The Target: BP Recognition Program celebrates physician practices and health systems for achieving blood pressure control rates at or above 70 percent within their adult patient populations. In fall 2018, the American Heart Association and the American Medical Association will recognize all healthcare organizations that participate in the Target: BP Recognition Program. To participate, a healthcare organization must be registered with Target: BP and submit their data for the 2017 calendar year by June 1, 2018.

TIP:For the best user experience, use Chrome, Safari or Firefox as your browser. If you have to use Internet Explorer, make sure to use version 10 or higher.

New participants

STEP 1: Go to targetbp.org and select “Register” under Join Target: BP.

STEP 2: At the login screen click Register.

STEP 3: Complete the form with details for the Healthcare Organization, Primary User and Secondary Contact.

NOTE:

Ensure the name of your healthcare organization matches how you would like it to appear for recognition and potential public promotion.

The Primary User is responsible for entering or making changes to data on behalf of your organization. The Secondary Contact serves as an alternate contact if the Primary User cannot be reached.

NOTE:

Only the Primary User can submit or change data. Secondary contacts are not registered as users and cannot see data unless they are later added as “View-only Users.” After the Secondary contact is added as a “View-only User”, they can opt-in/out of program emails.

STEP 4: Enter your total adult patient population (age 18-85).

STEP 5: Indicate if you want to participate in Organization Recognition. Ensure that the name of your healthcare organization matches how you would like it to appear for recognition and potential public promotion. If you submit data before June 1, 2018, but do not participate in Recognition, you will not be included in the Recognition materials.

STEP 6: Choose to opt in/out of receiving Target: BP program emails.

STEP 7: Agree to the end user license, data use and privacy policy agreements.

STEP 8: Use your mouse to sign in the Signature box.

STEP 9: Click Register.

STEP 10: Once you’ve completed the contact form and agreed to the license agreements, you’ll see a screen message that your account has been created. Check your email for an activation link that must be used within 24 hours to complete your registration. If you do not activate the account within 24 hours, you will need to go to “Sign-in/Register” to reset your password.

STEP 11: Create a password that meets the security requirements and select “Change Password” to confirm.

You have successfully completed the registration process and will be re-directed to the Home page at recognition.targetbp.org where

you can submit data for your healthcare organization.

Registering & submitting data for multiple healthcare organizations

If you are part of a healthcare system, you may submit data for multiple healthcare organizations within your system using the steps below. Each healthcare organization must submit data in order to be recognized by the Target: BP Recognition Program.

STEP 1: Go to the “Home” tab in the top navigation bar.

STEP 2: Select the green “Add New Organization” button below the top navigation bar.

STEP 3: Complete the requested information for that healthcare organization.

STEP 4: Click “Create.”

STEP 5: You will receive a confirmation message indicating that the healthcare organization was added and it will now be listed on the “Home” page.

Repeat the steps to register each healthcare organization.

We highly recommend collecting data prior to entering it in the system. Use the data collection worksheet to assist you. Your organization should report on data from Jan. 1 to Dec. 31, 2017. The deadline to submit data is June 1, 2018. Data may only be submitted ONE time and corrections are not allowed post-submission.

To submit data for recognition, complete the two-part submission process outlined below:

Part 1: Complete patient population questions

STEP 1: Go to recognition.targetbp.org and log in. Your login credentials are the Primary User email address and password selected upon registration.

STEP 2: Once logged in, your Home screen will list the organization(s) for which you are eligible to submit data. Select the red “Enter/Edit Data” button in the far right column to choose the desired healthcare organization.

STEP 3: Enter your healthcare organization’s data from Questions 1-4 below in the four data fields on the left side of your screen.

NOTE:

Hypertension data are based on quality measure “NQF #0018: Controlling High Blood Pressure,” defined as the percentage of patients age 18 to 85 who had a diagnosis of hypertension and whose blood pressure was adequately controlled to <140/90 mmHg. Enter data only from Jan. 1 to Dec. 31, 2017.

This Measure (NQF #0018) is endorsed by the National Quality Forum (NQF). In CMS programs, it is designated as “PQRS #236.” It is also used for quality benchmarking and reported as ACO #28 for accountable care organizations (ACOs) participating in the Medicare Shared Savings Program. The measures have yet to incorporate the November 2017 BP Guideline update thresholds and continue to define high blood pressure as >140/90. The AHA and AMA continue to track these quality measures to ensure Target: BP Recognition data align with them.

Use the following guidance to answer questions 1-4.

Q1: What is the total adult (18-85 years) patient population size for the healthcare organization? Enter the total number of unique patients in your healthcare organization, age 18-85, who had an office visit in 2017.

Q2: What is your total adult (18-85 years) patient population that has been diagnosed with hypertension? Enter the number of patients age 18-85 in your healthcare organization who have an existing diagnosis of hypertension prior to June 30, 2017. Exclude any patients with a diagnosis of end-stage renal disease, currently on renal dialysis, history of renal transplant, currently pregnant or those whose hypertension diagnosis occurred between July 1–Dec. 31, 2017. Hypertension is diagnosed when a patient has multiple office visits with blood pressures ≥ 140/90 mmHg, or when antihypertensive medication is prescribed to treat elevated blood pressure or target organ damage.

