Why Blood-Pressure Readings Can Vary DEAR DR. ROACH: I would like to comment on your column regarding blood-pressure differences between the arms.
A difference of greater than 10 mmHg between the two arms in systolic pressure is considered abnormal. We see patients not uncommonly in consultation for this reason alone, perhaps as many as one a month. Almost always, the reason for the difference is blockage of the subclavian artery. The wrist pulse on the side with lower blood pressure may be normal, diminished or absent in those patients.
From 30 years of observation, I have noticed that patients with BP differences between the arms are nearly always female smokers (or former smokers), and the left subclavian is much more likely to be affected (have a lower blood pressure) than the right. Why this sex difference should be and why the left side should predominate is not understood and, to my knowledge, has not been described in print. Additionally, the presentation is usually in a non-obese woman in her 40s or 50s who is still smoking.
The significance of the finding is that it is a red flag waving for tobacco cessation and is a marker that that person’s arteries have already been significantly altered by the habit. Fortunately, the incidence of arm symptoms associated with the lower BP on that side is quite low -- perhaps 1 in 10 have any symptoms. It is important, of course, for such patients to be aware of the difference between the arms only to remember which arm (the higher arm systolic number) has the correct measurement (most often, but not always, the right). -- Jerry Svoboda, MD, FACS
ANSWER: I thank Dr. Svoboda for his expertise and will add difference in arm blood pressures to my (already very long list) of reasons to quit smoking immediately.
*** DEAR DR. ROACH: I’ve heard many medical providers say that they believe they get more accurate blood pressure readings from the old-fashioned manual method, rather than the automatic cuff machines. I wonder how true this can be, though, since the former is subject to human hearing ability. My excellent hearing lets me pick up on beats far beyond what someone who has average hearing can detect, so it stands to reason that there would be a difference in the reading. -- C.B.
ANSWER: The very best machines are accurate at measuring blood pressure; however, a trained clinician remains the standard for blood pressure measurement. Fortunately, good, but not necessarily outstanding, hearing is required for accurate blood pressure measurement. Proper placement and inflation of the cuff, the correct slow deflation rate and, importantly, measuring both arms are as significant as good hearing. I was taught to take an average of three measurements.
Excellent hearing helps the clinician pick up on subtle heart murmurs and other noises, normal and abnormal, that the heart makes. If you decide to become a physician, consider cardiology. We physicians should take great care to protect our ears; sadly, some of us listened to music that was too loud at some points in our lives.

I am at the end of my patience . I go to one of the churches in [local town]. We have had SO many items stolen from our property it isn’t funny. We have had ladders, shovels, extension cords, ice melt, snow shovels stolen from our property over the last few years. The last straw just happened within the last week-- someone has stolen all of the screens right off our windows!! How can someone be so low? We will now no longer have the ability to open our windows and vent our dining hall (the windows are near ground level and the squirrels of the area
want to come in) when it is so hot during any of our community functions. It is so sad that someone feels that they have the right to steal someone else’s property, especially a church. We are looking into installing surveillance cameras!
I want to think people are kind and treat each other with respect. But this is not always true. I have noticed this as I get older. I noticed and heard the mean nasty comments and dirty looks people would give my dad with Parkinson’s disease. He would always like to walk through our local grocery store and go to the bakery. As his daughter & caregiver I would take him with me everywhere. He was my buddy. I would notice people staring at him and making fun of him because his head would wobble and his arm would shake. That’s what Parkinson disease does. I always worried he knew what was going on. Sadly my best buddy passed away a few years ago. I still see this going on and it’s so wrong. What has happened to being kind?! The same thing could happen to any of us. Think About It!

