    ARRIVAL & PARKING: The UC Davis Orthopaedic Clinic is

    located in the Lawrence J. Ellison Ambulatory Care Center,4860 Y Street, Suite 1700, Sacramento, CA 95817. Please

    see the map on the back page for directions. Parking is

    available in the adjacent parking structure. The parkingfee is $2.00 with a validated parking ticket. Pleasebring your ticket to the clinic for validation.

    BILLING INFORMATION: Our Registration staff will need tophotocopy your medical insurance information. Please providethem with:

    An insurance identication card and any co-payment

    amounts required by your insurance company

    A referral form from your referring physician (if applicable)A Medicare card, if Medicare is your insurance

    The name, telephone number, and address of your emergency

    contact person

    The name of the person nancially responsible for your

    medical bills

    Please Also Remember to Bring the FollowingX-rays, MRIs, Bone Scans, EMG results, and all other importantmedical records pertaining to your diagnosis

    In order for your physician to properly diagnose and treat yourmedical condition, there may be the need to obtain x-rays.This can add additional time to your visit. The average visit toUC Davis Orthopaedics is approximately 2 hours. Please plan

    to arrive 15 minutes prior to your appointment time tofnalize your paperwork.

    What is a teaching hospital?

    A teaching hospital or academic medical center is a hospitalthat is associated with a major medical school such as UC Davis.Teaching hospitals train medical students, nursing students,

    residents and others to become skilled medical professionals.

    When you come to UC Davis Orthopaedics for an appointment,you may be examined initially by a resident. After theexamination, the resident will present the clinical data tothe attending physician, who will then examine you andreview the studies available. Your care will always be anchoredby one of our many experienced, board-certied physicians.

    From the north (Sacramento Metro Airport)Take I-5 south to Business 80/Capital City Freeway east (Reno). Follow Business

    80/Capital City Freeway east to Highway 50 (Placerville). Take the 34th

    Street exit and turn left on 34th Street. Turn right onto T Street. Turn right

    onto Stockton Blvd. Continue three blocks and turn left on X Street. At theroundabout, turn right onto 48th Street. Turn left onto Y Street and enter

    the parking structure next to the building.

    From the south (Stockton, Los Angeles)Follow Highway 99 north to Business 80/Capital City Freeway east (Reno).

    Exit at T Street. Turn right on T Street, then right onto Stockton Blvd. Continue

    three blocks and turn left on X Street. At the roundabout turn right onto

    48th Street. Turn left onto Y Street and enter the parking structure next to

    the building.

    From the east (Placerville)Take Highway 50 to the Stockton Blvd. Exit. Turn left onto Stockton Blvd.

    Continue 5 blocks and turn left onto X Street. At the roundabout turn right

    onto 48th Street. Turn left onto Y Street and enter the parking structure

    next to the building.

    From the west (Davis, San Francisco)

    Take Business 80/Capital City Freeway east (Reno) to Highway 50 (Placerville).Take the 34th Street exit and turn left onto 34th Street. Turn right onto T
