Trigeminal neuralgia (TN, or TGN), also known

as prosopalgia, the suicide disease, or Fothergill's

disease is a neuropathic disorder characterized by

episodes of intense pain in the face, originating

from the trigeminal nerve. The clinical association

between TN and hemi facial spasm is the so

called tic douloureux. It has been described as among

the most painful conditions known to mankind.

About Trigeminal nerve:

The trigeminal nerve is a

paired cranial nerve that has

three major branches:

the ophthalmic nerve (V1),

the maxillary nerve (V2), and

the mandibular nerve (V3).

One, two, or all three

branches of the nerve may be affected.

V1 provides sensation to the forehead,

upper eyelids and eyes.

V2 provides sensation to the lower eyelids,

cheeks, nostrils, upper lip and gums.

V3 provides sensation to the lower lip and

gums and lower jaw. It also controls some

muscles of chewing.

Pathology:

When a patient's Trigeminal nerve was studied

under electron microscope during an acute phase

of TN, it was revealed that the abnormality existed

at the level of the inner nerve fibers (called axons)

which carry nerve sensation, as well as the outer

lining covering the nerve fibers (called myelin

sheath). Due to the damage to such delicate parts,

the nerve fibre behaves like a live electrical wire

with open ends, leading to pain similar to

electric shock, triggered by touch or jerk.

Sign and Symptoms:

Depending on which part of the nerve is affected

(V1, V2 or V3) the symptoms will have the

respective distribution.

The face pain is

usually one sided.

The pain is

usually limited to

one division of the trigeminal nerve (V1, V2

or V3).

Pain is severe, excruciating, shooting,

stabbing or lightening in nature, often

driving the patient to

suicidal thoughts.

The pain is often

described by the

patient as if a part of the

face is being drilled with a machine

or as if an electric wire is touching

inside of the face or as if the face or eye is

getting nailed.

The pain may last few seconds to minutes

or even longer.

Patient is usually pain-free in between the

attacks of TN.

Pain can be triggered by eating, drinking,

touching the face, talking, shaving,

brushing, blowing, kissing, applying make-

up, etc.

Causes:

The exact cause of Trigeminal Neuralgia is not

clearly understood but there are certain factors

that can trigger the onset of Trigeminal

Neuralgia:

Genetic due an inborn pattern of blood vessel

formation

A blood vessel pressing on the trigeminal

nerve

Nerve impairment following a trauma

Tongue piercing

Trigeminal Neuralgia

Post viral infections like herpes

Post bacterial infections like leptospirosis,

syphilis, etc.

Disorders like multiple sclerosis that damage

the myelin sheath (nerve covering which serves

as an insulator)

Stroke impacting lower part of the brain.

Compression of the nerve by a tumor or cyst

Enduring stress

Habitual intoxication from alcohol, lead and

other drugs

Brushing teeth, kissing, eating, drinking,

smiling, crying, talking, putting makeup,

shaving, washing the face, slight breeze, dental

work can stimulate the pain

Diagnosis:

If the doctor believes the symptoms indicate TN

the patient’s face will be examined more carefully

to determine exactly which parts are affected. The

doctor will also attempt to eliminate other

condition which sometimes have similar symptoms

such as tooth decay, a tumor, or sinusitis.

MRI – magnetic resonance imaging scan – it can

help to rule out other problem. It is very difficult

to see the blood vessel nect to the nerve root,

even on high quality MRI.

Conventional treatment:

This commonly includes prescription of drugs

like Carbamazepine, gabapentin, phenytoin as well

as some other anti- epileptic drugs. Surgical

options for TN include peripheral nerve blocks,

Gasserian ganglion ablative procedures,

microvascular decompression, radiosurgery

(Gamma Knife), cryosurgery, etc.
