Phonics Assessment
This is a sample Phonemic and phonics assessment that might provide instructional information about the literacy skills of your students. This is a fairly short survey. You can use this assessment as a model to develop a more expanded version for progress monitoring or to develop a diagnostic test for your struggling readers. You can start with Part 5 if you have older students and you are focusing on phonics skills.

Consider using the following criterion levels to place students, and identify areas of concern:1. Independent (95% or higher)2. Instructional –build mastery and fluency (75 – 95%)3. Instructional – Deficit area that you will need to strengthen through explicit teaching (< 75%) (e.g., analyze specific elements
that are unknown, teach, and reteach)
Continue testing until you identify at least 2 or more phonics skill areas that are areas of concern
The student test materials can be found at the end of this packet. This packet contains both the examiner’s copy of the test, as well as the student test materials.

Students should be assessed individually 3 or 4 times per year. The teacher is looking for student progress. If little progress is seen, the child should be given intervention strategies.

3. Name the medial Vowel Sounds in spoken words: "Listen carefully to the words I say to you. Tell me the vowel sound you hear in the middle position of each word." So if I said “bug”, you would say /u/ because that is the vowel sound in the middle of the word. Listen to these words and tell me the short vowel sound in the middle of the words. (requires phoneme segmentation skills & knowledge of vowel sounds)
