Executive summary The Trust missed the 4 hour A&E NHSI standard in April with NHSI performance recorded at 91.24%. This was a 1.7% improvement on last month (89.56%) and 1.2% above the agreed recovery trajectory of 90% for Q1. Attendances & admissions were marginally higher than April 2016. However (a) deficient social and community care capacity (b) provider changes (c) surges in admissions & (d) reduced staff availability due to IR35 tax avoidance changes, created slow flow from the Emergency Department to the wards. Total attendances in April (at 7,911) were 5.3% lower than March’s attendances (8,355) although 0.2% higher than April 2016 (7,898). Admissions during April (at 1,908) were 4.3% lower than March’s admissions (1,994) although 0.3% higher than April 2016 (1,902). The Trust continues with a risk of aggregate RTT non-compliance if; (a) non-elective bed pressures require significant elective cancellations, & (b) if capacity or demand management schemes cannot address the significant increase being seen in Dermatology, Neurology & Endoscopy demand. The Trust is expected to report compliance for 6 of 7 Cancer standards for April;  The failing standard was; (1) 31 Day Subsequent Surgery; 1 breach due to substantial & ongoing Dermatology demand outstripping capacity.

Recommendation: Review the paper and discuss the contents seeking additional assurance as necessary.

The charts in Appendix B identify a number of daily snapshots of system indicators where activity has continued to exceed expectations identified by the CCG to support flow within ED & the Trust’s hospitals. In line with recent NHSI/NHSE communication, the CCG‐led Local A&E Delivery Board are undertaking a full review of system commissioning requirements to achieve the hospital flow required to deliver the A&E 4hr & DTOC targets.

The 60‐74yr and 75+ age groups are the highest FY2017/18 increases at 5.8% and 1.9% respectively, with an overall 0.3% (6 patients) increase in admissions compared to last FY.

Comparing FY2017 A&E attendances (7,911) to FY2015 (7,630) & FY2016 (7,898) confirms an ongoing increase in demand attending the emergency department, with a corresponding increase also being seen in the number of admissions via A&E FY2017 (1,908) versus FY2015 (1,847) and FY2016 (1,902).

Ambulance Handover Delays 

When the hospital experiences patient flow difficulties A&E can become congested which causes delays in handing patients over from the care of ambulance crews to that of the Accident and Emergency Department. During April 2017 the Trust recorded 104 incidents where the handover took between 30 and 60 minutes, and 3 waits in excess of 60 minutes. This was a 30% reduction compared to April 2016.

Performance: 

Elective Procedure Cancellations due to Non‐Elective Demand

The Trust had to postpone 5 elective procedures during April due to no beds being available.

In accordance with the NHSI requirement, all non‐urgent surgery at the St Peters Hospital site was  suspended 19th December 2016 to 15th January 2017 to provide additional bed stock to cater for the non‐elective winter demand.

 

Overnight Waits in the Accident and Emergency Department 

Although the Trust has seen a substantial improvement in the non‐elective length of stay from an average of 7.25 days during April 2015 down to 5.89 days for April, the  level of admissions continues to cause patient flow difficulties through the hospital creating a backlog within A&E. This is evidenced by the number of patients waiting in A&E overnight with a decision to admit (DTA).  

During April the Trust recorded DTA patients waiting in A&E overnight on 17 days. On 9 days in April, 5 or more patients were waiting to be transferred (average 10 on these days).

Performance: 

Delayed Discharges

Overall the combined DTOC & MFFD delays showed an increasing trend over Q4 although have reduced during Q1.

A number of initiatives have been underway which include;•  Inreach support ‐ dedicated support in wards to assist with case managing patients throughout their hospital stay as well as facilitating discharge.  • Board round improvements ‐  to improve the quality and content of board rounds on medical wards.• Supporting community hospitals ‐ to reduce their length of stay in rehab beds, creating additional capacity.• Discharge to assess (D2A) started on 1st September ‐ creating system support to enable a reduction in delayed discharges.• A CCG led system provider reboot week  w/c 6th February 2017 to reduce delayed transfers of care to support discharges (& patient flow) at Community Hospitals & at the St Peters Hospital site.

A&E Performance (National Position)The Trust's A&E performance compared to other Trusts is included as Appendix A.  ‐ When comparing the Trust including Ashford WiC (NHSI position), for March  (the most recent data published), the Trust is positioned 68th of 139 Trusts.   ‐ During March only 26 Trusts reported a compliant performance greater than 95%, with 118 Trusts reporting an improvement, 20 Trusts reporting a decline, and 1 remaining the same.