Q3: Of those who have been diagnosed with hypertension (from Q2 above), what is the number of adult (18-85 years) patients under control, < 140/90? Enter the number of patients age 18-85 in your healthcare organization who have a diagnosis of hypertension and had a blood pressure at their last 2017 office visit that was <140/90 mmHg.

Q4: How many providers are there for the health system? Enter the total number of providers who manage hypertension in your healthcare organization. Providers are physicians, nurse practitioners, and physician assistants.

Part 2: Complete the Prevalence Estimator

To the right of the four questions, you’ll see four tabs with age groups. This section is powered by the Million Hearts® Hypertension Prevalence Estimator and is required for Recognition. This calculates the expected percentage of your patients with hypertension, based on your total adult patient population, and may provide an important tool for identifying undiagnosed patients in your organization. A value below the low-end value of the range suggests there may be undiagnosed hypertension in your practice due to missing data on the hypertension status of the patient population.

Use the following steps to complete the prevalence estimator:

STEP 1: Enter the total adult patient population data, from Question 1 in the data submission section, broken out by age, gender and race/ethnicity.

• Age groups are 18–44, 45–64, 65–74, and 75–85 years old.

• Designated race/ethnicity groups are: Non-Hispanic White; Non-Hispanic Black; Hispanic; American Indian or Alaska Native, Asian, Native Hawaiian or Other Pacitific Islander, and all other race/ethnicities; and Unknown.

NOTE:Patients who cannot be identified as male or female should be removed from ALL data reporting. In addition, those patients who cannot be categorized by the ethnicity groups, should be placed in “Unknown”.

STEP 2: Use the tabs across the top of the Hypertension Prevalence Estimator table corresponding to the four age categories to record the number of patients for each of the Race/Ethnicity and Gender sub-categories.

• If Race/Ethnicity is unknown for any patients, record in the “Unknown” sub-category for each age tab and gender column.

Part 3: Saving and submitting data

You may save your data at any time, but you may only submit data once. Saving Data

STEP 1: Select “Save as Draft.” The system will automatically alert you if any data errors are detected.

STEP 2: Once your data have been saved, log in any time to correct data errors or complete your submission.

Submitting Data

STEP 1: Select the button next to “I am ready to submit data for Target: BP Recognition.”

STEP 2: Acknowledge your agreement for “Data Attestation,” then select “Submit for Target: BP Recognition.”

• You must complete the data submission process by June 1, 2018, in order to be recognized by the Target: BP Recognition Program in fall 2018.

• You will see a notification confirming that your data have been successfully submitted. You will also receive an email confirmation of your submission.

NOTE:Data can only be submitted one time, so please check for accuracy prior to submitting. There are no edits post submission!

Congratulations! You have successfully submitted data for the Target: BP Recognition Program.

View benchmark data

After the submission deadline, benchmark data at the state, regional and national levels will be available to compare your organization’s results to other participants in the Target: BP Recognition Program. Log in to your Target: BP account and go to the “Benchmarks” tab in the top navigation bar.

User roles

Each healthcare organization designates a Primary User who can edit and submit data. This user is defaulted to the person who registers but can be changed at any time. The Secondary User must be added as a “View-Only” user to have access to the portal and Target: BP communications. The Primary User is also able to add an unlimited number of “View-Only” users who are able to access the healthcare organization’s Target: BP results but cannot edit or save data.

Select “My Profile” in the top navigation bar.

• Change your password.

• Review and download terms of end user license and data use agreements.

• Opt in/out of receiving Target: BP program emails. Program emails include monthly newsletters, notices about upcoming deadlines, webinars and other educational opportunities.

Add and deactivate users

The Primary User can add, edit, and deactivate users for their healthcare organization as well as reset passwords for View-Only users by using the following steps:

STEP 1: Go to “Users” in the top navigation bar.

STEP 2: Select the green “Add New User” button and complete the form to add a user to your healthcare organization’s account.

STEP 3: Newly added View-Only Users will receive an email containing a link confirming their accounts, which must be activated within 24 hours.

STEP 4: To edit a user, go to the Action(s) column and click on the Edit button. To deactivate a user go to the Action(s) column and click on the Exclude button.

If the Primary User has left the organization and did not assign a new Primary User before departing, complete the contact us form on targetbp.org to request reassignment.

Healthcare organization account maintenance

Primary Users can also use the “Organizations” tab in the top navigation bar to edit the following information for your healthcare organization:

• Healthcare organization name.

NOTE:Ensure the name of your healthcare organization matches how you would like it to appear for recognition and potential public promotion.

• Secondary Contact information including first and last name, email address and mailing address.

• Organization Recognition opt in/out - opting in is required for inclusion in recognition promotions.
