AFFIDAVIT OF UNDERTAKING

I, RONETH S. DELFIN, of legal age, Filipino, single, and a resident of Brgy. Buntay, Dulag, Leyte, after having been first sworn according to law, hereby depose and state: THAT---

1. My Birth Certificate shows my full name as RONETH SERRNO DELFIN, born 3 July 1987 at Dulag, Leyte, to parents Alfredo U. Delfin and Ramona Serrano Delfin.

2. Due to inadvertence and innocuous clerical error, my birth certificate states the maiden name of my mother (or my middle name) as SERRNO instead of SERRANO.

3. I shall undertake to have the discrepancy of the maiden name of my mother (or my middle name) corrected by the Civil Registrars Office of Dulag, Leyte, or by the court if so required, as early as possible.

4. I shall further undertake that I will submit a certified true copy of the corrected certificate of live birth on the discrepant entry of my middle name/maiden name of my mother, if and when I will apply for a renewal of my passport.

5. Meantime, I am willing to follow the registered middle name SERRNO for my passport application, pending its correction with the Local Civil Registrars Office of Dulag, Leyte, or by the court if so required.

Affiant further sayeth naught.
