This packet contains information students should read before applying for a scholarship administered by the Lake Park High School Educational Foundation. Students will find information about the various scholarships available and the specific requirements for each scholarship.

Following is a brief description of the Foundation-administered scholarships.

Aisling Scholarship - $1000 will be awarded to a graduating student planning to attend a four-year college/university, community college, technical institute, or accredited trade school who plans to major in an area focusing on public service, philanthropy, or nonprofit management. (Pages 5-6)

Altier Scholarship - $500 will be awarded to four graduating students planning to pursue a two-year degree program at the College of DuPage, another community college, or an accredited trade school. (Pages 7-8)

Alumni Scholarship - $500 will be awarded to two graduating students planning to attend an accredited trade school, technical institute, community college, or four-year college/university. (Pages 9-10)

Amanda Jane Dombrowski Memorial Scholarship - $2000 will be awarded to a graduating student planning to attend an accredited trade school, technical institute, community college, or four-year college/university leading to a certificate or degree; who is a current member of the Lake Park High School girls’ soccer team displaying leadership and sportsmanship while serving as a positive role model for younger players; and who displays a passion and commitment to the LPHS soccer program. (Pages 11-12)

Anthony Bonavolonta Family Scholarship - $2000 will be awarded to a graduating student planning to attend an accredited four-year college/university or community college, who demonstrates financial need, and who plans to major in fine and/or applied arts, including architecture, landscape design, industrial design, graphic design, art, urban planning, theater, or music. (Pages 13-14)

Barclay Family Scholarship - $1000 will be awarded to a graduating student planning to attend a four-year accredited college/university, community college, technical institute, or accredited trade school. (Pages 15-16)

Chris Frizzelle Memorial Scholarship - $500 will be awarded to two graduating students who are currently members of the boys’ soccer team; who overcame or are attempting to overcome some type of challenge; and who plan to attend an accredited trade school, technical institute, community college, or four-year college/university leading to a certificate or degree. (Pages 17-18)

Cychosz Scholarship - $750 will be awarded to a graduating student planning to attend an accredited college/university program leading to a certificate or degree, who demonstrates financial need, and who plans to major in education, public service, engineering, or science. (Pages 19-20)

Don Lovelace Memorial Scholarship - $1000 will be awarded to a graduating student planning to attend an accredited four-year college/university program leading to a certificate or degree and major in health, driver education, physical education, or related fields. (Pages 21-22)

Dr. Gary Annunziata Scholarship - $500 will be awarded to two graduating students planning to attend the College of DuPage. (Pages 23-24)

G. H. Elliott Memorial Scholarship - $500 will be awarded to a graduating student who has been accepted as a full-time student in an industrial technology or vocational degree or certificate program at an accredited school. (Pages 25-26)

The Gene Haas Foundation Scholarship - $1000 will be awarded to four graduating students who plan to attend an accredited trade school, technical institute, community college, or four-year college/university leading to a certificate or degree in manufacturing, machining, or engineering. (Pages 27-28)

Dan Hildebrandt Memorial Scholarship - $500 will be awarded to one graduating student on the boys’ tennis team, one graduating student on the girls’ tennis team, and one graduating student on the girls’ badminton team (3 total scholarships) planning to enroll in an accredited trade school, technical institute, community college, or four-year college/university leading to a certificate or degree. (Pages 29-30)

Itasca Lions Club Scholarship - $500 will be awarded to two graduating community service-oriented students from Itasca who are planning to attend an accredited four-year college/university or community college and who preferably desire to pursue a field of study that would help them foster a more humanitarian world. (Pages 31-32)

Lake Park Foundation EXCEL Scholarship - $1000 will be awarded to four graduating students who demonstrated excellence while at Lake Park High School; who plan to enroll in a four-year college/university or junior/community college; and who participated in athletics, activities, and or community service activities. (Pages 33-34)

Lake Park Foundation Gene Swierczewski Scholarship - $1000 will be awarded to a graduating student planning to attend an Illinois public state college or university and major in a foreign language, English, political science, sociology, or communications. (Pages 35-36)

Geoff Voigts Memorial Scholarship - $1000 will be awarded to a graduating student who is currently a member of the boys’ soccer team, who demonstrates financial need and who plan to attend an accredited trade school, technical institute, community college, or four-year college/university leading to a certificate or degree. (Pages 37-38)

Scott Bradley Memorial Scholarship - $1500 will be awarded to a graduating student who was a member of the Lake Park High School wrestling team that plans to attend an accredited four-year college/university program leading to a degree. Please note that this scholarship application is due on Tuesday, March 3, 2020. (Pages 39-40)

***IMPORTANT: STUDENTS INTERESTED IN APPLYING FOR ANY OF THESE SCHOLARSHIPS SHOULD***

1. Read the detailed description of the scholarship(s) they are interested in (see page numbers above) and note the specific requirements for that/those scholarship(s).