Urgent Care Improvement ProgrammeWe have in place an Urgent Care Improvement Programme which includes changes to our governance and clinical engagement. The improvement plan has now been revised following publication of the Rapid Implementation Guidance and reflects key improvement actions. The three key programmes of work are outlined below;

1. ED Streaming At The Front DoorThe objective is to create and run pathways that de‐compress the Emergency Department by streaming patients to other in‐patient services such as assessment areas or preventing attendance through telephone management.

2. Patient FlowThis objective is aimed at improving and ensuring the safe and effective flow of patients into the body of the hospital from ED at the point of referral to beyond discharge from the acute hospital.

3. Improving Discharge ProcessesThe key targets for this objective are to reduce in‐patient delays, overall Lengths of Stay (LoS) and the volumes of and bed days lost to Medically Fit For Discharge Patients.

Performance: 

TWR performance position for April is recorded as compliant at 93.7% with 26 of the 57 breaches (45.6%) due to patient choice.

During April the Trust has again received a high number of cancer TWR referrals at 1064 (inc Breast Symptomatic), creating additional demand on cancer capacity & diagnostic services.

The Trust is working closely with NWS CCG to understand the reasons for the increase & ensure satisfactory capacity remains available to meet the increase being seen in patient demand.

CANCER UPDATE

Performance: 

The Trust has recorded a compliant performance for April at 87.29%, with delays recorded due to complex pathways (9 patients), patient choice (2 patients), & patient fitness (1 patient). These breaches are undergoing clinical validation by the tumour group leads.

The Trust with tertiary partners are undertaking a shadow performance monitoring excercise aimed at assessing the impact of the proposed 38 day tertiary referral guidance and the ASPH Cancer Timed Pathway Analysis to review the effectiveness of the outcomes of the existing action plan and to enhance improvement activities to address current areas of concern.

The Trust continues to deliver its Cancer Improvement Action Plan in conjunction with NWS CCG which aims to address recent issues regarding cancer performance.

Diagnostic Tests 

The Trust has been compliant with the DM01 diagnostic standard since September 2015, with April performance also compliant, recorded at 99.1% (most breaches were due to capacity reduction in endoscopy services over the easter period & increasing demand).

RTT Forward View

The Trust remained compliant for April recorded at 92.3%, although 5 specialties; General Surgery, Urology, Oral & MaxFacs, Plastic Surgery, & Dermatology were non‐compliant at specialty level.

Specific specialty issues have been;

General Surgery ‐ ongoing capacity issues in colorectal surgery due to the suspension of non‐urgent surgery over the festive period & financial constraints during Q4.Urology ‐ capacity issues due to (a) gaps in consultant staffing due to sickness and recruitment timelines (b) during the festive period due to the suspension of non‐urgent surgery, (c) bank holiday downtime, & (d) urgent and emergency care pressures.Oral & MaxFacs ‐ capacity issues due to a lack of surgeon cover, alternative arrangements to address the capacity loss are underway.Plastic Surgery ‐ the specialty had a PTL volume of 15 at the end of the month (with a limited breach tolerance of only 1 pathway). Additional capacity is being agreed to enable the specialty to recover to compliance.Dermatology ‐ significant capacity issues due to a substantial & significant increase in both TWR and routine demand.Neurology ‐ capacity issues due to an increase in routine demand being diverted by Guildford & Waverley CCG away from RSCH.Endoscopy ‐ capacity issues due to an ongoing increase in TWR demand. The new endoscopy suite is due to be operational from July 2017 providing additional capacity.

A&E

A&E NHSI Performance was recorded at 91.2% for April. This was a 1.7% improvement

on March and 1.2% above the agreed recovery trajectory of 90% for Q1.

Attendances & admissions were marginally higher than April 2016, although; (a)

deficient social and community care capacity (b) provider changes (c) surges in

admissions & (d) reduced staff availability due to IR35 tax avoidance changes, created

slow flow from the Emergency Department to the wards.

We have in place an Urgent Care Improvement Programme which includes changes

following publication of the Rapid Implementation Guidance.

The Trust was placed 68th of 139 Trusts for A&E performance (Mar 2017 - latest data).

Trust performance for Mar = 89.6% [National performance = 90.0%].
