Board meetings are convened at 6:00 p.m. and Work Sessions are convened at 5:30 p.m., unless otherwise advertised in advance of the meeting(s). The meetings for the Board of Trustees are held in the Board Room at the Administration Center, which is located at 315 E. North Drive. The meeting site is fully-accessible. Any person requiring further accommodation should contact the Secretary of the board, at the School Corporation’s central office.

The annual calendar in no way restricts the Board’s ability to establish special meetings, executivesessions,worksessionsorretreats,provideditgivesthenoticerequiredbystatute.

WorkSessionshavebeenscheduledonthe2ndTuesdayofeachmonthwiththeexceptionofJanuary,March, September and December. These meetings and/or additional meetings may be scheduled or canceled if needed.

The Board of School Trustees is an elected seven-member board. The Board meets the fourth Tuesday of each month at 6:00 p.m. in the Board Room at the Administration Center, 315 E. North Dr., Bloomington. Occasionally, a meeting time or place may be changed. Meeting announcements and/or changes are publicized in advance via news media releases. Comment cards will be available prior to each meeting for any member of the community who wishes to address the board.

On behalf of our Board of School Trustees and entire staff,

congratulations on an excellent 2012-2013 school year and welcome

to school year 2013-2014!

We look forward to the opportunity to serve the academic needs of

our students.

Our staff continues to work collaboratively to develop curriculum and formative

assessments as a guide to frame and focus instruction. We continue to offer

instructional strategies and specific opportunities for students to receive help and

enrichment.

MCCSC students continue to demonstrate growth and our state scores continue to

increase along with numbers of students taking more rigorous courses. In addition,

we have focused on career pathways to assist students, parents and families on

suggested courses that will provide strong foundations and allow students to determine

their interests. We congratulate all of our students, staff and parents for their dedicated

efforts in ensuring their student’s learning.

We look forward to another outstanding year for our MCCSC students.

3. Are immunizations required for all children enrolled in school?

Yes. Students in all grades are requiredtomeettheminimum

immunizationrequirements.Im-munizationrequirementsextendtochildrenages3through5attending special education pro-grams, child care, or preschool within the school building.

4. What information must be included on the physician’s statement to document im-munization?

The statement must include thestudent’snameanddateof birth, the vaccine given and date (month/day/year) of each immunization.

5. What is considered ad-equate documentation of an immunization history?

Adequatedocumentationisasfollows: documentation from a healthcare provider, an im-munizationrecordfromanotherschool corporation, an immu-nizationrecordintheIndianaImmunizationRegistry(CHIRP),or a printed record from another state registry. This documenta-tion must include the month, day, and year for each dose of vaccine administered.

6. What is “laboratory evidence of immunity”?

Laboratory evidence of immunity isabloodtestfordisease-spe-cific immune globulin that mea-sures immunity to disease. This is often used to confirm immunity whenimmunizationrecordsarenot available, or a parent reports a history of disease.

7. Who should interpret lab results for evidence of immunity?

Laboratory results for evidence of disease immunity must be ordered by a physician. The ordering physician is responsible for interpreting the results and determiningadequateevidenceof immunity based on current medical guidelines.

8. Is lab evidence of immunity acceptable for ALL school required immunizations?

No. Lab evidence is NOT acceptable for Diphtheria, Pertussis, or Tetanus. Labora-tory evidence of immunity may beusedinplaceofimmunizationrequirementsforthefollowingschoolrequiredimmunizations:Measles Mumps Rubella Chickenpox Hepatitis A&BPolioSerology testing for chickenpox can only be used to document history of disease

9. What is the four-day grace period and when can it be used?

CDCandACIPallowa4-daygrace period. If a vaccine is given up to 4 days before the minimum recommended age for administration of the vaccine, it can be counted as valid. How-ever, this does not apply to every vaccine and does not change the recommended schedule for routine vaccine administration. The4-daygraceperioddoesnotapplytotheminimum28-dayinterval between 2 live virus vac-cines(MMR,LAIVandVaricella).

10. What is the minimum age for MMR vaccine to be counted as a valid dose?

For the MMR to be counted as

a valid dose, it must have been given on or after the first birthday. The four day grace period is ap-plicable to MMR vaccine.

11. When are 4 doses of Polio

vaccine required? Four doses of polio are con-sidered a complete series. The fourth dose must be adminis-tered on or after the 4th birthday, and be administered at least 6

12. What are the minimum intervals for Hepatitis B vaccine?

The minimum intervals between vaccinedosesare:Dose1and2is4weeks(28days)Dose2and3is8weeks(56days)Dose1and3is16weeks(112days)Note: The minimum age for the 3rd dose of Hepatitis B vaccine is 24 weeks (164 days).

13. If there is an extended interval between doses of Hepatitis B, does the student need to start the series over?

No. The hepatitis B series should never be restarted or additional doses given due to an extended interval between doses. The student should just complete the series with the remaining dose(s) due.

14. May a chiropractor give a medical exemption for vac-cination?

No. Onlyalicensedphysician(M.D.orD.O.)canprovideamedicalexemption.Anursepractitioner or a physician assistantunderaphysician’ssupervision can also give a medi-cal exemption.

15. What must a medical exemption contain? Amedicalexemptionisaphysician’scertificationthataparticularimmunizationmaybedetrimentaltothechild’shealth.

It must state in writing that the child has a medical contraindica-tion to receiving a vaccine. Many contraindications to vaccination are not permanent; therefore, a medical exemption should be obtained for the student each schoolyear.Astruemedicalcontraindicationstoimmunizationarevaccine-specific,medicalexemptions must be written for each vaccine that is contrain-dicated. There is a state form providers can use to submit a medical exemption for students.

