National Capital Area Joint Service Graduation Ceremony

On Friday, 17 June, we celebrated the achievements and graduation of our interns, residents and fellows at The Strathmore Music Center, in Bethesda MD.

During the celebration, Rear Admiral David Lane, Director of National Capital Region Medical Directorate, delivered the Opening Remarks, an Introduction of Speaker was made by Dr. Arthur Kellerman, Professor and Dean, F. Edward Herbert School of Medi-cine, Uniformed Services University of the Health Sciences (USUHS), and the Graduation Address was delivered by COL Frederick Lough, Deputy Chairman, Department of Sur-gery, USUHS. We were honored to have the United States Army Band play the National Anthem and Recessional.

Internal Medicine presented 19 Interns completion of Internal Medicine certificates, and 24 graduating residents completion of IM residency certificates CAPT Bill Shimeall, Internal Medicine Program Director, presented the completion of Internal Medicine (IM) internship certificates to 19 interns (representing the Army, Navy, and Air Force) and completion of IM residency certificates to 24 graduating residents (representing the Army, Navy, and US Public Health Service). CDR (SEL) Casey Flanagan and Dr. Julie Chen, both IM Residency Associate Program Directors, joined Dr. Shimeall on the stage to present the certificates.

Internal Medicine presented 19 Interns completion of IM certificates, and 24 gradu-ating residents completion of IM residency certificates Several internship graduates will be stay-ing at Walter Reed Bethesda for residen-cies in IM or other specialties, but many will be serving Sailors, Soldiers and Ma-rines as Flight Surgeons, Undersea Medi-cal Officers, or General Medical Officers with Army Brigades, Navy Fleet or Fleet Marine Force for 2-3 years before return-ing to future GME training programs. Our residency graduates have completed three years of training in Internal Medi-cine, and are also going to several diverse locations. Many are advancing to fellow-ship training at Walter Reed Bethesda, Johns Hopkins, National Institutes of Health or NMC San Diego in an internal medicine subspecialty like pulmonary medicine, critical care, hematology/oncology, geriatrics/palliative care, infec-tious diseases, or cardiology. Others will embark on their first utilization tours as Staff Internists at such locales as Walter Reed Bethesda, Fort Belvoir Community Hospital (VA), Naval Hospital Camp Lejeu-ne (NC), Naval Hospital Guam, Naval Hos-pital Pensacola (FL), Brian Allgood Army Community Hospital (Korea), Naval Hospi-tal Yokosuka (Japan) and Army Special Operations Command at Joint Base Lewis-McChord (WA).

Erskine award is pre-sented annually to the outstanding resident and fellow as determined by a selection com-mittee. These awards are sponsored by Mrs. Graves B. Erskine in the name of her husband, General Graves Blanchard Erskine, who served in both World War I and World War II. A big congratulations to everyone involved in mak-ing the Internal Medicine Lampoon 2016 such a big success and so enjoyable. They stand out as leaders, scholars and mentors. They are exceptionally patient-centered and have a zeal for learning and teaching medicine. Residents and faculty from other programs routinely observe that they are impressed with "how our resi-dents consider it a privilege to learn medicine." They are invested in the success of both Walter Reed Bethesda and their residency program and are not afraid to speak up and take chances on efforts to make the clinical and learning environment better than when they found it. Above all they leave me with a great confidence that the future of Military Health Care will be safe and thriving for years to come. “

APDIM SUCCESS Association of Program Directors in Internal Medicine (APDIM) Spring Meeting

Poster Winner for Best Original Research

Internal Medicine: Innovative Curriculum of "Panel Management in the 4+1 System” Congratulations to Dr. Julie Chen, Internal Medicine Residency Associate Program Direc-tor, on her winning poster presentation for best original research at the Association of Program Directors in Internal Medicine (APDIM) Poster Reception for the 2016 APDIM Spring Meeting. The meeting was held April 19-20 in Las Vegas, NV.