2. Complete the Common Scholarship Application – available at www.lphs.org /foundation .

3. Answer the specific essay question(s) for the scholarship(s) for which they are applying.

4. For each scholarship being applied for, print a hard copy of (a) the completed Common Scholarship Application; (b) the essay(s) for that scholarship; and (c) the required letter(s) of recommendation for that scholarship. (Note: Students applying for more than one scholarship may use a recommendation letter more than once as long as it meets the requirements of a particular scholarship.)

5. Turn all materials noted in #4 to Ms. Mina Field in the West Campus counseling office by Monday, March 9, 2020. The only exception is for the Scott Bradley Memorial Scholarship which needs to be submitted to the counseling office by Tuesday, March 3, 2020.

NOTE: Applications will not be considered if incomplete of if the appropriate support materials are not turned in by the identified deadline date for the scholarship for which a student is applying.

Lake Park High School Educational Foundation

The Lake Park Educational Foundation is pleased to announce the availability of a $1,000 Aisling Scholarship, established in memory of the Corbett triplets, to provide financial assistance to a student who is planning to pursue his/her post-secondary education at a four-year college/university, a community college, a technical institute, or an accredited trade school who plans to major in an area focusing on public service, philanthropy, or nonprofit management.

To be eligible for the Aisling Scholarship, the applicant must:

• be a Lake Park High School graduate of the Class of 2020.

• have a current cumulative grade point average of at least 3.5 on a 5.0 scale.

have been involved in community service/volunteer activities and have demonstrated leadership skills/qualities in school or community activities.

• complete the Common Scholarship Application.

• submit an unofficial transcript of all high school classes.

• submit two letters of recommendation from teachers.

• submit the essays noted on the following page.

The completed application and required support material must be submitted to the West Campus Career Center by no later than Monday, March 9, 2020. The Aisling Scholarship will be awarded to the selected recipient at Scholarship and Awards Night in May 2020.

A $1,000 check will be processed after the recipient has submitted a thank you note to the scholarship donor and a copy of a tuition bill showing enrollment as a full-time student in an accredited trade school, technical institute, community college, or four-year college/university program leading to a certificate or degree. Please send this documentation to the Lake Park Educational Foundation at 590 S. Medinah Road, Roselle, Illinois 60172. The check will be issued after July 1, 2020, and the request must be filed no later than June 30, 2021.

Essay #1

In 250 words or less, identify a community issue that is important to you, briefly state why this issue is important, and identify a plan that you would like to implement to make an impact on this issue.

Essay #2

Describe in 250 words or less a significant community service/volunteer experience or other service-related activity you participated in, what you learned from this activity, why you chose this activity, and how you will carry over your experience into the career you choose to pursue.

Lake Park High School Educational Foundation

The Lake Park Educational Foundation is pleased to announce the availability of the Altier Scholarship to provide financial assistance to students planning to pursue a two-year degree program at the College of DuPage/another community college or attend an accredited trade school. Four (4) $500 scholarships will be awarded.

The Altier Scholarship, established by Noreen Altier, honors Lake Park community residents Donald and Donny Altier.

To be eligible for the Alter Scholarship, the applicant must:

• be a Lake Park High School graduate of the Class of 2020.

• plan to enroll in a two-year degree program at the College of DuPage/another community college or attend an accredited trade school.

• have a current cumulative grade point average of at least 4.0 on a 5.0 scale.

• complete the Common Scholarship Application.

• submit an unofficial transcript of all high school classes.

• submit a letter of recommendation from a teacher.

submit the essays as noted on the following page.

The completed application and required support material must be submitted to the West Campus Career Center by no later than Monday, March 9, 2020. The Altier Scholarship will be awarded to four selected recipients at Scholarship and Awards Night in May 2020.

Lake Park High School Educational Foundation

The Lake Park Educational Foundation is pleased to announce the establishment of an Alumni Scholarship to provide financial assistance to a student who is planning to pursue his/her post-secondary education at a community college, a four-year college/university, an accredited trade school, or a technical institute. Two (2) $500 scholarships will be awarded. The Alumni Scholarship honors Lake Park alumni whose generosity over the years has provided a foundation of support for students seeking to continue their education beyond Lake Park High School

To be eligible for the Alumni Scholarship, the applicant must:

• be a Lake Park High School graduate of the Class of 2020.

• have a current cumulative grade point average of at least 4.0 on a 5.0 scale.