16. What must a religious objection contain?

Areligiousobjectionmuststatethat the objection to immuni-zationisbasedonreligiousgrounds. Each objected im-munizationmustbespecified.The objection must be in writing, signedbythechild’sparent,anddelivered to the school. There isnorequirementofproof.Toensure the continued religious objection status for a student, schoolsmustrequirewrittendocumentation of the religious objection each school year.

17. Is there a philosophical objection allowed in Indiana?

No. Indiana law only allows reli-gious and medical exemptions.

18. If a child does not presen-tan immunization record or is not up to date with his/her immunizations, may he/she enroll in school?

Yes.IndianaCode(IC20-34-4-5)states that a child is not permit-ted to attend school beyond the first day without furnishing a written record, unless: • Theschoolgivesawaiver(fora period not to exceed 20 days); or • Thelocalhealthdepartmentor a physician determines that thechild’simmunizationshave

been delayed due to extreme circumstances and that the requiredimmunizationswillnotbe completed by the first day of school. The parent must furnish a written statement and a time schedule approved by a physi-cian or health department; or • Amedicalorreligiousexemp-tion is on file.

19. If a student received a meningococcal vaccine noted from an electronic medical record, how do I know if they meet school requirements, and how do I document this in CHIRP?

Most likely, the student received a doseofthequadrivalentmenin-gococcal conjugate vaccine (MCV4). There are 2 conjugate vaccines: Menactra (MCV4P) andMenveo(MCV4O).If thelot# is included on the record, you can determine which vaccine the student received. Menactra is a Sanofi Pasteur product and lot#’stypicallybeginwitha“U”.MenveoisaproductfromNovar-tisandlot#’sbeginwiththeletter“M”.If thelot#isnotavailable,you may document the dose usingeitherMCV4PorMCV4O;use MCV4P if the vaccine was administered prior to March 2010.Onlydosesofmeningo-coccal vaccine administered on orafterthe10thbirthdaymeettheschoolrequirements.Anydose of meningococcal vaccine administeredpriorto2006isthemeningococcal polysaccharide vaccine (MPSV4). If a child received the meningococcal vaccine overseas, and the record does not specify MCV4, please contacttheISDHImmunizationDivision for further guidance.

20. If a child receives one dose of Varicella vaccine and then subsequently has chickenpox, is a second dose of Varicella vaccine needed?

No. The parent or physician will need to document the history of the disease.

21. Is a doctor’s statement required as proof of chicken-pox disease? • Forchildrenenteringpreschoolthrough5thgrade,asigned statement by a health care provider, documenting a diagnosis of varicella or verifying the history of disease, includ-ingdate(MM/YY)isrequired.Achild does not need to see their healthcare provider at the time ofillnessinordertorequestaverification of disease history. Aparentcanspeakwiththeirchild’shealthcareprovidertoascertain whether the child had chickenpox based upon the disease presentation and transmission. • Forchildrenenteringgrades6-12,documentationfromaparentissufficient.Awrittenstatement should include date ofdisease,aparent’ssignature,and date of signature. (Example: If a parent cannot recall exact dates, something as simple as stating that disease occurred in the spring of 2000 is acceptable)

Schools do not need to verify the history of disease for stu-dents with current documenta-tion in CHIRP.

Reporting

1. How do I submit my report using CHIRP?

IC20-34-4-6specifiesthatschools are to submit a written report of vaccination data levels. Schools are now reporting their vaccination data levels using CHIRP; there is no need to submit a written report to ISDH

withtheuseof theregistry.Onlystudents who are added to a school roster will be included in the annual reporting completed by ISDH. Parents must provide consentunderFERPApriortoastudent’srecordbeingaddedtothe school roster.

2. Do schools provide sum-mary reports to ISDH on the immunization status of students in all grades?

Whileallstudentsenrolledinschoolarerequiredtobeup-to-dateonallrequiredimmuniza-tions, schools only provide sum-mary data to ISDH on students enrolled in kindergarten, first, and sixth grades at this time.

3. If a child has an exemp-tion on file, may he/she be counted as complete?

No. If a child has an exemption onfileforanyimmunization,he/she must have the exemption reported in CHIRP.

4. Do schools need to report immunization data for all 6th grade immunizations?

Yes. Including varicella and Hepatitis B, MCV4, and Tdap.

5. What is the deadline for reporting school immuniza-tion data?

Student records must be checked upon enrollment for compliance with the school immunizationrequirements.PerIC20-34-4-5,studentsshouldnot be in attendance unless theappropriateimmunizationrecords have been presented to the school. For reporting purposes, data entered into CHIRPthroughFebruary1stwillbe included on the annual report for schools. 6. Does ISDH determine if a child is excluded from school

for incomplete immuniza-tions?

No. School exclusion is deter-mined by the school corporation accordingtoIC20-34-4-5.ISDHstrongly recommends adherence to this code.

7. What immunization education materials must be provided to the parents of enrolled students?

• Meningococcal disease–Allstudents(IC20-30-5-18)• Human Papillomavirus (HPV) Infection—6thgradefemalestudents(IC20-34-4-3).It is strongly recommended that this same information be provided to parents of enrolled 6thgrademales.

8. Are schools required to collect the response form included with the Human Papillomavirus (HPV) infection educational materials?

Yes.Schoolsarerequiredtocollect HPV response forms from parents of sixth grade female studentsunderIC20-34-4-6.However, forms should not includethestudents’nameand should not be returned to ISDH. Schools will complete a summary report of responses received from HPV forms and submit the report to ISDH. The survey can be completed online or on paper.

9. Are schools required to send parents information about Pertussis and the Tdap vaccine?

No. ISDH only recommends that schools send this information home to parents.