The poster on 4+1 block scheduling and the effects on care quality measures in residents' patients is the winner for APDIM's poster competition for best original research. Data tying an educational intervention to impact on patient quality outcomes also made an impact in the medical education community. Dr. Chen, along with co-authors LT Nick Fiac-co (Internal Medicine PGY-3 Resident) and CAPT William Shimeall (Internal Medicine Program Director) submitted their abstract which was accepted as one of 82 Research and Programmatic Innovation Abstracts out of hundreds of submissions for presentation at the APDIM Spring Meeting Poster Session. APDIM CHIEF RESIDENTS MEETING Congrats Adam, Ben and Erin! The out-going Chief Residents, Adam Barelski and Ben Vipler, along with Erin Vipler (Chief Resident at George Washington University and Ben’s better half) presented two work-shops at the APDIM Chief Residents Meeting in Las Vegas this past April. Their workshop, "Human Knot Versus Happy Hour: Enhancing Teamwork, Resiliency, and Identity Formation during Orientation and Beyond," presented a framework for team building and explored both quantitative and qualitative data from Walter Reed Bethesda and George Washington about the effective-ness of a variety of orientation program activ-ities and the cognitive psychology behind the tasks. CPT Christopher Lowe Places 3rd Dermatologic Society Resident Case Competition

Dr. Lowe presented a case at the Washington DC Dermatologic Socie-ty Resident Case Competition Wednesday April 13, 2016 and re-ceived third place for his presenta-tion, "Primary Cutaneous Zygomyco-sis in an extremely preterm infant". Ten cases total were selected to be presented. Cases were presented from dermatology programs from Walter Reed Bethesda, Howard Uni-versity, Georgetown/Washington Hospital Center, and George Wash-ington University.

Internal Medicine Residents and Faculty hold major presence at National ACP Internal Medicine 2016 Meeting in DC! Internal Medicine has unprece-dented 11 residents accepted for presentation at the American College of Physicians “Internal Medi-cine 2016” scientific meeting in Washington, DC, May 5-7, 2016.

LT Pat Moon One of the Top Winning Re-search Abstracts!! Lieutenant Moon's submitted an interim re-port of his research entitled, “Long Term Mortality Associated with Coronary Artery Calcium,” and was selected from a group of 2,834 resident and fellow submissions as one of the top twelve winning research ab-stracts." He presented his work in a special showcase podium presentation session on Thursday, May 5th. LT Ian Grasso (PGY-2) presented a case of shrapnel-related lead poisoning in a veteran, which previously won Best Clinical Vignette Podium at the 2015 Navy ACP Meeting in Bethesda, as a poster at the Internal Medi-cine 2016 ACP meeting. He was part of an unprecedented team of ten Walter Reed Bethesda Internal Medicine poster presenters at this preeminent national meeting, includ-ing LT Dean Drizin (PGY-1), LT Ayee Azah (PGY-1), CPT Zach Junga (PGY-2), LT Brett Sadowski (PGY-3), LT Alison Lane (PGY-2), LT Mickey Skaret (PGY-1), LT Laura Gilbert (PGY-3), LT Pat Moon (PGY-2), and CPT Akira Shishido (PGY-3).

A truly remarkable accomplishment repre-senting the program, Walter Reed, and the National Capital Consortium! Congrats to all of the presenters and their mentors!! Doctor’s Dilemma (Medical Jeopardy) Walter Reed Bethesda Army ACP Chapter-winning team advanced to the Battle Round!

(L-R) CPT Mike McMahon (PGY-3), CPT Ryan Jones (PGY-3), and CPT Joe Maciuba (PGY-1) took first place in their bracket in the 50-team elimination round and advanced to the battle round of 20 teams before being defeated by a strong UPenn team that ultimately placed second in the final. ACP Waxman Skills Sessions Led by MAJ Renee Mallory, Dr. Joan Ritter, and Dr. Julie Chen

Faculty conducted a workshop on the most common complaints involving the foot and ankle at the 2016 National ACP Meeting as part of the Herbert S. Waxman Clinical Skills Center. Participants learned the appropriate use of foot and ankle orthotics and were in-structed on how to recognize indications for imaging studies and subspecialty referrals. The Faculty have been asked to present this well received workshop multiple times.

Internal Medicine Had a Great Showing at the DC ACP Chapter Residents Meeting at Georgetown Medical Center

On Saturday, June 4, IM had six clinical vignette poster presenters; a clinical research podium presentation, a clinical vignette podium presentation, and our joint Army-Navy Jeopardy team took 2nd Place to rivals Washington Hospital Center—we’ll get them next year!! Clinical Research Podium Presentation We had one clinical research podium presentation by Ross Humes, who did an excellent job discussing research on the frequent use of opioids in patients with gastroparesis – “Opiates in Gastroparesis: Help or Hindrance?” Best Posters - Two Winners Hector Medina and Ben Goldenberg – selected from the Walter Reed cohort as having the best poster presentations representing Walter Reed Bethesda. Overall Winner!! - Ben Kurth – Clinical Vignette Podium Presentation We had one clinical vignette podium presentation by Ben Kurth – “Consider What’s Below the Diaphragm when Evaluating Pulmonary Hypertension.” This presentation on pulmonary tumor thrombotic microangiopathy was selected as the overall winner among the DC programs in this category.** Jeopardy Team Wins 2nd Place!! Our Jeopardy team, comprised of CPT Joe Maciuba, LT Ben Kurth, and LT Brett Sadowski, competed against five other DC teams in two rounds of competition. We won the first round convincingly, and fin-ished a hard-fought second to Washington Hospital Center's team. Overall a very strong performance!

