ofthe sample persons from 65 stands were notexamined as compared to previous surveys withresponse rates of more than 86 percent. Conse-quently, the potential for a sizable bias doesexist in the 1971-74 estimates in the publica-tion. Because more than 95 percent of thesample persons responded to a medicaI ques-tionnaire in 1971-74, one could examine thecharacteristics of the nonrespondents and thenature of nonresponse. This examination indi-cated that the likelihood of sizabIe bias is sma.11.

As was mentioned earIier, the data in thisreport were based on weighted observations, andone of the components of the weight assigned toan examined person was an adjustment fornonresponse. A procedure was adopted whichmultiplies the reciprocal of the probability ofselection of sample persons who were examinedby a factor that brings estimates based on ex-amined persons up to a level that would have beenachieved if all sample persons had been ex-amined. The nonresponse-adjustment factor wascalculated by dividing the sum of the reciprocalsof the probability of selection for all selectedsample persons in each of five income groupswithin each stand by the sum of the reciprocalsof the probability of selection for examinedsample persons in the same stand and income~roup. The five income groups were: under$3,000; $3,000-$6,999; $7,000-$9,999;$10,000-$ 14,999; and $15,000 and over. Forsample weighting purposes, income group wasimputed for 5.6 percent of the sample personsusing educational level of the head of thehousehold. To the extent that the income-within-stand classes were homogeneous withrespect to the health characteristics under study, the. adjustment procedure was effective in reduc-in~ the potential of bias due to nonresponse. The percent distribution of the nonresponseadjustment factors computed for the 65-standsample of HANES is shown in table II.

Missing Data

Examination surveys are subject to the Iossof information not only through the faiIure toexamine a.11sample persons but a.ko from thefaiIure to obtain and record alI items of informa-tion for examined persons. Age, sex, and racewere known for every examined person, but fora number of examinees one or more of theanthropometric measurements were not avail-able. The extent of these missing measurementsis indicated in table III.

Estimates for missing anthropometnc datawere generally made subjectively on the basis ofa multiple-regression type decision, substitutingfor the missing measurements those of an in-dividuzd who was of the same age, sex, and raceand who had other dimensions similar to thoseavailable for the examinee with incomplete data.

For those with no anthropometric measure-ments available, a respondent of the same age-sex-race group was selected at random and his

measurements were assigned to the non-examined person,

Small Numbers

In some tables, magnitudes are shown forcells for which the sample size is so sma.11thatthe standard error may be several times as greatas the statistic itself. In such instances the num-bers, if shown, have been included ordy to con-vey an impression of the overall story of thetable.

Standard Errors

The probability design of the survey makespossible the estimation of standard errors corre-sponding to the weighted estimates presented. The standard error is primarily a measure ofsampling variability, that is, the variations thatmight occur by chance because only a sample ofthe population is surveyed. As calculated for thisreport, the standard error also reflects part ofthe variation which arises in the measurementprocess. It does not include estimates of any

biases which might Iie in the data. The chancesare about 68 out of 100 that an estipate fromthe sample would differ from a compIete censusby less than the standard error. The chances areabout 95 out of 100 that the difference wouldbe less than twice the standard error and about99 out of 100 that it would be less than 2?4times as large,

Estimates of standard errors are obtainedfrom the sample data and are themselves subjectto sampling error when the number of cases in acell is small, or even occasionally wheri the num-ber of cases is substantial.

Estimates of the standard errors for selectedstatistics used in this report are presented intables 4, 11, 15, and 22, These estimates havebeen prepared by a replication technic@e whichyields overall variability through observation ofvariability among random subsampks of thetotal sample. Again, readers are reminded thatthese estimated standard errors do not reflectany residual bias which might still be presentafter the attempted correction for nom-esponse.

DEMOGRAPHIC TERMS

The demographic characteristics of the pop-ulation sampled are d~fined as follows:

Age. –The age recorded for each examineewas the age at his last birthday on the date ofexamination. The age criterion for inclusion inthe sample used in this survey was defined interms of his age at time of census interview. Some of those who were 74 years old at thetime of interview became 75 years old by thetime of the examination. There were 20 suchcases. In the adjustment and weighting proce-

0

dures used to produce national estimates, thesepersons were included in the 74-year-old group.

Race. –For each individual, race was re-corded by observation as ‘~white,” “black,” or“other races.” The last category included Ameri-can Indians, Chinese, Japanese, and all racesother than white or black. Mexican persons wereincluded with “white” unless definitely knownto be American Indian or of another race otherthan white’. Blacks and persons of mixed blackand other parentage were recorded as “black.”
