the judges and relevant individuals as part of the awards. By submitting this information you are giving consent for its use by the IOSH South Wales Branch for the purposes of

its awards process. By submitting this nomination you are confirming that the nominee has been made aware and has given their consent

to the nomination Once nominations have been submitted, they will be assessed by a panel of judges and a shortlist will then be

created. Shortlisted nominees will be contacted prior to the Annual AGM, and winners for each category will be announced during the awards ceremony, which will be held after the Annual AGM.

Do you work with someone who has made a significant contribution to the implementation or enhancement of health, safety or wellbeing standards within their own or a client organisation? The Rising Star Award is aimed at an individual who:

has demonstrated enthusiasm, passion and initiative for health, safety and wellbeing. has only started working in the health and safety profession within the last 5 years works in a position up to Junior Management has demonstrated a positive influence to enhance health, safety and wellbeing.

Submission Criteria:

Q1. In no more than 250 words, please describe how the individual has implemented health or safety strategy and has had a positive influence on health, safety and wellbeing within an Organisation. (30 Points)

Q2. In no more than 250 words, please give evidence of how this has ensured a safer and healthier working environment.
