2. I/We choose the following payment plan:

 ______ One-time full tuition payment—must be paid by July 15, 2012

 ______ Monthly payments--July 2012 through April 2013 using the FACTS Program.FACTS will deduct their $38 administrative fee from the account you specify.

3. The participating parishes (Guardian Angels, St. Victoria, St. Nicholas) offer a grant for students attendingGuardian Angels School whose families are “Active Parishioners” of their parishes as defined in the GuardianAngels School Handbook. Guardian Angels parishioners also need to complete the included application for theparishioner grant. 

4. Guardian Angels School sponsors several major fundraisers throughout the year: AngelFest in cooperation withthe Parish, Rake-A-Thon, and Spring Fling, along with an Annual Fund Drive. The school’s operating budget isdependent upon the success of these events. I/We understand that Guardian Angels School asks that eachfamily participate to the fullest extent possible.

5. Guardian Angels School relies heavily upon parent volunteers. Volunteering allows the school the keep expenseslower. I/We understand that each family is asked to volunteer 25 hours of their time and talent for the benefit ofGuardian Angels School.

6. The student(s) and parent(s) listed above are responsible for meeting the requirements outlined in the Guardian

Angels School Handbook.

7. Refunds will be prorated on a monthly basis without interest based on the date of withdrawal from GuardianAngels School. Please refer to the Guardian Angels School Handbook for full details regarding this policy.

8. Non-Sufficient Funds Policy (NSF) – FACTS tuition payments received with Insufficient Funds will be charged a$30 handling fee plus any applicable bank fees.

I /We agree to the terms of this Family Investment Contract. I/We understand that this Family Investment Contract isbinding and that I/we are responsible for its contents. Payment of the registration fee(s) and a signature on the FamilyInvestment Contract will complete the registration process. Please make checks payable to Guardian Angels School.
