The Declaration is a legally admissible form where you can share the truth of how abortion hurt you. Please read through these instructions to guide you through filling out the form:

Fill in the state and country where you currently reside

Fill in your name (if you check the box to use your initials or first name only, we will redact or conceal everything but that portion of your name before filing with courts or legislatures.) You do need to be over the age of 18 years to fill out this form. If you are under the age of 18, please contact Operation Outcry.

1) Put what you remember. If you can’t remember exactly when that is OK.

2) Enter how many weeks pregnant you were if you remember and if you know what kind of procedure (i.e. vacuum suction, D & C etc.)

3) Answer the rest of the questions with what you remember. You may attach additional pages if there is not enough room on this form.

4) When you sign this form, you sign it like you sign your tax return “I declare under penalty of perjury under the laws of the United States of America that the foregoing is true and correct.” There are penalties for being dishonest. Tell the truth as you remember it.

5) Check off whether to use your full name, first name or initials only. (It is very important to check one of those boxes) Then sign your full name. We will redact or conceal that portion of your name that you request be concealed.

6) Check the box whether we may contact you or do not contact. If you do not check a box, we will assume it is OK to contact you. Most contact is through email. You will then have an option to opt out of our email notifications.

7) Completely fill out the personal information box. You must fill this information out so we know that you are a real person. This is a legally admissible document. The contact information will NOT be released.

8) When you have finished filling out the form, mail it in to the The Justice Foundation , 8122 Datapoint, Suite 812, San Antonio, Texas 78229. If you have any other questions you can contact Operation Outcry at 1-866-4OUTCRY (1-866-468-8279)

Please note that once your testimony has been filed with a court or legislature, it becomes public record. We will protect your identity according to your wishes, but your testimony could be read by others. By signing this form, you are giving permission for your legally admissible testimony to be released.
