• Reviewed areas of improvement from the previous year to prepare for reporting

• Worked with staff in each program area to collect the most accurate information

• Each measure staff reported on was Not Met, Partially Met or Fully Met

• Director from each agency reviewed all the measures and instructions together

• Communication with the Governing Entity (CHB)

• Purpose of reporting is to provide consistent quality information for ongoing evaluation and state and local decision making

• Highlight strengths and areas needing improvement

• Carve out time to make reporting a priority

• Don’t wait until the last minute

• Stress the importance of reporting to staff

• Results from CHB report are only as good as the data collected

• Use the report to identify areas for improvement going forward

• A packet containing all of the responses is compiled

quality improvement and performance measures • Emphasize all four counties report into PPMRS • Report compares North Country to public health

agencies throughout the state. • Results from Community Health Assessment, CTC

& SHIP evaluations relate to PPMRS report data
