SUMMARY
Laryngectomized patients suffer from respiratory complaints. Breathing through
a tracheostoma results in insufficient warming and humidification of inspired air.
Improvement of pulmonary humidification with significant reduction of pulmonary
complaints is achieved by the application of a Heat and Moisture Exchanger (HME)
over the tracheostoma. HMEs are passive humidifiers that partially re-condition the
inspired air.
There are different approaches to assess the humidification effect of an HME
device. In vitro (International Organization for Standardization [ISO] standard 9360-
2:2001) and in vivo measurements of the humidity effects of an HME are complex
and technically challenging. The aim of chapter 2 was to develop a simple method
to measure the ex vivo HME performance comparable and validated with previous
in vitro and in vivo results. HMEs were weighed at the end of inspiration and at
the end of expiration at different breathing volumes. Four HMEs (Atos Medical,
Hörby, Sweden) with known in vivo humidity and in vitro water loss values were
tested. The associations between weight change, volume, and absolute humidity
were determined using both linear and non-linear mixed effects models. The
rating between the 4 HMEs by weighing correlated with previous intra-tracheal
measurements (R2 = 0.98), and the ISO standard (R2 = 0.77). Assessment of the
weight change between end of inhalation and end of exhalation thus appeared to
be a valid and simple method of measuring the water exchange performance of an
HME.
The aim of chapter 3 was to determine whether the new Provox HMEs (XM-HME
and XF-HME) have a better water exchange performance than their predecessors
(R-HME and L-HME respectively; Atos Medical, Hörby, Sweden). The other aim was
to assess the short-term clinical feasibility of these HMEs. The weighing method
described in chapter 2 was used to assess and further validate the water exchange
and humidity values. Study-specific questionnaires and tally-sheets were used in
the clinical feasibility study. The water exchange of the XM-HME is 3.6 mg, which
is significantly higher than that of the R-HME (2.0 mg). The water exchange of the
XF-HME (2.0 mg) was not significantly higher than that of the L-HME (1.8 mg). The
absolute humidity values of both XM- and XF-HME were significantly higher than

that of their predecessors. The clinical feasibility study did not reveal any practical
problems over the course of three weeks. The study was a further validation of
the ex vivo method and showed that the XM-HME has a significantly better water
exchange performance than its predecessor (R-HME), and that both newly designed
HMEs did succeed in the clinical feasibility study.
For medical professionals it is not easy to judge differences in humidification
performance of various HMEs due to the lack of universal outcome measures.
Therefore chapter 4 assessed the water exchange performance of 23 commercially
available HMEs for laryngectomized patients and validated these results with
absolute humidity outcomes. Also the role of hygroscopic salt in the core material
of hygroscopic HMEs was assessed. Measurements of water exchange and
absolute humidity were performed with the validated ex vivo method described
in chapter 2 and 3. Twenty-three HMEs of six different manufacturers were
tested. Water exchange of these tested HMEs at a breathing volume of 0.5 liter
varies between 0.5-3.6 mg. Both water exchange and wet core weight correlate
strongly with the end-inspiratory absolute humidity values (R2=0.89 / 0.87). The 23
tested HMEs for laryngectomized patients show wide variation in water exchange
performance. Water exchange correlates well with the end-inspiratory absolute
humidity outcome, which validates the ex vivo weight change method. Wet core
weight is a predictor of HME performance. Hygroscopic salt increases the weight of
the core material. The results of this study can help medical professionals to obtain
a more founded opinion about the performance of available HMEs for pulmonary
rehabilitation in laryngectomized patients, and uniquely allow them to make an
informed decision on which of HME type to use.
Standard of care is to advise patients to use HMEs for a maximum period of
24 hours, because after that period bacterial contamination might become a
source of infection, but also because of a potential decrease in water exchange
performance. Daily replacement does not cause additional exposure to pathogenic
microorganisms and shows that using an HME for 24-hours is bacteriologically
safe. Chapter 5 determined whether hygroscopic heat and moisture exchangers
still function after 24-hour tracheostoma application. Three hygroscopic heat and
moisture exchanger types were tested after use by laryngectomized patients in
long-term follow-up. Water uptake of 41 used devices (including 10 prematurely

