Counselor shall follow up to determine the extent to which the individual’s goals have been met.

WHEN DOES OPTIONS COUNSELING END?

When the individual: no longer seeks support no longer has unmet goals after six months has not responded when contacted has exhausted appeals and termination is necessary

OR is dissatisfied and the Options Counselor has no further

alternatives available

RE-ENGAGEMENT IN OPTIONS COUNSELING

An individual shall be re-engaged in Options Counseling at any point he or she indicates a desire to pursue additional support options.

Individual support records shall bemaintained for each individualreceiving Options Counseling.

UNIT OF SERVICEA unit of service is defined as the number of contacts, and the time spent in hours or portions thereof, with and on behalf of each individual. The hours shall be recorded in 15-minute increments.
