CLINICAL ACTIVITIES

CLINICAL ACTIVITIES SUMMARY My clinical expertise in patient safety and what I teach other nurses closely aligns with my program of scholarship and I feel the research, teaching, and clinical foci work together in a symbiotic relationship. For the past almost 25 years as a nurse, I have worked in a variety of highly ranked academic and community-based health care settings caring primarily for frail older adults. I have also worked with nurses in these settings on implementing and evaluating quality improvement and safety projects using the Clinical Microsystems approach on a variety of patient safety topics such as fall prevention, restraint and siderail reduction, minimizing laboratory errors, and minimizing problematic behaviors. I currently practice approximately 200 hours per year precepting nurse practitioner students in a variety of community based settings in San Francisco including the University Mound Ladies Home (2012- 2014), Heritage on the Marina (2014-present), and Baview Hunters Point Multipurpose Senior Services (2015- present) all focused on care of the older adult living with multiple chronic conditions. I coin this experience "Disparities of Elder Care in San Francisco." Prior to my arrival at UCSF School of Nursing, there were limited clinical experiences for the Adult Gerontological Nurse Practitioner students in care of frail elders. Thus, I have been able to significantly increase student exposure and building capacity in advanced practice geriatric clinical nursing for our nurse practitioner students.

UNIVERSITY AND PUBLIC SERVICE

SERVICE ACTIVITIES SUMMARY I have immersed myself in numerous internal and external service activities. I am actively involved in a wide variety of committees and organizations within the School of Nursing and across campus. I also work closely with the UCSF Nursing Development Directors from the University Development and Alumni Relations office to identify and cultivate potential donors, speak with faculty about their role in the development process, and attend development events. In 2018, I played a pivotal role in securing a $500K corporate gift to fund research at the School of Nursing in technology and mobile health. In 2019, I have been instrumental in securing over $500K in both restricted gift funding and endowed scholarship gifts from donors. At UCSF, these past two years I have participated on the UCSF Distinction in Mentoring Selection Committee and I am currently a member of the UCSF Graduate Council. Nationally and internationally, I participate in grant, journal, and scholarship reviews. I also participate on the planning committee for a biennial Canadian National Falls Prevention conference. My role on this committee is primarily to secure speakers as well as to lead the abstract submissions and approvals process. I have also served on a number of search committees (e.g., Family HealthCare Nursing Department Chair). I have served on the PhD screening committee since 2014 with assuming the Chair of PhD Admissions committee in cycles 2016, 2017, and 2018 and mentoring the new chair and my departmental replacement in 2019. I was also recently the Chair of the Education Design Team for the School of Nursing's Strategic Plan in 2018. In that capacity, I led several meetings and collaborated with the consultants on education strategies to present to the school to be included in the Strategic Plan.

CONTRIBUTIONS TO DIVERSITY

CONTRIBUTIONS TO DIVERSITY I am involved at the University level in the First Generation to College Initiative. In that capacity, I mentor and inspire a diverse group of students who are like me: the first in their family to attend college. I have received funding through a few sources, both intramural and extramural, to develop a program (First GenRN) to better support students who are First Generation to College. I am now expanding the program across four Bay Area Schools of Nursing through a grant from the CalWellness Foundation. I am proud to say that I am the Founder of this program. I work closely with the UCSF Office of Student Life on this project with the ultimate goal to disseminate this throughout all UCSF Schools and we frequently connect with health professions schools outside of UCSF. My work in this area has allowed me to receive national recognition in cultivating the careers of FirstGen Students. In 2018, I was invited to present an inaugural endowed lectureship on my work at UNLV and I presented on my research at the inaugural UCOP "First Generation Students at UC: Strengthening our Approach to Support their Success" at UCSD. Tae-Sun Kim, UCOP's Manager of Strategic Diversity Initiatives had this to say about my presentation, "Dr. Wagner’s presentation had to be assigned to the largest breakout room available. She presented on FirstGenRN, and gripped the audience with her explanation of race concordance in healthcare." In 2019, UCSF was named an inaugural FirstGen First Forward campus by NASPA - Student Affairs Administrators in Higher Education, a national organization leading the way in FirstGen initiatives. The FirstGenRN program was a critical piece to receiving this designation. I am now identified on a national level as the "go to" person regarding building capital among FirstGen students in the Health Professions. I am currently in the process of planning the first of its kind "FirstGen Health" Conference to be held in Fall 2020 which has secured both intramural and extramural funding. We hope to have at least 150 participants (e.g., Deans, Associate Deans) from across North America attend to learn more about support FirstGen students in Health Professions Education.