replaced devices) was compared to that of control (unused) devices of the same
type and to a control device with a relatively low performance. After 24 hours, the
mean water uptake of the three device types has decreased compared to that of
their control devices. For only one type this difference was significant. None of the
used heat and moisture exchangers had shown a lower water uptake than that of
the low performing control device. The water uptake capacity of hygroscopic heat
and moisture exchangers is clinically acceptable though no longer optimal after 24-
hour tracheostoma application. From a functional point of view the guideline for
daily device replacement is therefore justified.
Impaired mucociliary clearance and histological changes of respiratory epithelium,
such as loss of ciliated cells, previously have been described in laryngectomized
patients. Aim of chapter 6 was to assess both the effect of HMEs on tracheal
epithelium and on tracheal mucus transport velocity (TMV). Tracheal brush biopsies
were collected in three groups of TLE patients: 21 long-term HME users, 10 non-
HME users, and 16 non-HME users before and after 4-9 months HME use. Tracheal
epithelium biopsies were assessed using a digital high-speed camera mounted on
a light microscope. TMV was determined by scintigraphy in the first two patient
groups.
Significantly more ciliated cells were found in HME users compared to non-HME
users (p=0.05). TMV was higher in HME users (median 2 mm/min; 0-7.9) compared
to non-HME users (median 0.8 mm/min; 0-12.3), but this difference was not
significant (p=0.37). One hour breathing without HME in long-term HME users did
not measurably decrease TMV (p=0.13). This study shows that long-term use of
an HME restores and/or prevents the loss of tracheal ciliated cells. A significant
improvement in TMV was not found. Short-term (one hour) detachment of an HME
had no measurable effect on TMV.
Laryngectomized patients have reported considerable pulmonary complaints. It is
assumed that laryngectomized patients also run a higher risk of developing severe
respiratory infections. Unfortunately, there is little scientific information available
about the occurrence of respiratory infections and related health costs in these
patients with and without the use of an HME. Therefore, the occurrence of respiratory
infections in laryngectomized patients was investigated in the Netherlands Cancer

Institute and by means of a survey among head and neck oncology surgeons
throughout Europe (chapter 7). The number of tracheobronchitis and/or pneumonia
events were retrospectively scored between 1973-2013 in medical records of 89
laryngectomized patients treated in the Netherlands Cancer Institute and in follow-
up for at least 5 years. To assess expert experiences and opinions regarding these
pulmonary problems, a study specific survey was developed. The survey was sent
by email to head and neck surgeons from 10 different countries. In the medical
record study, an average of 0.129 respiratory infections per patient/year was
found in non-HME users and 0.092 in HME users. In the survey (response rate HN
surgeons 20%; countries 90%) 0.285 episodes per patient/year in non-HME users
was statistically higher than the 0.066 episodes per patient/year in HME users.
The average mortality in the HME user group per entire carreer of each physisican
was estimated on 0.0045 and for the non-HME user group this was 0.0152. Both
the retrospective study and the survey indicate that there is a tendency that the
number of tracheobronchitis and pneumonia episodes in non-HME users is higher
than in HME users.

Gelaryngectomeerde patiënten hebben veel luchtwegproblemen omdat het
ademen door een tracheostoma resulteert in onvoldoende verwarming en
bevochtiging van de ingeademde lucht. Verbetering van de ademluchtbevochtiging
met vermindering van de longproblemen kan worden bereikt door een warmte-
en vochtwisselaar (heat and moisture exchanger, HME) over het tracheostoma
te dragen. HME’s zijn passieve luchtbevochtigers die het vochtgehalte van de
ingeademde lucht gedeeltelijk herstellen.
Er zijn verschillende methoden om het bevochtigingeffect van een HME filter te
meten. In vitro (International Organization for Standardization [ISO] standaard
9360-2:2001) en in vivo metingen van vochteffecten van een HME zijn technisch
ingewikkeld. Het doel van hoofdstuk 2 was om een nieuwe, simpele methode te
ontwikkelen om de ex vivo werking van een HME te meten die vergelijkbaar is en
gevalideerd wordt met eerder gemeten in vitro en in vivo resultaten. HME’s werden
gewogen aan het einde van de in- en uitademing bij variërende ademvolumes van
een gezond proefpersoon. Vier HME filters (Atos Medical, Hörby, Zweden), waarvan
de in vivo vochteffect- en in vitro waterverlieswaarden bekend zijn, werden getest.
Associaties tussen gewichtsverschil (waterwisselcapaciteit), ademvolume en
absolute luchtvochtigheid werd bepaald met zowel lineair als niet lineair gemengde
effecten modellen. De score van waterwisselcapaciteit van de vier HME’s correleren
met de eerder gemeten intra-tracheale uitkomsten (R2 = 0.98) en de ISO standaard
waarden (R2 = 0.77). Het gewichtsverschil tussen het einde van de inademing
en einde van de uitademing blijkt dus een simpele en valide methode om de
waterwisselcapaciteit van een HME te bepalen.
In hoofdstuk 3 werd de werking van twee nieuwe Provox Heat en Moisture
Exchangers (XM-HME en XF-HME) bepaald en vergeleken met die van hun
voorgangers (R-HME en L-HME; Atos Medical, Zweden). Het tweede doel was een
haalbaarheidsonderzoek van deze HME’s in de dagelijkse praktijk. De weegmethode
uit hoofdstuk 2 werd gebruikt om de ex vivo waterwisselcapaciteit en absolute
luchtvochtigheid te bepalen en deze weegmethode werd verder gevalideerd. Studie
specifieke vragenlijsten en score formulieren werden gebruikt in het drie weken
durende haalbaarheidsonderzoek. Het gewichtsverschil tussen eind-inademing en

