Our ServicesMemorial Transitions offers customized services that address the specific needs and goals of each client. Clients can receive one-time or ongoing phone support and visits at their home or other place of residence. Services are provided by the Transitions Program Coordinator and trained volunteers.

The Transitions Program Coordinator helps clients and caregivers:

• Discuss feelings related to the life-limiting illness.

• Clarify goals so that care decisions can reflect what is most important to clients and their families.

• Plan for assistance that may be needed now or in the future.

• Access community resources that offer practical health care services and emotional/spiritual support

• Nurture important relationships

Transitions Volunteers can provide:• Companionship and emotional support

• Help reading and writing correspondence

• Running errands

• Food preparation

• Light housekeeping

• Respite breaks for family

• Limited transportation

Who is eligible

for theMemorial Transitions Program?

Any adult with a progressive life-limiting illness who resides in Yakima County may be eligible for Transitions. This includes those who currently receive other in-home services (such as respite, COPES or home health). Prospective clients and their physicians consult with the Transitions Program Coordinator to make a final determination of eligibility.HelpMemorial

Memorial Transitions Program offers comfort-focused services to adults with life-limiting illnesses and their families. We provide information, resources and support to help clients plan for anticipated life changes and make informed decisions about their care.

Why Transitions?It’s difficult living with a life-limiting illness. Transitions supports clients and families as they face medical challenges and other life changes. We help clients to identify needs and obtain assistance as necessary. By encouraging clients to define what “living well” means to them, Transitions enables clients and their loved ones to retain a sense of control over their lives, even in the face of a progressive illness.
