New certificates of live birth; duty of clerks of court and department
The clerk of the court in which any proceeding for adoption, annulment of an adoption, affirmation of parental status, or determination of paternity is to be registered, shall within 30 days after the final disposition, forward to the department a certified copy of the court order, or a report of the proceedings upon a form to be furnished by the department, together with sufficient information to identify the original birth certificate and to enable the preparation of a new birth certificate.

Q: How will all applications and fees to BVS be handled?
A: The application will need to be completed by the eligible applicant with the applicable fees and ID, then mailed to the state office.
Q: How are amended documents to be submitted?
A: We would prefer that you use the original Doc Type Code with the e-Portal to send the amended documents. An “amended” notation on the document will help distinguish them from original filings.
Q: How can we send original filings of marriage records?
A: Via electronic data file transfer. Contact BVS for spec sheet & sample scripts.
