1. Analysis was requested for the contract inorganic totalmetals fraction only.
2. Iron was reported in the field blank sample, J09X7007F/MGD056. Sample results were all greater than five times theblank level for iron, therefore no qualifications were made basedon the blank rules.
3. Antimony exceeded MES recovery control limits, but above30%. Since no positive responses were recorded in the samplesfor antimony, no qualifications were made on the sample data forantimony.
4. A performance evaluation audit was introduced as sampleJ09X7008P/MGD057. All metals were correctly identified althoughiron was quantitated at 2.5 times the spiked concentration.

3. The field blank sample, J09X7007F/MGD056, was reported tohave insignificant amounts of chloride and total alkalinitycompared with sample results. This field blank did have sulfatereported at the concentration level of the performance evaluationaudit (J09X7008P/MGD057) true value. Since sulfate was reportedas a non-detect in the audit for sulfate, and sulfate, chlorideand alkalinity are analyzed from the same sample container, andchloride and alkalinity audit results are acceptable, it appearsthe laboratory has switched the field blank and audit samples forsulfate analysis. The data were not qualified by the blank rulesbecause of the high probability of a sample mixup. All otheraudit sample parameters were correctly identified.

We have reviewed the above case. The following are our findings:
1. Analysis was requested for the contract inorganic totalmetals fraction only.
2. Iron was reported in the field blank sample, J09X7007F/MGD056. Sample results were all greater than five times theblank level for iron, therefore no qualifications were made basedon the blank rules.
3. Antimony exceeded MES recovery control limits, but above30%. Since no positive responses were recorded in the samplesfor antimony, no qualifications were made on the sample data forantimony.
4. A performance evaluation audit was introduced as sampleJ09X7008P/MGD057. All metals were correctly identified althoughiron was quantitated at 2.5 times the spiked concentration.

We have reviewed the above case. The following are our findings:
1. Analyses were requested for chloride, sulfate, nitrate,sulfide and total alkalinity. The audit sample, MGD057, was notrequested to be analyzed for sulfide.
2. Initial and continuing calibrations and calibrationverification solution analyses along with blank, duplicate andmatrix spike results were all satisfactory.
3. The field blank sample, J09X7007F/MGD056, was reported tohave insignificant amounts of chloride and total alkalinitycompared with sample results. This field blank did have sulfatereported at the concentration level of the performance evaluationaudit (J09X7008P/MG0057) true value. Since sulfate was reportedas a non-detect in the audit for sulfate, and sulfate, chlorideand alkalinity are analyzed from the same sample container, andchloride and alkalinity audit results are acceptable, it appearsthe laboratory has switched the field blank and audit samples forsulfate analysis. The data were not qualified by the blank rulesbecause of the high probability of a sample mixup. All otheraudit sample parameters were correctly identified.