• be enrolled in a four-year college/university, junior/community college, accredited trade school, or technical institute.

• be a participant in athletics, activities, and/or community service activities at Lake Park.

• complete the Common Scholarship Application.

• submit an unofficial transcript of all high school classes.

• submit a letter of recommendation from a teacher.

submit the essays as noted on the following page.

Lake Park High School Educational Foundation

The family of Amanda Jane Dombrowski and the Lake Park Educational Foundation are pleased to announce the availability of one scholarship in her memory. Amanda graduated from Lake Park High School in 2001 and later attended the University of Iowa. While at Lake Park, she was an integral member of the girls’ soccer team. Amanda lived with great passion for her family and friends, which carried into her life as a teacher, mentor, and caregiver for children. In her memory, Amanda’s family wishes to award one $2,000 scholarship to a deserving Lake Park High School student.

To be eligible for the Amanda Jane Dombrowski Memorial Scholarship, the applicant must:

be a Lake Park High School graduate of the Class of 2020. be a current member of the Lake Park High School girls’ soccer team who has displayed

leadership and sportsmanship and has served as a positive role model for younger players. have displayed passion and commitment towards the LPHS soccer program. plan to attend an accredited trade school, technical institute, junior/community college, or four-

year college/university program leading to a certificate or degree. have a current cumulative grade point average of at least 3.5 on a 5.0 scale. complete the Common Scholarship Application. submit an unofficial transcript of all high school classes. submit a letter or recommendation from a teacher. submit the essays noted on the following page.

The family of Anthony Bonavolonta and the Lake Park Educational Foundation are pleased to announce the availability of a scholarship to provide financial assistance to a student who is planning to pursue his/her post-secondary education at an accredited four-year college or junior/community college.

The Family of Anthony Bonavolonta Scholarship was established in honor of Bonavolonta family members and Lake Park alumni Anthony (Class of 1980) and Jan (Class of 1981). One $2,000 scholarship will be awarded to a deserving Lake Park High School student.

To be eligible for the Family of Anthony Bonavolonta Scholarship, the applicant must:

be a Lake Park High School graduate of the Class of 2020. planning to focus on the fine or applied arts, majoring in one of the following areas:

Architecture, Landscape Design, Industrial Design, Graphic Design, Art, Urban Planning, Theater, Music

demonstrate financial need (attach a copy of the first page of your parents’ most recent federal tax return to this application).

participate in a Lake Park High School club, sport, or activity for at least 6 semesters. plan to attend a four-year college/university or a junior/community college program leading to a

degree. be in the top 25% of the graduating class. complete the Common Scholarship Application. submit an unofficial transcript of all high school classes. submit a letter of recommendation from a teacher. select and submit two essays as noted on the following page.

The Lake Park Educational Foundation is pleased to announce the availability of a $1,000 Barclay Family Scholarship to provide financial assistance to a student who is planning to pursue his/her post-secondary education at a four-year college/university, a junior/community college, a technical institute, or an accredited trade school.

The Barclay Family Scholarship was established in honor of Barclay family members Kathy (Barclay) Goedeker (Class of 1971), Andy Barclay (Class of 1974), Betty (Barclay) Bentley (Class of 1975), Kelly (Barclay) Deterding (Class of 1979), Gavin Barclay (Class of 1980), and Ellen (Barclay) Watkins (Class of 1982), all alumni of Lake Park High School, and Kay (Barclay) Cahill who was a Lake Park High School staff member for 27 years and served for 6 years as secretary on the Lake Park High School Educational Foundation’s Board of Trustees.

To be eligible for the Barclay Family Scholarship, the applicant must:

• be a Lake Park High School graduate of the Class of 2020.• have a current cumulative grade point average of at least 3.5 on a 5.0 scale. have been actively involved in Lake Park athletics and/or activities for at least 4

semesters.• complete the Common Scholarship Application.• submit an unofficial transcript of all high school classes.• submit a letter of recommendation from a teacher.• submit an essay as noted on the following page.

The completed application and required support material must be submitted to the West Campus Career Center by no later than Monday, March 9, 2020. The Barclay Family Scholarship will be awarded to the selected recipient at Scholarship and Awards Night in May 2020.

The family of Chris Frizzelle and the Lake Park Educational Foundation are pleased to announce the availability of a scholarship in his memory. Chris was a valued member of the Lake Park High School staff for twenty-one years. In addition to working in the Technology Department, he was also an integral part of the Lake Park soccer team for many years, where he served in a variety of capacities, including assistant coach and announcer for the boys’ team and head soccer coach and announcer for the girls’ team. In his memory, his family wishes to award two $500 scholarships to deserving Lake Park High School students.