New pilot curriculum for medic and corpsman training in the gen-eral internal medicine clinic. The internal medicine chiefs, LT Vipler and CPT Barelski have recently implemented a new pilot curriculum for medic and corpsman training in the general internal medicine clin-ic. The chiefs met with the GIM Clinic NCOIC, HMC De La Cruz, and partnered with him in order to plan for and create the curriculum. The comprehensive curriculum of sick-call and deployment relevant topics was devel-oped and will be taught by internal medicine residents to the medics and corpsman every Monday afternoon. The ten topics were spe-cifically chosen based on feedback of those who have deployed as well as issues that

corpsman and medics face when assisting with sick-call at Walter Reed Bethesda. The new curriculum will impact medic and corps-man performance at Walter Reed Bethesda but more importantly is aimed at training them to enhance readiness for future deploy-ments. The course will also provide medicine housestaff with a better understanding of the roles of medics and corpsmen better prepar-ing medicine housestaff for future roles with line units in the Army and Navy. “Alex did an incredible job today. Most important-ly he ‘started with why’ explaining both how he was former enlisted and knew where the corps-men/medics were, and also pointed out…they will be on the front lines taking care of all of us (beyond sick call).” - big fan

2016 ACP Steps to Success

The 2016 ACP Steps to Success Meeting was held at Harbor Hospital on Saturday, 09April2016. Over 140 students and faculty from USUHS, Georgetown, GW, Johns Hop-kins, University of Maryland, Ross, AUA, and others participated in the event. Congratulations to 2d Lt Gyan Das for win-ning the Clinical Vignette Competition! His poster was entitled "Progressive Multifocal Leukoencephalopathy: Debilitating and Dead-ly." His mentor is LCDR Kristina St. Clair (NMC Portsmouth Infectious Disease Faculty and NCC ID Fellowship graduate) 2LT Bryan Adams, ENS Jessica Saeger, 2LT Dallin Rowley, 2LT Michael Baird, and ENS Lucy Ma also presented posters. First Ever Uniformed Services University (USU) Internal Medi-cine Interest Group (IMIG) Pro-cedure Day at the Walter Reed Simulation Center