Holley student sworn in as Gates police officer for the day
Monday, May 23, started out as a normal day of school for 18-year-old Nick Pifer, but it would end in a remarkable way. Nick, a special needs student, graduated from Holley last year, but he remains in the Service Learning Program at Holley so he can receive job training. He cheerfully works in the MS/ HS Dining Hall making sandwiches, and he was a player on the district’s Unified Basketball team this year. He has also befriended Officer Shawn O’Mara, a police officer from the Gates Police Department, who works part-time for the Holley School District as a C.O.P.S. security officer. Nick is fascinated by law enforcement and constantly talks to Officer O’Mara about his job.
“We don’t talk about sports or anything else,” said Officer O’Mara. “We just talk about police work. We watch police takedown videos in the security office and Nick loves it. At one point, he asked to see my badge and ID to confirm my identity. Nick always has a smile on his face and it’s just contagious. I can come in from having a tough day, but he instantly cheers me up. He’s been asking if he could do a ride-along all year and I told him we’d see.”
Nick was part of the crowd of students who filed into the High School Auditorium for a surprise assembly on May 23. There was a lineup of law enforcement officials on the stage, many of them Holley alumni. Officer O’Mara called Nick up to the stage and Nick calmly sat down at the end of the row of law enforcement officers like he was already part of their community. Nick was called over to the podium by Officer O’Mara and told that he would finally have his wish granted: Nick would be sworn in as an honorary police officer for the day and ride along with Officer O’Mara in a Gates Police Department vehicle. Nick would even receive an official uniform, complete with his name, “Officer Nick,” embroidered on the shirt, and wear a hat and badge.
Students erupted in applause and cheers for Nick when he reappeared on stage in the uniform and tipped his hat to the audience. Nick was then sworn in by Gates Town Supervisor Mark Assini while Gates Police Chief James VanBrederode held the Bible. Once Nick was sworn in, students, faculty and staff gave him a standing ovation.
After posing for some photos onstage and being interviewed by the media, Nick happily waved from the front seat of Officer O’Mara’s police car as they hit the siren and left the Holley school campus. Nick spent the day with Officer O’Mara answering calls; making stops in Gates; and most importantly, visiting Dunkin’ Donuts to eat as many free donuts as he wanted.

Making new friends after a move or other life event
Downsizing and other life changes often find seniors leaving their comfort zones to move to new neighborhoods or regions of the country. It can be difficult to leave those comfort zones behind, especially when it means saying goodbye to close friends or family members. Establishing new social circles as a senior can be challenging. But with a little effort and the right attitude, seniors can meet new people and enjoy the excitement that comes with new friendships.
· Join a club. If you have a particular hobby or interest, rekindle it in your new location. Find a local gardening club, church-sponsored organization or fitness center where you can meet like-minded men and women. Ask the real estate agent who helped you relocate to make suggestions on where to find community information and read community notices in the local newspaper.
· Get a dog. Dogs make great companions inside of the house and also serve as an ice breaker when you are outdoors. Take plenty of walks and take advantage of opportunities for conversation when people come up to you to inquire about your dog. Explain your situation and you may make some new friends along the way.
· Volunteer your time. Many people make new friends through volunteering. Volunteer and you’re likely to meet people who share the same interests as you. Sign up with a favorite charity or volunteer at nonprofit events and look for familiar faces. Start talking to those people you meet again and again.
· Participate in church events. Places of religious worship are often cornerstones of a community, and they frequently host different events to get parishioners or members together. Read the bulletin and get involved in pot lucks, retreats, movie nights, and other church-sponsored events.
· Work at a school. Schools also serve as hubs of community activity. Volunteer or work for a local school and you will soon find yourself immersed in your community’s weekday hustle and bustle. This is a great way to meet people and learn more about your new neighborhood in the process.
· Host your own party. Go out on a limb and plan a “new to the neighborhood” party. Put invitations in neighbors’ mailboxes and invite everyone over for snacks and cocktails. Remember, neighbors may be just as nervous about new faces as you are, and a party is a great way to break the ice.
Change can be hard for people seniors starting out in new communities. With some gumption and a few strategies to get started, anyone can expand their circle of friends.
