ASSESSMENT INSTRUCTIONS
GuidanceThe candidates must complete this assessment as a crew of Driver and a Guard. Each must however write down their responseand then present their answers in turn. You will need to provide each person with the scenarios and paper to write down theirresponses. Ensure they write their name on the top of the papers and the scenario number.
Each Group will be given 2 scenarios to work through and 15 minutes to develop their solution. They are to work on theirindividual solution. Once the development time is up the responses are to be handed to the Assessor.
Each crew will be called into a room or to a location, given their responses back and then asked to present their solution to theAssessor.
Each candidate will have 2 minutes to present their solution (Driver then Guard Action). The trainer/assessor will check theirresponse against the marking guide. The Assessor can ask question to seek clarity to the information provided and/or to moderate
the responses. Moderated responses are to be added to the written responses.
When all of the groups have presented their solutions the Assessor will provide feedback to the candidates on all of the scenarios.
Where a candidate has not provided sufficient evidence in their response the Assessor may:
Give them a new scenario to work through and represent or
Orally question the them against a new scenario to draw evidence of competence
Guidance on Conduct of Assessment Prepare assessment forms so there is a front and second page and 2 scenarios per candidate. Pack them into crew
groups with scenarios 1 and 2 for the first group and then so on.
Issue assessment forms and paper to crew groups.
Read out the Guidance to Candidates
Use effective questioning technique to probe the information provided to gauge the depth of understanding of their roleand relationship with the Train Captain and operation of the train. The questions may include as examples:
- If the Train Captain had a different assessment that you, then what authority would you state to supportyour position?
- How did you come to that conclusion?
- If the Train Captain asked you to proceed to the next station what would be some of your safetyconsiderations and how would you monitor those?
- As the Guard on the train, did you consider any options that may assist in resolving the problem?
- If you thought of that alternative or solution how would you have communicated that to the Driver orTrain Captain?
Once you are satisfied that sufficient evidence has been gathered from the crew on that scenarios, rotate to the nextcrew and apply the same process.
Once all the groups have presented their solution to their scenarios Provide feedback to all candidates on all of thescenarios.

Scenario 1 Evacuation
You are operating the PPTV when smoke is reported. The Train Captain requests the PPTV be
stopped and all personnel to be evacuated. You are midway between stations and it is 2AM in
the morning.
Your solution must include:
Driver:
Your action in the Cab prior to evacuation
- Hit the emergency button on the Metronet and inform Train Controller/Signaller of status- Inform Guard and direct as required- Liaise with Test Captain on actual status of the fire- Inform Train Captain of the best location for evacuation purposes- Coord with Train Captain on location and safety of personnel on the train- Take direction from Train Controller/Signaller on evacuation plan- Inform Guard and commence actions
Your actions on the network
- Establish protection as required- Driver maintain liaise with Train Controller/Signaller for as long as possible- Coordinate with Guard to contain and control personnel with the assistance of the Train
Captain
- Coordinate with Guard to confirm all personnel safely alight the rail corridor
When will personnel be allowed out of the PPTV, what you would do to reduce the effects on
personnel, is any?
- When safe to do so under instruction from the Driver-

issues. Provide first aid as required
- Guard to contain and control personnel with the assistance of the Train Captain- Guard confirm all personnel safely alight the rail corridor

Scenario 2 Faults - eTIS
You are operating the PPTV when you identify erratic air pressure levels on eTIS and a number
of alerts and alarms from eTIS. The Train Captain requests you to continue to the next station.
Your solution must include:
Driver:
When would you inform the Train Captain of the erratic air pressures on eTIS?
- After driver has conducted necessary brake evaluation actions
How would you assess the trains safe operation?
- Check analogue gauges to confirm the pressures are within minimum operatingrequirements
- Check brake and traction controls and check brake cylinder pressures are respondingaccordingly
- Work in line with RailCorp procedures
State what would be the basis of that assessment.
- Validate that brake actuates and responds on demand within minimum operating standards

- Driver in consultation with the Train Captain
Guard:
You are operating the PPTV when the Driver informs you that there are erratic air pressurelevels on eTIS and a number of alerts and alarms from eTIS. You are also informed that the

- Driver has total control over the brakes on the train- Driver has had consultation with the Train Captain- Possibility of a SPAD or overrun of a station or limit of authority- Possibility of having to operate the emergency brake valve

Scenario 3 Faults - Vibration
You are operating the PPTV when the train starts vibrating badly. The Train Captain requests
you to continue to the next station.
Your solution must include:
Driver:
What steps would you take to assess the trains safe operation?
- Inform the Train Captain that I am going to stop the train at a suitable location and conductan inspection with the Train Captain or the appropriate Engineer to ascertain the cause of
the vibration and impact on the safe running of the PPTV
- Inform the Train Controller/Signaller of the vibration and the need to do an assessment- Inform the Guard of intentions- Work under instruction of the Train Controller/Signaller and in line with RailCorp
procedures
State what would be the basis of that assessment.
- Safety of all personnel on board and the PPTV from derailment

what inputs would be your guidance?

Scenario 4 Faults High Tension Power Fault
You are operating the PPTV when the train looses high tension.
Your solution must include:
Driver:
What would be indications of likely cause?
- Numerous Fault indications related to traction and auxiliary systems- Unable to power or loss of power- Only Auto Brake available- May observe overhead lines faulty or down- Contact from the Train Captain and/or Guard confirming cause
State what may be directions from the Train Captain, based on your experience.
- Stop the train if still moving- Identify from the Drivers position what is the cause of the fault- Undertake a visual inspection of the train- Follow procedures to attempt to regain power- If fault not recovered, organise with the Train Captain for Dead Tow

Scenario 5 Faults Analogue Gauges Fail
You are operating the PPTV when the analogue gauges fail but the eTIS is still providing data.
The Train Captain requests you to continue to the next station.
Your solution must include:

- Minimum operating standard is still in place and can continue to operate

- Accurate information is provided from eTIS

- Informed Train Captain and if there was agreement that the train was still at minimumoperating standard and testing will not affect eTIS information, then continue to the next

Scenario 6 Faults Loss of HVAC
You are operating the PPTV when the No1 end HVAC fails. The Train Captain requests you to
continue to the next station.
Your solution must include:

- The level of failure; cooling, heating air movement etc and comparison to minimumoperating standard for the Drivers Cab
What would be the issues you would discuss with the Train Captain?
- The impact on the comfort level of the Drivers Cab- The ability of the Train Captain to rectify the fault- The impact on testing- Agreement on options and action

Scenario 7 Faults Crew Cab Door
You are operating the PPTV when the crew cab door fails to close and lock and cycles open and
close. The Train Captain requests you to continue to the next station.
Your solution must include:

- If other people in the cab, then come to an agreement with the Train Captain to continue atan agreed speed and when safe to do so ONLY TO A SUITABLE LOCATION TO
RECTIFY

- If the door can only be secured open then the impact on MOS

Scenario 10 Locked Axle