TEACHING AND MENTORING

TEACHING SUMMARY My philosophy of teaching centers around using the "flipped classroom" approach with learners. In this approach, students prepare by watching online lectures and come to a smaller seminar fully engaged to work through a particular case study or problem. While I co-teach a large core course at the graduate level with over 150 students, I am a full supporter of differentiated learning in the seminar break-outs which provide students the opportunity to not only teach their peers but also gives them the freedom and flexibilty to be creative as they work through the cases in small groups. I am a strong supporter of student classroom assignments not being just "academic exercises" but rather a valuable experience that results in some type of deliverable such as a publication. My students seek me out for this quality, in that their work will result in a high yield investment. My primary course since arriving at UCSF is N262A: Advanced Scholarship in Research I which all graduate (MSN) nursing students take in the first year of their program. This course has consistently been evaluated by students at or above the mean for courses within my department and have found the course to be extremely organized and especially helpful for writing their culminating comprehensive examination. I have also had the opportunity to mentor teaching assistant and teaching residents each year in the course which has resulted in positively shaping their perceptions in pursuing a career in academic nursing. Further, the course remains an increasingly significant "feeder" for PhD applications and the majority of our incoming PhD students are now former MS students. I continue to be a Specialty Director of the Adult Gerontology Primary Care NP Program, one of the largest NP training programs in the School of Nursing, where I lead and mentor 10 faculty and over 60 students' training. I am the only Senate faculty member in the school who is directing a large clinical training program. This program is nationally ranked by USNWR and my role is multifaceted and expansive. In 2019, I also assumed the leadership position of NP Director for the School of Nursing- a requirement for the California Board of Nursing. In that role, my primary responsibilities are ensuring any new policies or state regulatory changes are implemented in the School (e.g., SB 1109).

MENTORING SUMMARY I have formally mentored over forty research assistants, DNP, MSN, and PhD students over the past ten years. Because of their time in my lab, most of these students have continued to pursue a graduate or professional degree in higher education (law school, medical school, nursing graduate school, PhD programs) or leadership positions, and several have participated in co-authoring my publications. I also have mentored my students in submitting their own publications. My philosophy of mentoring is to provide the mentee as much "hands-on" experience as possible in the experience and adding to their skill base qualities such as: a strong work ethic, passion for the profession, a high level of enthusiasm, and the importance of being organized in verbal and written work. My mentees seek me out for these experiences, and I often have several students who volunteer their time in my lab to gain research experience when no paid positions are available. Students who have worked in my lab in the past have told me consistently that their experience has prepared them well for their future studies. Currently, I am the academic advisor for 3 PhD students. All of these students are conducting research in my area (e.g., patient safety and geriatrics) and they are all First Generation to College. I also am serving increasingly in a mentoring role to the junior faculty in our department- providing career advancement opportunities, mentoring them into leadership roles, and providing support along the way. In 2019, I co-sponsored the First Norwegian nurse to be inducted as a Fellow in the American Academy of Nursing.

RESEARCH AND CREATIVE ACTIVITIES

RESEARCH AND CREATIVE ACTIVITIES SUMMARY As an applied health services researcher, my program of research focuses on reducing adverse events and improving patient safety in nursing home settings. Specifically I focus on improving care processes and identify ways to ensure that safe nursing care is: efficient, ethical, and evidence-based. I have conducted research on bed siderail reduction; improving adverse event reporting systems; assessment of patient safety culture perceptions; examining nursing care activities around the management of falls, including risk assessment and communication among care providers; and adverse event disclosure perceptions among nurses. This research is innovative in that I am one of only a handful of researchers who has tackled the issue of patient safety and translated the Institute of Medicine "To err is human" recommendations in the non-hospital setting. I have received a number of research awards based on my publications and research. I have a number of current research grants and LOIs in progress or under review. Currently, I am 60% extramural research funded despite my extensive service, teaching and administrative responsibilities. I serve as a co-Director for the HRSA UCSF Workforce Center Research in Long Term Care. I am conducting an evaluation study for a pilot inaugural home care voucher program through the City and County of San Francisco, and I have extramural research funded for the FirstGenRN program. In 2019, I received an affiliate faculty appointment with the UCSF Institute for Health Policy Studies given my increasing role in research with my mentor, Joanne Spetz. I have also published a number of papers over the past few years in highly ranked journals (e.g., Journal of the American Geriatrics Society; Nursing Outlook) on my research and have participated in media outreach. Several of my recent publications include my participation as a co-author with colleagues who have sought me out for my expertise as well as my mentees.

