UTILITY QUALITY LEVEL INFORMATION INDEX:QUALITY LEVEL D: “QL D”: Records Search: This level of service provides information that has been obtained from existingrecords, which includes utilizing existing plans, contacting utility companies, verbal recollections, obtaining as-built plansand researching prior rights (i.e. easements) issues.
QUALITY LEVEL C: “QL C”: This level adds field surveys of visible above-ground utility systems such as maintenance holes,valve boxes, fire hydrants, etc., and reconciles this information to existing utility records (Quality Level D)
QUALITY LEVEL B: “QL B”: Designating Underground Utility Systems: This level includes using surface geophysicalprospecting techniques, including electromagnetic, magnetic, sonic, or other energy fields to determine the existence,horizontal position of underground utility systems. approximate vertical positions of subsurface utilities can be identifieddepending on the specific subsurface prospecting technique utilized.
QUALITY LEVEL A: “QL A”: Locating Underground Utility Systems: This level is the highest level of accuracy, which includesobtaining precise horizontal and vertical position of the utility line by excavating a test hole. The test holes shall be doneusing vacuum excavation or comparable nondestructive equipment in a manner as to cause no damage to the utilityline. After excavating a test hole, a field survey was performed to determine the exact location and position of theutility line, type, size, condition, material, and other characteristics of underground utility system.

EXISTING COMPOSITE UTILITY PLAN - QL-B1. This plan was prepared in conformance with ASCE standard CI/ASCE 38-02 “American Society of Civil EngineersStandard Guideline for the Collection and Depiction of Existing Subsurface Utility Data”.
2. WE will only warrant the three dimensional location of utility lines based on quality level A information. QL “A”information is only valid within the visible limits of the test hole.
3. WE will advance a vacuum test hole until a condition of practical refusal for vacuum excavation is reached.Practical refusal is defined as any of the following conditions occurring: bedrock, water table or reaching adepth of 12'.
4. Below ground structures, unless dimensioned, are symbolic only.
5. Prior to excavation utility owners shall review and approve this drawing.
6. This drawing is prepared in color. Reproduction may alter the information contained herein. WE do not warrantthe information contained herein in the event of reproduction.
7. Base mapping/survey control is shown for information only and is not warranted.
8. WE recommend that all existing utilities critical to proposed construction activities be located by vacuumexcavation (quality level A) prior to any excavation (if not already located by vacuum excavation).
9. Utility Lines shown on the plans labeled as Quality Level B have been traced and surveyed using variousgeophysical instruments. Unless noted as Quality Level B it should be assumed that lines are shown asapproximate only.
10. Utility Lines depicted on the plan as ASCE Quality Level D are from various plans and records from public or privatestakeholders. WE requested that all available plans and records be provided from all of the known stakeholders.
11. WE obtained numerous One Call design tickets for this project and the locations of all markouts have beensurveyed and documented.
12. Electrical lines shown on the plans were traced using hand held locators by indirect induction methods. The linestraced are depicted as ASCE 38-02 Quality Level B.
13. Telephone and communication lines shown on the plans were traced using direct and indirect connectionsthrough building basements, vaults, and hand holes. The lines traced are depicted as ASCE 38-02 Quality Level B.
14. Water lines were traced using direct and indirect induction methods. The lines traced are depicted as ASCE 38-02Quality Level B.
15. Underground structural elements such as tie backs, sheeting, shoring, framing, piles or pile caps have not beenplotted within this set. The respective public and private plans should be reviewed prior to demolition andconstruction in order to identify underground structural elements.
16. Utilities located within fenced in electrical sub-stations are not accessible by field personnel, therefore can not befield located.
17. Utility vault structures are shown using best available information collected at the time of thesurvey/investigation. However, data may be limited by accessibility and measurement accuracies. All below gradeutility vaults structures shown hereon should be considered approximate and are depicted as ASCE 38-02 QualityLevel C.
18. (**) Denotes a portion of utility alignment that has been located via alternate geophysical locating methods(acoustic pipe locator and/or ground penetrating radar). When design or construction is affected by the portrayedlocation of this alignment, additional potholing investigation is recommended.
19. Any and all depth of utilities depicted at a QL-B standard of care hereon shall be considered approximate as of thedate of the investigation and shall not be used as a basis of design, constructability analysis, or constructionmethodology without further investigation to expose the utility in question and obtain a hard measurement. It isthe responsibility of the contractor to utilize soft digging best practices to expose all utilities having a potential forconflict within the area of excavation prior to any hard digging excavation. McCarthy shall not be held liable forany decisions of incidents that result from the reliance upon any of the QL-B provided depth information.
