Medical Exam Instructions

All immigrant visa applicants, regardless of age, require a medical examination prior to the issuance of a visa. Only a physician accredited by the U.S. Embassy can perform this exam. It is your responsibility to schedule a medical exam with one of the clinics listed below before your visa interview appointment at the U.S. Embassy. Medical examination results from other physicians will not be accepted.

Items to bring to your medical examination

The doctor will need the following items to complete the medical exam forms:

• Your visa interview appointment letter

• Your passport

• Four ( 4) recently taken passport-sized color photographs

• A copy of your immunization records

• DS-260 Confirmation Page Any medical examination fees, including x-ray and blood test fees, must be paid directly to the examining physician. The current exam fee is $230 USD for applicants age 15 years and older, and $100 USD for children age 14 years and younger. Any necessary vaccinations or additional tests are not included in this fee. During the medical exam

The medical examination will include a medical history review, physical examination, and chest X-ray, gonorrhea test, and blood tests (for applicants 15 years of age or older). The United States also requires tuberculosis (TB) testing for all applicants two years of age and older. Please be prepared to discuss your medical history, medications you are taking, and current treatments you are undergoing. Medical Exam Instructions

After the medical exam

When your examination is completed, the doctor will provide you with exam results in a sealed envelope or send them directly to the U.S. Consulate. IF GIVEN AN ENVELOPE TO CARRY TO YOUR INTERVIEW, DO NOT OPEN THIS ENVELOPE.

Instead, bring it to your visa interview. Any x-rays taken will be given to you. You DO NOT need to bring the x-rays to your visa interview unless you suffer from tuberculosis (TB). However, you must carry the x-rays with you when you travel to the United States for the first time. The medical report must be less than six months old when you enter the United States as an immigrant.

Pre-Interview Checklist

Please use the list below to determine the items that every applicant must bring to the immigrant visa interview:

Please monitor the email address that you provided on your immigrant visa application form DS-260. You may receive an email from [email protected] with a request to take some action on your case or prepare additional documentation before you come to your visa interview.

A copy of your NVC interview letter (does not apply to Diversity Visa, fiancé(e), adoptive, or asylee/refugee applicants).

Unexpired passport valid for six months beyond your intended date of entry to the United States and a photocopy of the biographic page (where your name and photo are located).

Two (2) color photographs of each person applying for a visa sized 5 cm x 5 cm (2 inch x 2 inch). Please review our online photo requirements.

Confirmation page from the Form DS-260 Application for an Immigrant Visa you submitted online at ceac.state.gov/iv.

Your original birth certificate and a photocopy.

Medical examination results in a sealed envelope (if the physician gives you these results).

Original birth certificates for all children of the principal applicant (even if he or she is not accompanying).

Applicants who fall into any category listed in italics below should bring these additional documents:

For family-based visa applications:

The appropriate Form I-864 Affidavit of Support for each financial sponsor along with a photocopy of the sponsor’s IRS transcript or most recent U.S. federal income tax return, and any relevant W-2s.

Proof of your U.S. petitioner’s status and domicile in the United States (photocopy of a U.S. passport, naturalization certificate, or lawful permanent resident card).

Evidence of the relationship between the petitioner and visa applicant (such as photographs, letters, or emails).

If you are married: Your original marriage certificate, and a photocopy.

If you were previously married: Your original divorce or spouse’s death certificate, and a photocopy.

If you are older than 16 years of age: The original police certificate from your country of current residence and countries of previous residence. If these three items are all true, you must bring a more recent police certificate to the interview:

1. You are older than 16 years;

2. You obtained a police certificate more than one year ago; and

3. You still live in the country that issued the police certificate.

Pre-Interview Checklist

For employment-based visa applications: Letter from your U.S. employer dated less than one month ago.

If you have ever been convicted of a crime: Court and criminal records, and a photocopy.

If you are adopted: Adoption papers or custody documents, and a photocopy.

If you are the petitioner’s stepchild: The original marriage certificate of the petitioner and your biological parent, and a photocopy along with original divorce records for any previous marriages of either parent.