Nine internal medicine residents (CPT Mike McMahon, LT Huyn Park, CPT Jed Mangal, CPT Ryan Jones, CPT Diana Dougherty, CPT Sarah Ordway, LT Camille Costan-Toth, CPT Zachary Junga, and CPT Cynthia Philip) volunteered their time and teaching talent for the first ever USU Internal Medicine Interest Group (IMIG) Procedure Day at the Walter Reed Simulation Center on 11 and 13 April. The resi-dents gave 50 preclerkship students instruction and hands-on practice performing thoracentesis, paracen-tesis, central line, arterial line, and lumbar puncture. Student feedback about the event was overwhelm-ingly positive and the IMIG looks forward to making this Procedure Day an annual event. Big Turn Out 4th Annual Internal Medicine Residency Clinical Leadership/Quality Improvement Fair Thank you to everyone who attended and supported our internal medicine residents as they presented their year-long quality im-provement projects at the 4th annual QI poster fair on Tuesday June 7th. Administrative Professionals Day Team DOM GME Admin, I wanted to take a moment on this Adminis-trative Professionals Day to recognize all of the hard work and incredible teamwork that you continually display in meeting the mission of our GME programs - outstanding patient care, education and research while ensuring that our trainees are ready to meet tomor-row's mission and challenges in the MHS. Our GME programs have seen ever-increasing complexity through engagement with more and more Military, Federal and civilian part-ners in medical education (i.e. more and more temporary licenses, training agreements, and MOUs), and our trainees and faculty continue to increase their scholarly accomplishments (i.e. Travel requests) as they have matured since BRAC. I am impressed and humbled how you so often pull together collectively to meet the challenges of increasing travel, GME and Military deadlines, and continual accredi-tation cycles. Outstanding Patient Care CAPT Shimeall, I wanted to make you aware of the out-standing care that [CPT Zachary] Dr. Jun-ga provided to one of our CCU patient's tonight. He admitted a young otherwise healthy man in heart block, presumed to be related to lyme carditis. The patient developed pause dependent TdP sudden-ly this evening while in the ICU, resulting in a ventricular fibrillation arrest. Dr. Junga lead a successful resuscitation effort, promptly defibrillated this patient with return of spontaneous circulation. This was not just a matter of being in the right place at the right time. Dr. Junga promptly responded to a call from the interns, identified the acute issue, and treated it without delay, resulting in no neurologic consequences for this patient. Dr. Junga literally saved this patient's life with his knowledge and action. It's been a pleasure to have him on our CCU service. Unity of Effort with NIH, and Outstanding Teamwork All The Way Around!! Thank you all for making a difference for our patients! Great work by LTC Collen, LT Alison Lane and the WRNMMC Sleep Lab Dr. Nations and Dr. McKay, Just wanted to highlight some of the work that Dr. Jacob Collen did this week. He was the ICU attending, but he still found time to set up a bipap titration study with trans-cutaneous CO2 monitoring for a complicated NIH patient with hyper-IgE syndrome, a CO2 of 78 and chronic pneumatoceles. Somehow he also coordinated pre and post PSG CHEST imaging to prove to the NIH that NIPPV wouldn't make her pneumatoceles worse. He quarter-backed the PSG and it ended up running perfectly. Patient felt significantly better the following morning, and pCO2 was down in the 40s. NIH was thrilled. This could not be pulled off in another sleep lab (or by another doctor). If you get a chance to highlight his work in the weekly newsletter, please do so. SIX FOR SIX LTC Mike Cahill again scored a perfect 300 on his Army Physical Fitness Test (APFT), making this 6 for 6 during his cardiology fellowship where he maxed his APFT (in fact, he has maxed every single one of them during his career)!! As most of us know, the APFT is aimed to assess a Soldier's level of aerobic fitness and strength. To max it is difficult and to max it every year during one's career is rare.

[Both are faculty in Pulmonary and Critical Care Medicine.] CDR Saira N. Aslam is our next Director of Healthcare Operations, replacing COL Tom Burklow. CDR Aslam [Internal Medicine and Cardiology faculty] has been serving as the Executive Assistant to the Director for the last two years. In addition, congratulations to CDR Satyen M. Gada [Allergy faculty], who was elected by the medical staff to serve as Vice President of the Medical Staff and Vice Chair of ECOMS. He will replace CDR Randy Bell, who, in turn, will succeed CDR Jason Schroder as President of the Medical Staff and Chair of ECOMS. Department of Medicine Colleagues, Please join me in welcoming Dr. Sean McKay as our newly selected Pulmonary Medicine Service Chief… He is published in pulmonary medicine and leads as an ardent educator for military relevant medicine topics, including arranging GME education as well as CME during Continuing Promise 2015 and most recently as Navy ACP Speaker on Academics Afloat. We are privileged to have Dr. McKay leading in Pulmonary Medicine, sincerely thank him for his leadership as interim Pulmonary Service Chief and welcome him as the newly selected Pulmonary Medicine Service Chief. I would like to thank Dr. Aaron Holley who Dr. McKay is relieving. Dr. Holley has been selected to PCS next month. While this is a huge loss to our team, it has been our privilege to have him lead in Sleep Medicine as well as the Pulmonary Service Chief during 2015 and early 2016. Selection of Endocrinology Service Chief, Dr. Mai, and Farewell to Dr. Clyde Department of Medicine Colleagues, Please join me in welcoming Dr. Vinh Mai as our newly selected Endocrinology Service Chief. CDR Mai is a 2002 graduate of the Kansas City University of Medicine and Biosciences where he graduated Magna Cum Laude. He trained in Internal Medicine at the Naval Medical Center Portsmouth and in Endocrinology at the National Naval Medical Cen-ter. Dr. Mai is widely published with 16 peer re-viewed articles in print and is frequently requested to present at national endocrine society meetings. He is also a scientific reviewer of multiple journals. In 2016, he was advanced to the rank of Associate Professor at the Uniformed Services University… As Dr. Mai takes the helm in Endocrinology, I would like to thank CAPT Patrick Clyde for his tireless leadership of the Endocrinology Service for the last 10 years. He led in Endocrinology prior to and through the BRAC process and continued to lead as the newly integrated Walter Reed National Military Medical Center Endocrinology Service Chief.
