Funding for Texas Children’s Hospitals Must be PreservedBy Bryan Sperry

It is no secret that Texas is facing a serious budget shortfall that will have to be addressed in the2011 legislative session. Difficult decisions must be made, and clearly, spending cuts are inevitable.

Our state is expected to experience a 32% increase in the number of children (under age 18) from2010 to 2030. The children’s hospitals of Texas—already dealing with financial challenges—willbe asking the legislature to preserve funding for children’s healthcare in the state.Even at current funding levels, Texas children are at a disadvantage on many fronts. For the lastten years, Texas has had the highest rate of uninsured children in the nation. Twenty percent ofTexas children are uninsured compared to the national average of 11 percent. Only 46% of Texaschildren have health insurance through their parent’s employer. More than 2.8 million children inTexas are enrolled in Medicaid or the Children’s Health Insurance Program (CHIP), however500,000 of Texas children who are eligible for Medicaid/CHIP are not enrolled. The recession hasmade a bad situation even worse, and more families need help for their children. To the children’s hospitals of Texas, these are not just statistics. These numbers represent thechildren and families that we see in our emergency rooms and hospitals each and every day. Ourhospitals are the pediatric safety net for all Texas children and have unique services and expertise.In fact, 15 to 20 percent of patients in children’s hospitals are transfers from other medical facilitiesthat could not provide the necessary pediatric specialty care.Children’s hospitals provide care to our state’s most ill children—specialized care that they cannotreceive elsewhere for congenital heart conditions, transplants, cancer and other serious illnesses. Given the relatively low rate of privately insured children in Texas, children’s hospitals rely heavilyon Medicaid to pay for patient services. For the seven member hospitals of the Children’s HospitalAssociation of Texas, over one-half of admissions are Medicaid patients. And, even at currentfunding levels, the actual cost of care exceeds the amount that Medicaid pays. Many hospitals mustabsorb the cost for poor children who are not insured or are underinsured. In addition, the ratio of pediatricians to children in Texas is significantly lower than the nationalaverage. In order to ensure that children can see a specialist when needed, investments should bemade in pediatric graduate medical education to allow medical school students to stay in Texas tocomplete their residency programs.
