Supplier Environmental Compliance Requirements

Intel’s Expectations:

• Suppliers are required to provide a material declaration form for each part, product or product family supplied to Intel, which includes disclosures for RoHS, REACH, Low Halogen and other requirements as necessary.

• Suppliers are required to submit Class-D: full material disclosure (FMD) according to IPC 1752A standard for products such as integrated circuits and articles used to assemble integrated circuits; suppliers can submit Class-C according to IPC 1752A or IEC 62474 standard for products such as board and system.

• Suppliers are required to submit material declaration collaterals through Intel’s Environmental Compliance Portal; link to the portal is also provided in the EC data request email notifications to suppliers.

Accepted Reporting Formats:

• Intel accepts reporting formats in either an IPC 1752A XML file generated from a 3rd party tool that has been certified by IPC or IEC 62474 XML file that conforms to the standard. Please be advised that Intel no longer accepts the IPC 1752-2 v1.1 PDF and declarations made by the Intel online “Material Content Declaration (MCD) tool.

• Material Disclosure Form (MDF). This is an Intel provided Excel form designed according to the IPC 1752A data exchange standard.

What is MDF?

7

• MDF is an Excel based custom form that is made available to Intel suppliers to collect product compliance information for submission to Intel. The form offers the following features:

• Ability to handle simple to complex product structures

• Ability to configure the file based on the declaration types required by your product (Full Material Declaration or Material Summary only)

• Validation features – prompts the user when any mandatory information as required by Intel is missing

• Ability to generate IPC 1752A xml file from the information

How to use Material Disclosure Form – Intro Tab

8

iNote: This document requires a minimum of Excel 2003 and Windows to operate

This Material Disclosure Form allows companies to declare the use of reportable/banned substances based on directives and regulations such as RoHS and REACH. All mandatory fields are highlighted in red and must be filled in prior to submission.

The tabs available for completion are:

2) Products - Enter the products here. Mass and quantity data must be provided. If needed, sub-products can be declared by clicking the 'Add Child' button

4) Mfr Process Info - This section is optional and allows for the submission of Manufacturing Process Information relating to all of the products declared in the Products tab.

6) FMD - If a 'Class D' has been selected, this section allows you to complete a Full Material Disclosure for the selected parts in the Products tab. All the product and sub products must be accounted for in order for the file to pass validation.

Note: You must click 'Sign and Lock' in the Business Info tab prior to submission. Any validation errors will be shown at the top of this worksheet and must be corrected prior to submission.

Form InstructionsTo complete a valid material disclosure, please review these instructions first

1) Business Info - All fields marked in red must be completed in this section. One of 2 declaration types can be selected for this form:

Class C - For each product and/or sub-product, you must provide a positive or negative disclosure under the Material Summary tab stating whether your products contain a hazardous or reportable substance. Material

Summary must be added to the product or product part and the substance threshold must be set to Yes/No. If above threshold is Yes, provide the information in the other columns. If Above threshold is No, other columns

are optional. A substance group may be called out multiple times, so take note of the reporting level, threshold and Reportable Application to make sure you are reporting the right substance category. When more than one

exemption is claimed for the same substance, insert a new row, then copy and paste the substance row into the inserted row, fill in the substance and material columns and select the exemption.

Class D - This represents a Full Material Disclosure (FMD) requesting for a complete breakdown of homogenous materials and substances for each product/product part listed in the Products section. Class D is Intel's most

preferred Form Type. Class D is required for products such as integrated circuits and articles used to assemble integrated circuits.

5) Material Summary - If a 'Class C' has been selected in the Business Info tab, this section allows you to complete a positive or negative declaration. If no rows are visible, please make sure you click 'Add Material Summary' in the

Products tab. Note: Only substances or chemicals that remain in the product must be reported.

3) Declaration - All query list statements must be answered in this tab. If any exemptions are used for the products being declared, these must also be provided in the first table. If exemption is claimed, the exemption must also be

claimed at the Material Summary of FMD RoHS substance row in order for it to be properly applied to the Eu RoHS Compliance verdict.

Field Name Intel Requirement

Form Type Select Form Type for the declaration.

Class D – Full Material Disclosure (FMD) is required for products that are used for IC manufacturing (i.e., resistor, capacitor, solder, etc.)

Class C – Material Summary is used when suppliers only

want to submit declarable substances within their products.

(i.e., cables, heat sink, battery, power supply, etc.) Not

applicable to product used for IC manufacturing.

Color Codes Any field highlight in red are mandatory information either to display the information required (Form Type), or information is required to pass validation.

