AFFIDAVIT OF UNDERTAKING

I, ESTRELLA J. ABLITES, of legal age, Filipino, married and a resident of Brgy. Diit, Tacloban City, after being first sworn according to law, hereby depose and state: THAT---

1. I purchased on installment a certain HONDA TMX155T motorcycle with Plate No. 7514HA from MOTORTRADE, Tacloban City, covered by Certificate of Registration (CR) No. 6792881-4 dated June 2, 2008 and Official Receipt (OR) No. 602257865 issued on May 30, 2008.

2. Upon release of the motorcycle to us, the original CR and OR of the aforedescribed motorcycle was kept in the possession of the sellerMOTORTRADE.

3. At present, the undersigned is in need of the original copies of the CR and OR in relation to her application for change of unit before the Permits and Licenses Division of the Office of the City Mayor of Tacloban City; thus, she needs to borrow the said original documents from MOTORTRADE to authenticate and compare photocopies with the said originals before the latters office.

4. Upon being allowed to borrow the original copies of said CR and OR, I hereby undertake to return the same to MOTORTRADE within a reasonable period of time.

5. I execute this affidavit to attest to the truth of the forgoing facts, and to comply with aforesaid undertaking in good faith.