eind-uitademing van de XM-HME was 3.6 mg bij een teugvolume van 0.5 liter, wat
significant hoger is dan dat van de R-HME (2.0 mg). Het gewichtsverschil van de XF-
HME (2.0 mg) was niet significant hoger dan dat van de L-HME (1.8 mg). De absolute
luchtvochtigheid was voor zowel de XM-HME en de XF-HME significant hoger
vergeleken met hun voorgangers. Het haalbaarheidsonderzoek liet geen praktische
problemen zien. De studie betekende een verdere validering van de ex vivo methode
en toonde aan dat de XM-HME een significant betere waterwisselcapaciteit heeft
dan de voorganger (R-HME) en dat beide nieuwe HME types in de dagelijkse praktijk
probleemloos functioneren.
Vanwege het gebrek aan een universeel beschikbare uitkomstmaat voor de
luchtbevochtiging werking is het verschil tussen HME’s moeilijk te bepalen voor de
medische professional. In hoofdstuk 4 werd de ex vivo waterwisselcapaciteit gemeten
van 23 commerciële HME’s voor gelaryngectomeerde patiënten. Ook werd de rol van
hygroscopisch zout dat geïmpregneerd is in het HME materiaal van hygroscopische
filters onderzocht. Waterwisselcapaciteit en luchtvochtigheidsmetingen werden
verricht met de gevalideerde ex vivo methode beschreven in hoofdstuk 2 en 3.
In totaal werden er 23 HME types van zes verschillende fabrikanten getest. De
waterwisselcapaciteit van de geteste HME’s varieerde tussen de 0.5-3.6 mg bij een
teugvolume van 0.5 liter. Zowel de waterwisselcapaciteit als het natte gewicht van
het HME materiaal correleert met de absolute luchtvochtigheid waarden (R2=0.89
/ 0.87), waarmee de weegmethode nogmaals gevalideerd is. De conclusie is dat de
23 HME types een grote variatie in waterwisselcapaciteit laten zien. Daarnaast blijkt
het gewicht van nat HME materiaal een voorspeller voor de HME werking te zijn.
Hygroscopisch zout zorgt voor een toename van het gewicht van HME materiaal. De
resultaten van deze studie kunnen de medische professional helpen bij het maken
van een beter beargumenteerde keuze bij het voorschrijven van een HME aan
gelaryngectomeerde patiënten.
Het standaard advies voor de gebruiksduur van een HME is 24 uur omdat hierna
mogelijk bacteriële contaminatie een bron van infectie kan vormen, maar ook
vanwege een potentiële achteruitgang in de werking van een HME. Eerder is bewezen
dat dagelijkse wisseling van de HME geen extra blootstelling aan pathogene micro-
organismen geeft waardoor het gebruik van een HME tot 24 uur veilig is wat betreft
bacteriële contaminatie.