To be eligible for the Chris Frizzelle Memorial Scholarship, the applicant must:

be a Lake Park High School graduate of the Class of 2020. be a current member of the Lake Park High School boys’ soccer team. have overcome or is attempting to overcome some type of challenge

(i.e., personal, family, or financial). plan to attend an accredited trade school, technical institute, junior/community college, or four-

year college/university program leading to a certificate or degree. complete the Common Scholarship Application. have a current cumulative grade point average of at least 3.5 on a 5.0 scale. submit an unofficial transcript of all high school classes. submit a letter of recommendation from a teacher. submit the essays as noted on the following page.

Dr. Annunziata, who graduated from Lake Park High School in 1979, was the personal physician to such famous Americans as actor Bob Hope. He is the founder and President of Desert Gastroenterology Consultants, has invented a non-surgical weight loss device, and founded a $300 million private-funded hospital in Palm Springs, California. Dr. Annunziata established this scholarship to assist two students planning to attend the College of DuPage, which he attended after graduating from Lake Park High School. Two $500 scholarships will be awarded to students planning to attend the College of DuPage.

To be eligible for the Dr. Gary Annunziata Scholarship, the applicant must:

• be a Lake Park High School graduate of the Class of 2020.

Gaylon H. Elliott was one of the founding faculty members of Lake Park High School and was a teacher/department chairman of the Industrial/Vocational/ Technology Department for approximately thirty years. Mr. Elliott assisted and guided many young people in reaching a decision to select industrial and vocational technology as their occupational intent. A $500 scholarship will be awarded to a graduating student who has been accepted as a full-time student in an industrial technology or vocational degree or certificate program at an accredited school.

In 1999 Gene Haas founded the Gene Haas Foundation. Haas is the owner of Haas Automation, Inc., America’s leading builder of CNC machine tools, which he started in 1983. Haas Automation is now a billion dollar company and this extraordinary growth has all come in an era when Americans were being told that the United States doesn’t manufacture anything anymore. A recent report, called “The Skills Gap in U.S. Manufacturing 2015 and Beyond,” projects that, “Over the next decade, nearly three and a half million manufacturing jobs will likely need to be filled, and the skills gap is expected to result in 2 million of those jobs going unfilled.” Gene’s commitment to the importance of US manufacturing has incited him to focus on manufacturing education in the form of scholarships for CNC machinist training. The Itasca Lions Club is actively involved in helping individuals who have visual or hearing impairments and fosters volunteering to aid the environment and global disaster relief. The club works locally to provide support for youth, the handicapped, and senior programs.

Geoff Voigts graduated from Lake Park High School in 1993 and was an integral part of the boys’ soccer team. He played soccer with dedication, while always balancing his love for the game with his commitment to academics. Throughout his adult life Geoff maintained an incredible passion and respect for the game of soccer. Lake Park High School Educational Foundation

Scott Bradley was a Lake Park student and wrestler whose connection with the school began many years before, when he wrestled with the Lancer Kids Club as a young child. Scott was a gentleman who displayed sportsmanship, loyalty, and dedication to the Lake Park wrestling program. He constantly strived for excellence through hard work on the mat and in the classroom.

Unfortunately, Scott’s wrestling career ended suddenly during his junior year when he was diagnosed with leukemia. For six months, Scott battled his illness with the same determination that he battled his opponents on the mat. On May 5, 1986, the Lake Park community suffered a great loss. The following year his parents, Mr. and Mrs. Olav Bradley, established the Scott Bradley Memorial Scholarship as a loving tribute to their son. In his memory, his family wishes to award one $1,500 scholarship to a deserving Lake Park High School student. The student athlete considered for this award must display similar characteristics to Scott: loyalty, commitment, dedication, and a strong work ethic.

To be eligible for the Scott Bradley Memorial Scholarship, the applicant must:

be a Lake Park High School graduate of the Class of 2020. be a member of the Lake Park High School wrestling team who has shown passion and a

commitment to the program while displaying leadership, sportsmanship, dedication, and a strong work ethic.

plan to attend an accredited four-year college/university program leading to a degree. have a current cumulative grade point average of at least 3.5 on a 5.0 scale. complete the attached application form. submit an unofficial transcript of all high school classes. submit a letter of recommendation from a teacher. submit the essays as noted on the following page.

The completed application and required support material must be submitted to the West Campus Career Center by no later than Monday, March 2, 2020. The Scott Bradley Memorial Scholarship will be awarded to the selected recipient at Scholarship and Awards Night in May 2020.

It is the responsibility of the scholarship winner to determine that this award from the Lake Park Educational Foundation will not interfere with NCAA eligibility.
