Reactions 1451 - 11 May 2013 S Varenicline Neuropsychiatric events: 4 case reports Three men and four women with psychiatric comorbidities developed neuropsychiatric adverse events during treatment with varenicline for smoking cessation [routes and dosages not stated]. A 58-year-old man with PTSD developed worsening mood and suicidal ideation within 30 days of starting varenicline therapy. Varenicline was stopped and his suicidal thoughts resolved. A 39-year-old man with depression and PTSD began experiencing worsening mood with a passive death wish about 35 days after varenicline initiation. He began receiving citalopram, which was tapered off after varenicline was later withdrawn because his mood improved. A 42-year-old woman with bipolar disorder developed emotional lability, depression and hallucinations from about 7 days after starting varenicline. Twenty days after withdrawal of the drug, most of her symptoms had resolved. A 49-year-old man with depression was hospitalised for depression and suicidal ideation after a rechallenge of self- initiated varenicline. The man had initially taken it for 2 weeks before self-discontinuing due to low mood. After 5 weeks he restarted the drug, taking it for 3 weeks; he stopped a week before the current admission. He began receiving fluoxetine and nicotine replacement, and received one session of electroconvulsive therapy. He was discharged after a 3-day admission, and required no further treatment for depression since discharge, despite becoming non-compliant with fluoxetine.
