Medical Care: The mainstay of management of vascular dementia is the prevention of new strokes. Recent guidelines from the American Psychiatric Association provide both treatment principles and possible specific therapies Drug treatment primarily is used to prevent further worsening of vascular dementia by treating the underlying disease such as hypertension and diabetes. The general management of dementia includes appropriate referral to community services, addressing legal and ethical issues (eg, driving, competency, and advance directives), and consideration of caregiver stress. Some Summary Thoughts: Alzheimer's disease alone accounts for about 50 per cent. It usually affects people between the ages of 60 and 75 and is slightly more common in men than women. Vascular dementia is a term for dementia associated with problems in the circulation of blood to the brain (cerebrovascular disease). If blood vessels in the brain burst (cerebral haemorrhage), if arteries blocked by plaque formation or clots (thrombosis or embolism), or if there is insufficient blood flow to parts of the brain (ischaemia) brain tissue will die. It is thought that about a fifth of people who have strokes will develop problems involving their mental abilities including dementia. The risk factors for vascular dementia are those associated with all forms of stroke, the most significant of which is high blood pressure (hypertension). Following is an overview of types of vascular dementia, causes, diagnosis and progression. Blockages may be caused by build up of plaque on the inside of the arterial wall or by clots which have broken off, jamming a smaller tributary upstream. Clots can also result from abnormal heart rhythms, or other heart pathology, or can form on the inside of the major cartoid arteries that run up the side of the neck and supply the brain. The symptoms generally do not last for more than a few days, that is why they are called transient. The doctor will look for signs of a stroke by checking for weakness or numbness in the arms or legs and difficulty with speech.

Abstract: Vascular Dementia A diagnosis of dementia does not always mean an unavoidable decline. One of the most feared consequences of aging is dementia, a set of symptoms marked by profound memory loss and impaired thinking. Thanks to a lot of research and public education, most people are aware that dementia is not an inevitable part of growing older. In fact, it is most often the result of a specific illness, Alzheimer's disease, that strikes many-but by no means all-people in their senior years. The bottom line is that aging does not necessarily lead to "senility," unless Alzheimer's or some other disease is present. One of those "other" diseases is vascular dementia..... Caused by blockages and breaches in the brain's blood supply that damage the brain, vascular dementia can be prevented and, in some cases, even reversed.

Abstract: Introduction CADASIL is the English acronym for Cerebral Autosomal Dominant Arteriopathy with Subcortical Infarcts and Leukoencephalopathy. Recognized more than a decade ago as a new cerebral vasculopathy (1), and maybe reported 40 years ago by Van Bogaert as an aggressive form of Binswanger’s disease (2), CADASIL has been continously diagnosed in the last years, with more than 50 affected families reported (3). Pathogenesis CADASIL still has an unknown pathogenesis but alterations in the q12 region of chromosome 19 have been detected in some cases (4). Histopathologically, a characteristic eosinophilic granular deposit of an unknown composition has been present in the media of the small penetrating cerebral arteries. This seems to cause vascular obliteration and, consequently, multiple infarcts, mainly in the white matter, basal ganglia and leptomeninges (5). Extracerebral vascular abnormalities have also been noted (6).

vessels (vasculature), and when these changes occur in the brain (cerebrum) they are referred to as cerebrovascular disease. The most common vascular change associated with age is the accumulation of cholesterol and other substances in the blood vessel walls. This results in the thickening and hardening of the walls as well as narrowing of the lumen of the vessels (the space where blood flows), which can result in a reduction or even a complete cessation of blood flow to brain regions supplied by the affected artery. When this occurs suddenly the result is a stroke, with symptoms ranging from weakness, to incoordination, to abnormal sensations, depending on the location of the injury in the brain. In some cases, a sudden loss of cognitive function (such as language, memory, complex visual processing, or organizational skills) can occur. If they result in a permanent cognitive impairment, this is easily recognized by the patient, their family and their physician, because of the obvious relationship of the change to a particular event. Such cases are not usually diagnosed as dementia, but rather the difficulties are appropriately described as residual cognitive impairment from the stroke. what we mean by the term 'vascular dementia' The term Vascular Dementia (VaD) is usually reserved for an insidiously (subtly) progressive worsening of memory and other cognitive functions. In this way and others, VaD patients present with similar symptoms to Alzheimer's disease (AD) patients. VaD may progress in a "stepwise" fashion such that initial cognitive deficits (e.g., memory difficulty) plateau for a period of time followed by the sudden onset of more cognitive deficits.
