Reactions 1442 - 9 Mar 2013 S Triamcinolone Malignant melanoma in an elderly patient: case report A 68-year-old man developed a malignant melanoma at the site of triamcinolone injection for keloid scarring. The man had a history of a malignant melanoma on his right upper back treated with wide local excision. Five years later, a dermatologist observed a lump at the site of the melanoma removal, consistent with keloid. A one-time dose of 0.5–1.0cc of triamcinolone 6 mg/cc was injected subcutaneously at the site. Over the next 2 months, the region steadily grew, and a subsequent biopsy revealed a 3.5mm residual malignant melanoma with angiolymphatic invasion. There was no evidence of metastatic disease found on CT scan. The man was treated with wide local excision and followed closely with frequent visits. He developed metastatic disease to the bones, lungs and musculature over the next 4 years, which responded well to radiation, chemotherapy and immunotherapy. Author comment: "The melanoma was detected in the exact cutaneous area of steroid administration, suggesting a relationship exists between steroid use and the initiation and progression of melanoma". Malignant melanoma in an elderly patient: casereport
