We are so excited that you have chosen to be a part of the Roar for Wellness Rewards Program. Use this program to help you reach your health and fitness goals this academic year and beyond. It is our goal that we equip you with the tools, resources, and encouragement that you will need to continue your wellness journey.

Throughout this program we will embark upon many new

Town & Country Supermarket

The US HealthCenter Wellness Portal is a tool to aid you on your wellness journey. Your Personal Health Dashboard (PHD) is located in the portal which is used to record points and access information regarding the challenges and activities offered throughout the program. All activities and wellness. Challenges must be logged through the US Health Center portal for points to be recorded. The PHD is available to you 24 hours a day and is accessed by your own personal username and password.

Remember: You will receive an email from USHC to register for wellness challenges, it is essential

that you understand how to use the portal to track challenges and activities.

Be sure to practice logging in with your username and password prior to the USHC training session.

Please call customer support at 1-877-322-7398, ext. 2 if you have issues logging in; the session will not be a resolution of usernames and password issues.

Core Components Description 1) Biometric Screening Pre-Screening (Registration Required) September 14-18 200 points

The screening components include a glucose and lipid panel as well as measurements for height, weight, blood pressure, and self-reported tobacco use.

UNA Biometric Screening • September 14 - 18 from 7:30am – 12pm @ Human Performance Lab located in

Flowers Hall, Rm 401 • Register at una.edu/wellness > Roar for Wellness Current Enrollees > Sign-up

Sheet • A 12-hour fast is required prior to your screening. You may drink water but no

other food or drink please. 2) Online Health Risk Assessment Pre-Assessment September 23 – Oct 2 200 points

Complete the online HRA after your lab results are in the system. Completing the HRA takes about 20 minutes. The Health Risk Assessment is an online questionnaire that will assess your physical/emotional health, stress, exercise, eating habits, alcohol use, safety, readiness to change, preventive health care, medical history, cardiac risks and cancer warning signs. To Complete the HRA

• Log on to the PHD between September 23 – October 23 > Click on Assessment > Health Risk Assessment.

• Complete all 8 sections until your receive a “Congratulations” message. Once you have completed your assessment, you will be able to view all your results online. To print your personalized report, click on Medical Records > Health Assessment Report.

3) HRA Review October/November 100 points

This is a confidential 20-minute telephonic appointment to review your screening and HRA results with a Health Coach. To Register for Your Appointment

• Log on to www.ushealthcenter.com > Click on Health Counts > select HRA Review Registration > Choose date and click Sign Up > Under Time Slot click the drop-down menu and select your desired time and enter phone number.

Post - Assessment Spring 2021

There will be a screening, HRA, and HRA Review offered in the spring as a post-assessment. More details to follow.

Activities Description

Educational Sessions 50 points per session

Wellness Wednesdays will be virtual education sessions. Completion is tracked via an event attendance online. The theme for this series is strengthening your immune system

Minutes Activity Tracker September 28 – April 11 25 to 50 points

Achieve your physical activity minutes goal by getting a specific number of physical activity minutes per week (Monday through Sunday). This will require tracking via the USHC portal. Attend the USHC session for more information.

• 25 points earned each week you get 150 – 199 minutes of exercise • 50 points earned each week you get 200 or more minutes of exercise

Important Note for Tracking Walking as Exercise

Walking for exercise should be tracked via steps OR minutes. It should not be tracked both ways. Please decide how you will track walking and use one method only.

Step Tracker September 28 – April 11 25 to 50 points

Achieve your step goal by getting a specific number of steps per week (Monday through Sunday). This will require tracking via the USHC portal. Attend the USHC session for more information.

• 25 points earned each week you get 50,000 to 69,999 steps • 50 points earned each week you get 70,000 or more steps

Roar Partners September 28 – April 11 50 Points

Having support and someone you are accountable to can make all the difference in being successful in living a healthy lifestyle. Each month enter a self-report on ways you supported your roar partner. Go to Health Counts > Roar Partners > Select Month Completed > Record > Enter the name of your Roar Partner and a few words about how you supported them > Record. Enter within 15 days of the following month. For example, self-record September completion by October 15. NOTE: Your Roar Partner must be the same person throughout the duration of the program to be eligible for the Roar Partner Award.

Stress/Exercise Challenge October 1 – October 18 50 Points

Enjoy the benefits of stress reducing exercise. Based on your experience level, aim to get 150 – 200 minutes of exercise per week. Program materials with details will be released separately.

Nutrition Challenge October 19 – Nov 1 25 points

Aim to incorporate more immune boosting foods into a day. Program materials with details will be released separately from USHC.

Sleep Challenge November 2 - 15 50 points

Track your sleeping habits aimed at getting a good night’s rest. Program materials with details will be released separately from USHC.

Spring Challenges More challenges will be offered in the spring. Program materials with details will be released separately from USHC.

Health Coaching 50 points

After completion of the HRA Review, participants are eligible for up to two additional coaching sessions. The Health Coach is a certified health coach, RN or licensed health professional able to interpret results, assist with referrals, and facilitate development of goals. Information discussed with the coach is confidential. You will have the opportunity to work on lifestyle or disease management goals that are important to you.

Flu Shot 50 points

Flu shots are critical this year as a preventative measure so as not to add to the current COVID-19 pandemic.

• Self-report your completion of your flu shot on your PHD by selecting Health Counts > Healthy UNA Activities > “I completed a flu shot during the fall of 2020/21” > Record

Biometric Points You are eligible for these points twice (once per screening) 15 to 25 points

You will receive a WellCentive score (viewable on your PHD) that is based on the result of your biometric screening. You will receive points as follows for results in the categories below: WellCentive Score % Excellent (92-100%) = 25 points Good (75-91.9%) = 20 points Fair (65 – 74.9 %) = 15 points HDL 60+ = 15 points Triglycerides under 150 = 15 points LDL under 100 = 15 points Glucose 60 – 100 = 15 points

Strengthening our immune system during this time of the Covid-19 pandemic is more important than ever. Wellness Wednesdays is a series of immune

strengthening sessions on a variety of topics. All sessions will take place live via zoom. For the first time ever, we will record each session and add to our

website at una.edu/wellnesswednesdays *You must register for each session to receive a zoom link for access. **Only live zoom session attendees receive points for Wellness Wednesday sessions.

As a full time faculty and staff member you have free access to the UNA Recreation Center (UNARec), valued at $120 per semester. Amenities: A gymnasium facility including three multi-purpose courts and a

walking/jogging track A fitness center with work out areas dedicated to cardio machines and free

page Locker rooms with full-sized lockers and showers (bring your own lock) Access to the Outdoor Adventure Center (OAC), which offers various