Role: Co-Investigator. This research is significant as it provided critical foundational information to develop the Food and Drug Administration and Hospital Bed Safety Workgroup's "A Guide to Bed Safety Bed Rails in Hospitals, Nursing Homes and Home Health Care" Documents that provided solutions to reduce the incidence of patient entrapment in hospital beds. An adaptation of this work was similarly disseminated by Health Canada "Final Guidance Document - Adult Hospital Beds: Patient Entrapment Hazards, Side Rail Latching Reliability, and Other Hazards" in 2008. Currently, because of this work, I am a reviewer member with the ASTM International/Consumer Products Safety Commission to develop a standard and warning label for portable bedrails for those over 5 years of age. We currently have bedrail standards for children but none for adults who are living in the community so this policy change is expected to provide an important contribution to improve safe care among home bound elders.

Role: Principal Investigator. This research, one of a series of four papers total, highlights the impact of Voluntary Accreditation such as Joint Commission on a variety of outcomes such as deficiency citations, quality care outcomes, patient safety culture, and rehabilitation-focused outcomes in Commission on Accreditation of Rehabilitation Facilities. This research, the first to be conducted in nursing homes, concluded that nursing homes that are accredited beyond their required state survey accreditation, consistently have better quality outcomes, patient safety culture, rehabilitation outcomes, and less deficiency citations. As a result of this research, the Joint Commission has used it as part of a large dissemination process to encourage nursing homes to seek voluntary accreditation. Furthermore, many providers who insure nursing homes are also requiring that the nursing home be Joint Commission accredited, especially in the State of Illinois.

Role: Principal Investigator. This AHRQ R03 funded research is significant as it builds on the work from my dissertation on the importance of improving adverse event reporting systems in nursing home settings. This paper, as well as others I have published are featured on the AHRQ Patient Safety Network "The Collection: Residential Facilities" bibliography. As a result of the earlier research from this study, I was awarded the 2008 Springer Award in Geriatric Nursing Research and was invited to join the National Quality Forum's Expert Panel to expand the Serious Reportable Events to Skilled Nursing Facilities. The impact of my work in this area led me to being inducted into the American Academy of Nursing in 2013.

Role: Principal Investigator. This research, in addition to my other work on examining infection control processes in nursing home settings, is significant as it highlights the importance of nursing homes having dedicated infection preventionists and their role in improving influenza vaccination and pressure ulcer prevention specifically. As a result of my work on this patient safety topic, I was invited by the Joint Commission to participate as a member of the AHRQ-funded Project Leadership Team "High Reliability Practices to Reduce Transmission of Infection in Long Term Care" (PI: Braun; R13 HS022174-01) from 2012-2014. The work over a two-year period resulted in (1) a national hearing at the Joint Commission of which I was a significant team member in developing and (2) educational module deliverable that has been reviewed by over 4,400 long term care providers (as of December 2014).

Role: Co-Investigator. This longitudinal (4 year follow-up) research study funded by the Canadian Institutes of Health Research is being conducted in Toronto, Canada at Baycrest Geriatric Healthcare where I was the Gerontological Nursing Scientist from 2005-2010 and continue to hold an adjunct faculty appointment. This is the first study of its kind to examine the potential impacts of senior volunteering on preserving physical, cognitive, and social functioning among older adults. The first paper in this project, a critical review of the literature, was published in the second highest ranking Psychology journal (2013 Impact Factor 14.392). My research team also received the 2013 Excellence through Teamwork Award at Baycrest which is regarded as the premier international centre focused on Brain Aging.

ACADEMIC LEADERSHIP Since July 2016 I assumed the position as Director of the Adult Gerontological Primary Care Nurse Practitioner program at the UCSF School of Nursing. In this position, I am responsible for leading the curricular and day-to-day activities. This includes, but not limited to: collaborating with faculty (~n=10) on courses and clinical practicum updates as well as admissions; students and faculty advisors on challenges and opportunities (~ n=65); departmental staff and administration on the fiscal health of the program, program support, and strategic direction. The position is approximately 60% effort. My plans are to continue in this leadership position until Summer 2020.