If any of the documents you are submitting is in language other than English or Serbian and congenial languages: Certified English translation

Interview Guidelines

Sending documents to the U.S. Embassy

If the U.S. Embassy requests additional information or documentation, the letter given to you by the Consular Officer at the interview will instruct you on how to complete your application. Please follow the instructions in the letter. Rescheduling or cancelling your interview

If you are unable to attend your appointment, please send an email to the U.S. Embassy in Belgrade. There may be a significant wait before the next available appointment, so please attempt to attend the date already assigned. For some family-based and employment preference visa categories, a visa became available within the month you have been scheduled by NVC. DV applicants should be aware that visas are numerically limited and must be issued by September 30 of the program year. There is no guarantee that a visa will still be available on the date of your rescheduled interview. Please carefully consult the Visa Bulletin before you decide to reschedule your interview. Security screening procedures

All visitors to the U.S. Embassy must follow certain security procedures. Any visitor who declines to be screened by U.S. Embassy security personnel will be unable to enter the embassy. To avoid delaying your entry and that of those in line behind you, please bring only what is required for your interview. The following items are not permitted at the embassy: Electronic devices including, but not limited to: cell phones, computers, cameras, earphones, USB flash drives, etc. Sharp objects including but not limited to: Knives, tools, scissors etc. Liquids and sprays including but not limited to: Deodorants, perfumes, bottled drinks, etc. Accompanying persons

The following persons may accompany a visa applicant to their interview:

• Interpreter: Applicants may bring ONE interpreter if they do not speak English or Serbian well enough to participate in an interview.

• Petitioner: Applicants may bring the petitioner if they are applying for the immigrant visa in a family based classification.

• Special Needs Visitors: Applicants may bring ONE person to help if they are elderly, disabled, or a minor child. Attorneys are not permitted to accompany clients into the waiting room or to their interview. Please send an email to the U.S. Embassy in Belgrade prior to your interview if you plan to bring an accompanying person to the interview. Immigrant visa fees

If you have not paid all required fees to either the National Visa Center or via the appointment website, please be prepared to pay these fees on the day of your interview. All fees may be paid in U.S. dollars or the equivalent in Serbian currency at the embassy’s exchange rate. We accept cash or credit card (Visa, MasterCard, American Express, Novus/Discover or Diners). Checks are not accepted. Please note that if you are found ineligible to receive a visa, the application fee cannot be refunded. A complete list of fees can be found here.

After Your Visa Interview

A consular officer can make a decision on a visa application only after reviewing the formal application and interviewing the applicant. There is no guarantee that you will receive a visa. Do not sell your house, car or property, resign from your job or make non-refundable flight or other travel arrangements until you have received your immigrant visa. If more information is needed

Sometimes a consular officer is unable to make a decision on a visa application because he/she needs to review additional documents or the case requires further administrative processing. When additional documents are requested, the consular officer will give you a refusal letter that asks you to submit additional documents. The letter will include instructions on how to send those documents to the Embassy. Administrative processing takes additional time after the interview. Most administrative processing is resolved within 60 days. However, the timing varies based on the circumstances of each case. Before inquiring about the status of administrative processing, please wait at least 60 days after your interview. What happens after visa approval

Passport, Visa, and Sealed Immigrant Packet – We will place your immigrant visa on a page in your passport. Please review your visa to make sure there are no spelling errors. We will also give you a sealed envelope containing documents that you must give to U.S. immigration authorities when you arrive in the United States for the first time. Do not open this envelope. You must carry it with you; do not put it in your checked luggage. If you receive X-rays during your medical examination, carry those with you and give them to the U.S. immigration authorities. USCIS Immigrant Fee – All individuals who are issued immigrant visas overseas must pay an Immigrant Fee to U.S. Citizenship and Immigration Services (USCIS) prior to traveling to the United States. This fee is for processing your residency status and printing your Permanent Resident Card. The only people exempt from paying this fee are: children entering the United States under the Hague Process, returning residents, and people traveling on a K visa. When You Should Travel – You must enter the United States before the expiration date on your visa, which is usually six months from the date of your medical exam. Your visa cannot be extended and all fees are nonrefundable. The principal applicant must enter before or at the same time as other family members with visas. Getting a Green Card – Your Form I-551 Permanent Resident Card, also known as a green card, will be automatically mailed to the address in the United States that you write in your visa application form. This is a very important document that proves you have permission to reside in the United States. Do not travel outside of the United States until you receive your Permanent Resident Card. Once your card is issued, you should not stay outside of the United States for more than one year. If you do, you will lose your status as a Lawful Permanent Resident. Children’s Issues – Children are required to have certain vaccinations before they can enroll in school in the United States. Therefore, we recommend that you bring your child’s complete vaccination records with you to the United States. Additionally, if your child is adopted, you have full custody as a result of a divorce, or you share custody with the child’s other parent, we recommend that you bring a copy of all applicable adoption or custodial papers from the authoritative court in your home country. You will likely need these papers (translated into English) in the United States for issues such as school enrollment, medical care, and eventual citizenship. Diversity Visa Applicants – Additional Information

