ATTACHMENT A Terms of Reference for District Extender 1. Background The MI wishes to continue its technical and financial support to the VAS program in 6 selected provinces in Indonesia. MI also plans to replicate the elements of the processes which have worked to improve coverage and utilization of zinc and ORS in the treatment of childhood diarrhoea and IFA supplementation programs for pregnant women in the West Java Province. The district extender will work closely with the Provincial Extender, the Child Survival Program Coordinator, and Senior Program Officer - Maternal, Adolescent and Child Health, to strengthen the district health office in delivering on the VAS, zinc and ORS in the treatment of childhood diarrhoea, and IFA supplementation programs, and support the provincial health office in providing the technical assistance with focus on the low performing districts within the province. 2. Consultant’s Role Planning o Facilitate problem identification at the district level. Facilitate planning with DHO to improve VAS, zinc and ORS in the treatment of childhood diarrhea, and IFA supplementation coverage particularly in low performing districts to develop an action plan which clearly states what needs to fixed, by when and who will be responsible for the task. Deliverable: Plan of action for DHO for improving VAS, zinc and ORS in the treatment of childhood diarrhoea, and IFA supplementation coverage in low performing districts is developed by first quarter of the year and evaluated annually at the end of the year. Capacity building o Build capacity of DHO at district levels to analyze and take corrective actions on coverage data, particularly in low performing districts within the province. o Build capacity on the standardized processes for supply chain, micro planning, monitoring and reporting of the VAS, zinc and ORS in the treatment of childhood diarrhea, and IFA supplementation programs. o Support DHO to train the health center staff during program review meetings and workshops. Deliverables: i. Training of the trainers (ToT) at the DHO level to train health center staff rolled out. ii. DHO staff has adequate understanding and knowledge of key program processes for the VAS, zinc and ORS in the treatment of childhood diarrhoea, and IFA supplementation programs. Supply chain o Liaise with the pharmacist health service in the MoH and highlight the need to develop link between the PHO and DHO to monitor the Logistic and Supply Chain Management in each district so that that vitamin A, zinc, ORS and IFA supplements supply are delivered in a timely manner and is adequate. Deliverables: i. District plans for the procurement of an adequate supply of Vitamin A, zinc and ORS, and IFA supplements will be developed. ii. o Monitor the VAC distribution every 6 months (February and August) to ensure that adequate number of the VACs are timely supplied and identify gaps and areas of support in identified districts within the province.

o Monitor all components of zinc and ORS in the treatment of childhood diarrhoea, IFA supplementation program in identified districts within the province and to ensure that IFA supplements are delivered on time and in adequate number, and identify gaps and areas of support.

o Support streamlining of VAS reporting, zinc and ORS in the treatment of childhood diarrhoea, and IFA supplementation program HMIS reporting from districts to province, data compilation and analysis.

o Support the implementation of QRS of VAS program by DHO and ensure the data quality assurance.

o Travel to districts as required.

Deliverables: i. Established reporting system, data compilation and analysis from health center to district in

each round (February and August) for VAS program, zinc and ORS in the treatment of childhood diarrhoea, and IFA program’s monthly reports which include HMIS and the monitoring checklist to track progress and share with the Health System Coordinator and MI Director.

ii. Use of available data and QRS tools for program decision making is undertaken. iii. Identify gaps and propose solutions during field visits.

• Advocacy and Liaison o Facilitate and support the advocacy meeting/activities to increase budget allocation for VAS,

zinc and ORSin the treatment of childhood diarrhoea, and IFA supplementation programs at the district level.

o Facilitate organization of technical or programmatic workshops at the district level. o Work closely with key players at district levels including UNICEF, other UN/NGOS,

Universities and relevant tasks forces or committees and act as resource person for the VAS, zinc and ORS in the treatment of childhood diarrhoea, and IFA supplementation programs.

o Travel to districts and province as required. Deliverables: i. Budget for VAS, zinc and ORS, and IFA supplementation program in the district from /local

district government is adequately allocated for program components including procurement of Vitamin A capsules, zinc, ORS, and IFA tablets, VAS and IFA supplementation program training, home visit (sweeping) activities for VAS and regular monitoring from DHO to visit health centers for both programs.

ii. Good collaboration and networking with other partners and MI personnel recognized as resource person in VAS, zinc and ORS in the treatment of childhood diarrhoea, and IFA supplementation programs at the district level.
