Observations:

The actual alignment of curve to the length of 30 mtrs as per the readings of versine is shown in red in fig-2. It was taken at an interval of 3 mtrs.

From the above it can be seen that the curve is not uniform and there is a excessive variation in versine between en the vehicle was moving from stn 3 to stn 6 & 7, the flange was forced to move suddenly by 17 mm towards LH rail which in turn would have offered excessive flange force. This is the place where the vehicle was mounted over the rail table.