If you are interviewing for a Diversity Visa (DV), all of the above instructions apply to you. Please schedule and attend a medical examination prior to your visa interview; enroll in the required courier service; and gather the required documents. Below are additional instructions that apply only to DV applicants. Bring to your interview

In addition to the documents listed on the Pre-Interview Checklist in this package, DV applicants should also bring the following items to your visa interview:

Appointment information printed from the “Entrant Status Check” on the E-DV website

Documents showing that you have either a qualifying high school education OR have two years of qualifying work experience in the last five years immediately prior to application (for the principal applicant only; more information is available online).

Payment of the $330 Diversity Visa Application Fee

Review your DV Lottery entry

Prior to your visa interview, we recommend that you review the data on your initial E-DV entry. On your initial E-DV application, you must have correctly entered your marital status. If you are legally married you must have listed your spouse, even if you are currently separated from him/her (unless your spouse is a U.S. citizen or Lawful Permanent Resident). Additionally, you must have listed ALL of your living children who are unmarried and under 21 years old. This includes your natural children, your spouse’s children, or children you have formally adopted in accordance with the laws of your country. Failure to have listed an existing spouse or children at the time of your entry in the Diversity Visa lottery will result in the denial of your visa and visas for your family. Any fees paid to the U.S. government in support of your visa application(s) are nonrefundable. If you failed to include a child who had already been born, or a spouse to whom you were married when you entered the lottery, you should not proceed with the visa application. Lekarski pregled je neophodan za sve podnosioce molbe za useljeničku vizu, bez obzira na uzrast. Samo lekar akreditovan od strane Ambasade SAD može da obavi ovaj pregled. Vaša je obaveza da zakažete lekarski pregled u jednoj od dolenavedenih ordinacija, pre razgovora za vizu u Ambasadi SAD. Rezultati lekarskih pregleda od drugih lekara neće biti prihvaćeni.

Rentgenski snimci će vam biti predati, ali NEMA POTREBE da ih donosite sa sobom na razgovor, osim ako ne bolujete od tuberkuloze (TB). Pripremna čeklista za razgovor

Smerice za razgovor Slanje dokumenta u Ambasadu SAD

Ukoliko Ambasada SAD zahteva dodatne informacije ili dokumentaciju, pismo koje dobijete od konzularnog službenika na razgovoru će da vas uputi kako da dopunite svoju molbu. Molimo vas da pratite instrukcije iz pisma. Pomeranje ili otkazivanje razgovora

Ukoliko niste u mogućnosti da dodjete na sastanak, molimo vas da stupite u kontakt sa Ambasadom SAD u Beogradu putem elektronske pošte. Moguće je da novi sastanak nećete moći da dobijete odmah, tako da vas molimo da dodjete na zakazani sastanak. U nekim useljeničkim kategorijama na osnovu srodstva i na osnovu zaposlenja, viza je postala dostupna u mesecu u kome vam je NVC zakazao razgovor. Podnosioci molbe za vizu u DV programu moraju da znaju da je broj viza numerički ograničen i da vize moraju da budu izdate najkasnije 30. septembra u programskoj godini. Svi posetioci Ambasade SAD moraju da prođu kroz određene bezbednosne provere. Posetiocima koji odbiju proveru od strane obezbeđenja ambasade neće biti dozvoljen ulazak. Molimo vas da ponesete samo ono što je neophodno za razgovor da ne bi dolazilo do zastoja pri ulasku. Ovi predmeti nisu dozvoljeni u ambasadi: Elektronski uređaji kao što su: Mobilni telefoni, računari, kamere, slušalice, USB prenosive memorije i sl. Oštri predmeti kao što su: Noževi, alat, makaze i sl. Tečnosti i raspršivači kao što su: Dezodoransi, parfemi, flaširana pića i sl. Osobe u pratnji