Of HME’s nog steeds functioneren na 24 uur gebruik door gelaryngectomeerden
werd onderzocht in hoofdstuk 5. De wateropname capaciteit van drie
verschillende hygroscopische HME types werd getest na 24-uur gebruik door 10
gelaryngectomeerde patiënten in lange termijn nacontrole. De wateropname
capaciteit van 41 HME’s (inclusief 10 HME’s die vroegtijdig waren gewisseld) werd
vergeleken met controle (ongebruikte) HME’s van hetzelfde type en met een HME
type met een relatief lage wateropname capaciteit. De wateropname van de drie
geteste HME types was na 24-uur patiënten gebruik afgenomen in vergelijking met
de controle HME’s, deze afname was voor 1 type HME significant. Geen van de
drie geteste HME’s had een lagere wateropname dan de HME type met de relatief
lage wateropname capaciteit. De conclusie is dat de wateropnamecapaciteit van
hygroscopische HME’s na 24-uur tracheostoma applicatie klinisch nog acceptabel,
maar niet langer optimaal is. De richtlijn om de HME dagelijks te vervangen is vanuit
een functioneel opzicht gerechtvaardigd.
Verminderde mucociliaire klaring en histologische veranderingen van het respiratoire
epitheel, zoals verlies van ciliacellen, zijn eerder beschreven in gelaryngectomeerde
patiënten. Doel van de studie beschreven in hoofdstuk 6 was zowel de effecten
van een HME op tracheaal epitheel als op tracheaal mucus transportsnelheid te
onderzoeken. Brush biopsies uit de trachea werden verricht in drie verschillende
patiëntengroepen: 21 lange termijn HME-gebruikers, 10 niet-HME gebruikers, en
16 niet-HME gebruikers zowel vóór als na 4-9 maanden HME gebruik. De brush
biopsies van tracheaal epitheel werden geëvalueerd met een lichtmicroscoop en
een digitale high-speed camera. Tracheaal mucus transportsnelheid werd bepaald
door middel van scintigrafie in de eerste 2 patiënten groepen. In de HME gebruikers
werden significant meer ciliacellen gevonden in vergelijking met de niet-HME
gebruikers (p=0.05). De mucus transportsnelheid was hoger in de HME gebruikers
(mediaan 2 mm/min; 0-7.9) dan in de niet-HME gebruikers (mediaan 0.8 mm/
min; 0-12.3), maar dit verschil was statistisch niet significant (p=0.37). De mucus
transportsnelheid was niet meetbaar verminderd na 1 uur ademen zonder HME in
de HME groep (p=0.13). Dit onderzoek toont aan dat lange termijn gebruik van een
HME het verlies van ciliacellen in de trachea herstelt en/of voorkomt. Een significant
verschil in tracheaal mucus transportsnelheid werd niet gevonden. Korte termijn

(1 uur) afnemen van de HME van het tracheostoma gaf geen meetbaar effect op de
mucus transportsnelheid in de trachea.
Gelaryngectomeerde patiënten hebben aanzienlijke pulmonale klachten. Het
wordt verondersteld dat deze patiënten ook een hoger risico hebben om ernstige
luchtweginfecties te ontwikkelen. Helaas is er weinig wetenschappelijke literatuur
beschikbaar over de incidentie van luchtweginfecties en medische kosten in
deze patiënten, zowel zonder als met HME gebruik. Hoofdstuk 7 heeft daarom
het voorkomen van luchtweginfecties in gelaryngectomeerden onderzocht
in het Nederlands Kanker Instituut en door een vragenlijstonderzoek onder
Europese hoofdhalschirurgen. Retrospectief tussen 1973-2013 werden het aantal
tracheobronchitis en/of pneumonie episoden gescoord vanuit de medische dossiers
van 89 gelaryngectomeerde patiënten behandeld in het Nederlands Kanker Instituut
met een nacontrole periode van tenminste 5 jaar. Een studiespecifieke vragenlijst
werd ontwikkeld om de ervaringen en meningen van medische experts te evalueren
en elektronisch verstuurd naar hoofd-halschirurgen uit 10 verschillende Europese
landen. In de medische patiënten dossiers was het gemiddeld aantal respiratoire
infecties per patiënt/jaar 0.129 voor de niet-HME gebruikers en 0.092 voor de HME
gebruikers. Het responspercentage van de vragenlijsten was 20% wat betreft de
hoofd-halschirurgen en 90% wat de landendekking betreft. Uit dit deel van het
onderzoek bleek dat de episoden van respiratoire infecties 0.285 per patiënt/jaar
was voor de niet-HME gebruikers, wat statistisch significant hoger is dan de 0.066
episoden in de HME gebruikers. De gemiddelde mortaliteit in de niet-HME-groep
werd geschat op 0.0152 per carrière/per arts, en voor de HME-groep was dit getal
0.0045. Zowel de retrospectieve studie als de vragenlijsten wijzen er dus op dat
het aantal episoden van tracheobronchitis and pneumonie in niet-HME gebruikers
hoger is dan in HME gebruikers.
