Nature and Purpose of AgencyGrace Plaza Nursing and Rehabilitation Center was established in 1972. It is located in a five story building in Western Nassau. It is conveniently located and is easy to get to from major roads and highways, the Long Island Railroad, and area hospitals. Grace Plaza, is a 214 short and long term resident facility and offers progressive, quality healthcare in an atmosphere that speaks to the unique needs and challenges of every individual in our care. We have learned to understand and respond to the complicated needs of the families we serve. Our programs and services are designed to provide education, support and counsel in every aspect of care.
Tasks Available for StudentsStudents will be asked to work along with the recreation department in the direct care of the residents. A hands on approach is encouraged, to increase student awareness of the environment, resident needs and the approached utilized to enhance resident ability and wellbeing

NYS Department of Health has implemented that every volunteer must get the flu vaccination. Documentation of vaccination must include the name and address of the individual who ordered or administered the vaccine and the date of vaccination. This must be given to the nursing home before starting service.
Students must be informed that there is to be NO FOOD and NO FLUIDS given to any resident at any time. Also, if students are not sure of any situation at anytime during the visit to reach out to the nurse or administrator at the home.

Heart Share is a dynamic, caring agency serving children, adults and families city-wide.First Step serves children with and without special needs from three to five years of age.
Tasks Available for StudentsA wide variety of center-based services are available in our Preschool program for children ages three to five. First Step serves pre-schoolers with speech and language delays, behavior disorders, developmental disabilities, physical challenges, autism, as well as typically developing children.

Nature and Purpose of Agency
IRI is striving to create opportunities to meet the needs of each person we currently support as well as provide an array of support to individuals with developmental disabilities and their families. We have kept our existing programs and services at a high level of quality through continuous quality improvement measures by regular staff and staff of our Quality Improvement department. IRI has gone out in front of current service designs to create new opportunities for support residentially and have established our own restaurant, the Independence Café, which provides a wide range of work experiences to people with vocational interests. Finally, our Family Support Services strive to anticipate the need of families who may need respite for caring for their loved one with developmental disabilities, financial assistance, and a range of crisis supports. Services that can proactively reduce the likelihood of a crisis by altering the activities of the focus person to avoid the setting where past crisis occurred, or just by providing more attention that in doing so reduces the likelihood of an occurrence.
Tasks Available for Students
Tasks and activities available for Students will be tailored to the individual and the program. Population supported are individuals with disabilities whose diagnosis include but are not limited to Autism, Physical limitations, Cognitive delays, personality disorders and a vast array of psychiatric and behavioral challenges. Responsibilities include:
Work with an ABSS (Applied Behavioral Science Specialist) under the supervision of a BCBA Board Certified Behavior Analyst
Assist with the development of a behavioral Support Plan (BSP) inclusive of conducting FBAs ( Functional Behavioral Analysis )
Data Collection and analysis Assist ABSS in conducting staff training sessions Assist in peer mentoring and social skills training groups Assist in the development of person centered valued outcomes
Exposure to Interdisciplinary team meetings, Human rights committees, Self advocacy groups, Incident review committee and Continuous Quality Learning
Opportunities are also available to serve with IRI’s Mobile Crisis Team
Time Requirements
Weekday Program: 9:00 am to 4:00 p.m. (minimum three hour commitment per day) with a minimum of a four to six week commitment. Other times are available upon request, need and flexibility of schedule.

Nature and Purpose of AgencyVolunteers at Ozanam Hall perform many valuable services and give of themselves in numerous ways to better the quality of life for our residents, both physically and spiritually.
Tasks Available for StudentsVisiting residents to lend friendship and compassion; Assisting with various activity programs; Accompanying residents on outings; Transporting residents to activities (Mass, rehab, patio visits, etc.); Assist residents with feeding through our Loving Spoonful Program; Assist visually impaired residents by distributing and reading mail; Teaching computer skills to residents; Participate in Music Therapy with residents; and assist staff with light clerical work.

Nature and Purpose of AgencyQCH is a multi-service settlement house that serves nearly 25,000 children, youth, adults and older adults at 22 different sites in 11 neighborhoods throughout Queens. Queens Community House provides individuals and families with the tools to enrich their lives and build healthy, inclusive communities.
Description of Opportunities with Youth The QCH Pomonok Center and Forest Hills Center provide free After-School Programs for children in the community from Kindergarten through sixth grade from September through June. Children are able to enjoy an array of hands-on educational and recreational activities that encourage academic success, positive decision making, and positive relationship building. Free summer camp for children ages 6-10 in July and August. Children can enjoy a summer full of: academic enrichment, sports, games, arts ‘n’ crafts, fitness, field trips, and many other exciting hands-on educational and recreational activities.
Tasks Available for Students:Assist with after-school and summer camp programs. Opportunities to develop workshops and educational activities for students and campers are available.
Time Requirements:90 hours during the school year or summer months. Weekdays only.

Description of Opportunities with Older Adults:The QCH Rego Park Senior Center serves a hot lunch Monday through Friday to community members 60 and over. The QCH Kew Gardens Community Center offers computer lab time for community members to email, print, use the internet, etc.
Tasks Available for Students:Assist with serving meals and interacting with seniors at Rego Park Senior Center. Assist with computer questions and concerns at the Kew Gardens Community Center.