Osobe koje mogu da budu u pratnji podnosioca molbe za vizu su:

• Prevodilac: Podnosioci molbe za vizu mogu da povedu jednog prevodioca sa sobom ukoliko ne govore dovoljno dobro ni srpski ni engleski jezik da bi učestvovali u razgovoru.

• Podnosilac peticije: Podnosioci molbe za vizu mogu da povedu podnosioca peticije, ukoliko traže vizu u useljeničkoj kategoriji na osnovu srodstva.

• Posetioci sa specijalnim potrebama: Podnosioci molbe za vizu koji mogu da povedu jednu osobu ukoliko su starije životne dobi, deca ili lica sa posebnim potrebama.

Pravnim zastupnicima nije dozvoljeno da budu u pratnji svojih klijenata u čekaonici ili na razgovoru. Molimo vas da stupite u kontakt sa Ambasadom SAD u Beogradu putem elektronske pošte, pre razgovora, ukoliko planirate da povedete pratnju na razgovor. Takse za useljeničku vizu

Ukoliko niste platili sve neophodne takse preko Nacionalnog viza centra, ili preko internet stranice za zakazivanje, budite spremni da platite ove takse na dan svog intervjua. Takse mogu da se plate u američkim dolarima ili dinarskim ekvivalentom po kursu ambasade. Moguće je platiti gotovinom ili kreditnom karticom (Visa, MasterCard, American Express, Novus/Discover or Diners). Čekovima nije moguće platiti. Posle razgovora

Konzularni službenik može da donese odluku samo nakon razmatranja zvanične molbe za vizu i razgovora sa podnosiocem. Nepostoje garancije da ćete dobiti vizu. Nemojte prodavati svoju kuću, kola ili imanje, ostavljati posao ili praviti aranžmane za let ili drugi vid putovanja bez mogućnosti povraćaja sredstava, pre nego što dobijete svoju useljeničku vizu. Ukoliko su potrebne dodatne informacije

U određenim slučajevima konzularni službenik nije u mogućnosti da donese odluku po molbi za vizu zbog toga što je neophodno da se razmotri dodatna dokumentacija, ili je u predmetu neophodna dodatna administrativna procedura. Ukoliko su potrebna dodatna dokumenta, konzularni službenik će vam dati odbijenicu koja traži da se podnese dodatna dokumentacija. U odbijenicu će biti uključena uputstva kako da dostavite ovu dokumentaciju ambasadi. Administrativna procedura će da uzme dodatno vreme nakon razgovora. U najvećem broju slučajeva procedura se rešava u roku od 60 dana. Međutim, ovaj rok varira u zavisnosti od okolnosti u svakom predmetu. Molimo vas da sačekate najmanje 60 dana, pre nego što počnete da se raspitujete o statusu administrativne procedure. Nakon odobrenja vize