Form Validation The field is enclosed in red line by default – indicates the validation has not been run. If Validate Form Before Submitting is clicked and validation errors are found, this filed list the items that failed validation. User is then able to go back to each failed item, make corrections and then rerun the validation. Any file that failed validation will fail file upload/submission to Intel.

Supplier Information This section collects the supplier company information and contacts. Company Name – must reflect what is in the Intel systemResponse date, supplier Representative and contact details. The names, email and phone numbers are mandatory information so in case of questions, Intel is able to contact the right supplier contacts.

Legal Statement A True response is expected to affirm supplier’s submitted data is accurate and the supplier understood that Intel’s reliance on the data to indicate compliance to regulatory standards.

Uncertainty Statement

This is the section where supplier can add any uncertainties and caveats to their declaration.

Create IPC 1752A section

This section offers 3 features:Create an optional IPC1752A file for their declaration. This is provided as an extra feature that allows suppliers the flexibility of generating an IPC1752A xml file, if the same data is provided to their other customers and these customers can only process IPC1752A xml files.

Sign and Lock. Once the file passed validation, Sign and Lock feature allow the users to sign the file, locks the data, save a copy of signed/locked file for submission to Intel. Sign and lock ensures that once submitted, the file content is preserved.

Clone Declaration allows user to edit the file after the data has been signed/locked.

Product id This is the Manufacture Part Number (MPN) as captured in the system. When the file is uploaded to the Adhoc inbound, the system checks the Company Name + Product ID and initiate a search against the CMAP part tree for the associated record. This is how the file is mapped to items in the Adhoc Inbound.By default, one L1 row is added – indicating that one product is expected for every file submitted. The starting row cannot be deleted from the table.

Product Description Product Description. This is a supplier entered description for their product

Qty This is grayed out for Level 1 – not applicable – but is required for L2 and lower (subpart – how many of these L2 (subparts) make up the L1 (product). This key attribute is important to calculate the final product weight.

Mass Mass is the weight of the product or product part

Mass Type The product or product weight unit of measure either in g, mg or ug.

Add Top Level This button is used to add more products to the product table. Although the system is able to handle multiple products, users must be careful when providing multiple products in one file, making sure all the products share the same declaration and claimed exemptions.

Add Child This button is used to add subparts to product, or subparts to another subpart. There is no limit to the level of children but typically, most products will only have 2 levels: Product and product parts.

Delete Row This button is used to delete entry(ies) in the product table. To delete, highlight the product or product part row you want to remove from the list and then click Delete row.

Exemptions This table is used to capture any valid EU ROHS exemptions

claimed for the product.

- A blank exemption row is added by default. If

exemptions are not going to be claimed, leave this

row as is.

- If exemption is to be claimed, click the Exemption ID

cell, a dropdown will then available. Selecting the

Exemption ID automatically populates the Exemption

List, Authority and Exemption columns.

- If more than 1 exemption is to be claimed, click Add

Record option to add a new exemption row for filling

in.

Query List The Query List displays a list of standard and custom query

statements that supplier will need to respond to. The

respond is a summary of the data whether the product is

compliant or not compliant to EU ROHS, whether the

product if Low Halogen and if the product contain REACH

substance above threshold.

1. All the query statements must be responded to

2. For ROHS, the responses must not be contradictory

or ust be consistent with any claimed exemptions.

3. In cases where there is conflict between the query

statement response and the supporting data,

compliance software will take the worst case.

Example For Training Purpose Only

The responses for Low Halogen only apply to plastic materials.

If the query statement is not applicable to your component/part,

the Low Halogen query response is “TRUE” if the material is metal, glass, or ceramic.

Product/SubProduct This column is prepopulated with the product and subproductsentered in the product tab. Clicking on the cell activates the dropdown list. A value is required for each material and substance

Material This cell is a free form text field to capture the material name (see Intro for definition).

Material Mass (mg) This refers to the material weight. If multiple materials are declared, the sub of all the material weights must roll up to 100% of the product part or product weight

Substance This is the substance being reported.

CAS This is the CAS # of the substance

Substance Mass (mg) This is the substance weight in mg. The total substance weights must roll up to 100% of the material weight.

Exemption If a ROHS substance is reported and an applicable substance is claimed, click on the Exemption cell to activated dropdown list.

Note Notes:

1. Rows 7-12 show an example of correct FMD reporting

structure. These rows cannot be deleted from the tab.

2. All the columns within the rows must be filled. System

interprets a blank cell row or column as end of the data entries

thus ending the data ingestion process.

3. When the substance name and the CAS number are not

matching, system takes the CAS number and populates the

substance name from the CAS number stored in the system.

Remove the filtering prior to sign/lock.
