INTRODUCTION:-Good morning everybody, I hope you people are well known me, but still I will introduce myself to you. I am K.SELVI Studying Post Basic B.Sc. Nursing Final Year.

When I was posted in the ward I met a patient with the complaints of confusion, drowsiness, disorientation, rapid heartbeat.

So, if these symptoms are found to the patient what will be the diagnosis it is UNCONSIOUS.

Let me discuss in detail about UNCONSIOUS.

Maintain a patient airway trachestomy and oral or nasal intubation. Maintain adequate oxygen therapy and mechanical ventilator. Will be maintain the airway patternTo promote airway clearance and pulmunarate by the patient position.

1. To assess the respiratory rate, rhythm, effort, oxygen level and breath sound.

2. To give the frequent oral care every 2-4 hrs.

3. To maintain the position such as upright position or semifowler position.

4. To administer the oxygen as per DR order.It helps to breahe the normal pattern.

Avoid over sedation because it may after respiration which increase the intracranial pressure in patient level of consciousness.

Perennial care should be performed at least every 8 hrs and after every episode of incontinence.