Pasoš, viza i zapečaćeni useljenički paket – Useljenička viza će da bude na stranici u vašem pasošu. Molimo vas da proverite da li na vizi ima eventualnih grešaka. Takođe ćemo vam predati zapečaćenu kovertu sa dokumentacijom koju morate da predate imigracionim vlastima kada po prvi put stignete u Sjedinjene Države. Ne otvarajte ovu kovertu. Neophodno je da je nosite sa sobom; nemojte je pakovati u prtljag koji se predaje. Ukoliko ste dobili rentgenski snimak tokom lekarskog pregleda, nosite ga sa sobom i predajte imigracionim vlastima Sjedinjenih Država. USCIS useljenička taksa –Neophodno je da osobe kojima je izdata useljenička viza plate useljeničku taksu Službi za državljanstvo i imigraciju (USCIS) pre puta u Sjedinjene Države. Ova taksa je za obradu vašeg rezidencijalnog statusa i izradu vaše kartice stalnog rezidenta. Jedine osobe oslobođene plaćanja ove takse su: Deca koja ulaze u Sjedinjene Države u haškom procesu, rezidenti povratnici i osobe koje putuju sa K vizom. Kada treba da putujete – U Sjedinjene Države morate da uđete pre datuma isticanja naznačenog na vizi, a to je u najvećem broju slučajeva šest meseci od datuma vašeg lekarskog pregleda. Viza se ne može produžiti, a sve takse su nepovratne. Nosilac porodične aplikacije mora da uđe pre ili u isto vreme kao i drugi članovi porodice sa vizama. Prijem zelene karte –Vaša I-551 kartica stalnog rezidenta, poznatija kao zelena karta, će biti automatski poslata na adresu u Sjedinjenim Državama naznačenu u molbi za vizu. Ovo je veoma važan dokument koji dokazuje vaše pravo da boravite u Sjedinjenim Državama. Nemojte da putujete van Sjedinjenih Država dok ne primite karticu stalnog rezidenta. Kada vam je kartica izdata, ne treba da ostajete van Sjedinjenih Država duže od godinu dana. Ukoliko ostanete izgubićete status stalnog rezidenta.

Napomene o deci – Neophodno je da deca budu vakcinisana pre nego što se upišu u školu u Sjedinjenim Državama. Zbog toga preporučujemo da ponesete sa sobom vakcinacioni karton. Ukoliko je vaše dete usvojeno , ili ukoliko imate puno starateljstvo nakon razvoda, ili ukoliko delite starteljstvo sa drugim roditeljom, preporučujemo da ponesete sa sobom odgovarajuću dokumentaciju o usvojenju, starateljstvu od vlasti u vašoj domovini. Ova dokumentacija, zajedno sa prevodom na engleski će vam verovatno biti potrebna u Sjedinjenim Državama da da biste rešili upis deteta u školu, zdravstveno osiguranje i eventualno državljanstvo. Ukoliko imate razgovor kao učesnik u DV programu, sva prethodno data uputstva se odnose na vas. Molimo vas da zakažete i obavite lekarski pregled pre razgovora; registrujte se kod kurirske službe; prikupite sva neophodna dokumenta. Slede dodatna uputstva koja se odnose isključivo na učesnike u DV programu. Ponesite na razgovor

Pored dokumenata nabrojanih u pripremnoj čeklisti za razgovor, učesnici u DV programu takođe treba da ponesu sledeće stavke na svoj razgovor:

Documentaciju koja pokazuje da imate ili srednjoškolsko obrazovanje koje vas kvalifikuje za ovaj prgram ILI dve godine radnog iskustva u posledenjih pet godina neposredno pre podnošenja molbe za vizu u određenim zanimanjima (samo za nosioca programa; više informacija je dostupno online).

Proverite podatke na prijavi za DV program

Preporučujemo da pre razgovora proverite podatke sa svoje inicijalne E-DV prijave. U ovoj prijavi ste morali da registrujete svoj bračni status. Ukoliko ste zvanično u braku bilo je neophodno da registrujete bračnog druga, čak i ako ste trenutno razdvojeni od njega ili nje (osim ako vam je bračni drug državljanin-ka ili stalni rezident Sjedinjenih Država). Pored ovoga, bilo je neophodno da registrujete SVU decu koja nisu u braku i imaju manje od 21 godine. Ovde spadaju vaša biološka deca, deca vašeg bračnog druga, ili deca koja su zvanično usvojena u skladu sa zakonom vaše zemlje. Propust da uključite postojećeg bračnog druga ili decu u vreme prijave za DV program će dovesti do odbijanja vaše vize i viza za vašu porodicu. Takse plaćene vlastima Sjedinjenih Država za podnošenje molbe za vizu su nepovratne. Ukoliko ste propustili da uključite dete koje je bilo rođeno, ili bračnog druga s kojim ste bili u braku kada ste se prijavili za lutriju, ne treba da nastavite sa procedurom.
