IV. Health Information
I, the undersigned official veterinarian, certify that the hatching eggs
described above meet the following requirements:
a) The United Kingdom, where the flocks of origin are located is
officially free from notifiable avian influenza (NAI) according to
the criteria of the OIE;

b) The flocks of origin are located in a poultry establishment in the UK
which is officially free from Newcastle disease according to the
criteria of the OIE;
c) vaccination of poultry for avian influenza is not permitted in the UK
and the flock(s) of origin have not been vaccinated against avian
influenza;
d) The flocks of origin have been resident in the UK for at least 90 days
or since hatching;
e) the farm(s) of origin are subject to regular official veterinary
inspection there has been no evidence of clinical symptoms of
infectious disease at the farm(s) of origin during the 90 days
immediately prior to shipment;
f) the flock(s) of origin are subject to routine veterinary supervision
and there has been no evidence of the following diseases during the
6 months immediately prior to export:
Newcastle disease, fowl pox, fowl cholera (Pasteurella multocida), fowl
typhoid (Salmonella gallinarum), infectious bronchitis (IB), infectious
laryngotracheitis (ILT), infectious synovitis, infectious bursal disease
(IBD or Gumboro disease), egg drop syndrome (EDS ’76), pullorum disease
(Salmonella pullorum), typhroid disease, ornithosis, Marek’s disease,
lymphoid leucosis, mycoplasmosis, chronic respiratory disease (CRD),
swollen head syndrome (avian pneumovirus), chick anaemia agent, avian
nephritis, leucocytozoonosis, infectious coryza, avian spirochaetosis,
avian encephalomyelitis, avian salmonellosis, avian tuberculosis;
g) The flocks of origin of the hatching eggs have undergone a disease
surveillance programme in accordance with EU Directive 2009/158/EC
with negative results to serological or bacteriological tests for:
Salmonella pullorum (Pullorum disease) and S.gallinarum (Fowl
thypoid); (a copy of the latest laboratory report is attached);
h) The flocks of origin of the hatching eggs have undergone a disease
surveillance programme in accordance with EU Regulation 2160/2003
with negative results for: Salmonella enteritidis and S.typhimurium
(a copy of the latest laboratory report is attached);
j) the hatching eggs have been sanitised before packing for export and
the boxes in which the eggs are packed for export are new, unused and
clean, and contain only the eggs certified in this certificate;
k) The vehicles for the transport of the hatching eggs have been cleaned
and disinfected prior to use with approved disinfectants.
V. This certificate is valid for 10 days.
