Job Description
JOB TITLE: Young Stroke Care Services Co-Ordinator
(Belfast and South Eastern Trust Areas)
ACCOUNTABLE TO: Director of Care
RESPONSIBLE TO: Area Services Manager
HOURS PER WEEK: 35 Hours per week
BASE: Service Venues & Home
SALARY: £ 20,661 p.a
Contract: 12 month Maternity cover
KEY RESPONSIBILITIES
You will work closely with local hospital and community stroke teams, as well as NICHS colleagues, you will make a difference to younger stroke survivors and their families by supporting them in achieving outcomes which will enhance their quality of life and independence. You will also work with NICHS management team, local trusts and services users to develop the service model for Young Stoke survivors.
You will be providing a needs based support service to people affected by stroke which will include:
Working with our Family Support Service delivered in multi-disciplinary settings; Co-ordination and delivery of our Post Rehabilitation Exercise Programme; Co-ordination and delivering of Young Stroke Activity Groups; Service Development to ensure service user and funder need is met Managing teams of volunteers

DUTIES INCLUDE:
Client Specific:
1. To support the implementation of the NICHS Outcomes Framework and to contribute to its continuing development to meet the needs of survivors and carers in the future.
2. To receive, assess and action referrals against established criteria; 3. To undertake assessments and set goals with clients using standardised tools; 4. To provide listening, understanding, advice, information and support to clients and
their families; 5. To liaise with health professionals and others, and play a role within multi disciplinary
teams, to ensure the assessed needs of clients and their carers are met; 6. To establish a strong network base, and refer onto other services as appropriate(both
internal and external); 7. To deliver NICHS’ Post Rehabilitation Exercise Programme in the community, as per
policies and procedures;8. To maintain established regular NICHS’ Young Stroke Activity Groups, and deliver a
client focussed programme;9. To plan and deliver group activity sessions in line with NICHS policies and
procedures;10. To encourage an ethos of self management and empowerment where appropriate, to
act as advocate for the client and to ensure that client opinions are sought regularly on the services provided;
11. To assist in the delivery of other services as required
Reporting and recording:
1. To prepare reports as required for the management team of NICHS and appropriate Health and Social Care Trusts;
2. To maintain client records and files as required, in line with NICHS policies and procedures;
3. To input into the maintenance of NICHS’s Client Relationship Management System; 4. To set goals with clients, maintain individual progress reports for each client and
organise reviews accordingly;5. To assist in the development of service budgets while ensuring that financial
procedures are adhered to and comply with NICHS accounting procedures and; 6. To ensure that confidentiality of information is maintained at all times
Recruitment, Selection and Management of Volunteers
1. In conjunction with the Volunteer Development Coordinator, recruit, select, induct and train a pool of volunteers to assist in the provision of services to clients and their carers in your service areas;
2. To supervise and support the volunteers in all matters relating to the services provided;

3. To maintain volunteer records including attendance at training and development sessions;
4. To ensure all volunteer expenses are accurate and claimed on monthly basis within the accounting procedures timeframe.
Health & Safety
1. To observe safe working practices and to comply with NICHS Health and Safety policies;
2. To monitor and control hazards in the workplace; and3. Undertake regular risk assessments.4.
Support Functions
1. To assist from time-to-time in the fundraising activity of the organisation.2. To liaise with local Fundraising Support Groups regarding common areas of work.3. Any other duties as requested by the Director of Care and Area Services Manager
which fall within the scope of the post.
*This post falls within ongoing NICHS Service development consequently there may be changes and amendments to some elements of the role and subsequent duties and responsibilities.
